BACKGROUND: To establish diagnostic/assessment criteria for awake bruxism (AB) using electromyography of the masseter muscle during the diurnal awake state (d-EMG), it is necessary to refer to the actual state of variation of d-EMG waveforms between different days (d-variation). OBJECTIVE: This study aimed to clarify the actual state of d-variation in various indices of d-EMG waveforms. METHODS: The participants included 91 volunteers with or without awareness of diurnal tooth clenching. Measurements were conducted on three days within a two-week period using a wearable EMG device set at the unilateral masseteric region. A waveform with an amplitude more than twice the baseline and a duration of more than 0.25 s was defined as a burst. The number of bursts/h, number of episodes/h, number of bursts ≥ 20% maximum voluntary clenching (MVC)/h, burst integral value/h, and standardised burst integral value/h were calculated. The coefficient of variation (CV) of the three measurement days was calculated for each index. RESULTS: The five indices showed no significant differences among the three days. The median CV for the number of episodes/h, number of bursts/h, number of bursts ≥ 20% MVC/h, burst integral value/h, and standardised burst integral value/h were 0.233, 0.276, 0.507, 0.417, and 0.482, respectively. Of the indices, bursts/h and episodes/h had the smallest day-to-day variation. The CV for the number of waveforms tended to be smaller the larger the number of waveforms. CONCLUSION: These findings are considered useful as reference data for establishing diagnostic/assessment criteria for AB using masseter EMG in the future.

Is Oral Function Associated with the Development of Sarcopenic Obesity and Sarcopenia in Older Adults? A Prospective Cohort Study International journal

BACKGROUND: Sarcopenic obesity, defined as the concurrent loss of muscle mass and adipose tissue accumulation, is associated with reduced physical function and poor health status in older adults. Although oral function can impact the overall health of older adults, its role in the development of sarcopenic obesity remains unclear. Herein, we aimed to examine the association between oral function and the incidence of sarcopenic obesity. METHODS: This longitudinal cohort study included 597 independent older adults (aged ≥65 years) from Tamba-Sasayama, a rural region of Japan, who participated in academic studies between June 2016 and December 2023. Participants underwent surveys at least twice, with a minimum two-year interval. The participants were divided into four groups (robust, obese, sarcopenic, and sarcopenic obese) according to their health condition. Sarcopenic obesity was diagnosed based on the guidelines of the Japanese Working Group on Sarcopenic Obesity. The oral function was evaluated by assessing the number of remaining teeth, tongue pressure, occlusal force, masticatory performance, and oral diadochokinesis. Cox proportional hazards regression analysis evaluated the association between oral function and the incidence of sarcopenic obesity after adjusting for relevant confounders. RESULTS: The sarcopenic obesity group was older, had lower skeletal muscle mass, and inferior physical function. This cohort also had the highest prevalence of hypertension and significantly fewer remaining teeth. The proportion of individuals with sarcopenic obesity was 1.7% of the total population, with 2.8% in the obesity group at baseline, and 8.0% of those were diagnosed with sarcopenia progressing to sarcopenic obesity. The Cox regression model revealed that reduced tongue pressure was significantly associated with an increased risk of sarcopenic obesity, with a hazard ratio of 0.906 (95% confidence interval: 0.829-0.990; p = 0.028), independent of other variables related to sarcopenia and obesity. CONCLUSIONS: Our findings suggest that oral function is associated with the incidence of sarcopenic obesity but not with that of sarcopenia or obesity alone.

BACKGROUND/OBJECTIVES: Oral frailty, first identified in Japan in 2014, refers to a state between healthy oral function and severe decline, marked by minor issues, such as tooth loss and chewing difficulties. The oral frailty five-item checklist (OF-5) enables non-dental professionals to evaluate oral frailty using five key indicators: remaining teeth count, chewing difficulties, swallowing difficulties, dry mouth, and articulatory oral skills. Limited studies exist. METHODS: This study examined the relationship between oral and physical frailties in older adults and assessed the prognosis of physical frailty using the OF-5. Participants aged ≥65 years were recruited from the frail elderly in the Sasayama-Tamba area, Hyogo, Japan, and their physical function was assessed in terms of grip strength, walking speed, and skeletal muscle mass. Blood markers, such as cystatin C, an indicator of renal function, were also analyzed. RESULTS: A cross-sectional analysis indicated that oral frailty was correlated with reduced muscle mass, walking speed, and physical function. Women had lower hemoglobin and albumin levels and a greater prevalence of frailty than men. Longitudinal analysis revealed that initial OF-5 scores predicted increased physical frailty after 2-3 years, especially in those with higher baseline scores. The OF-5 was a significant factor for frailty progression in both sexes. CONCLUSIONS: These results suggest that early detection of oral frailty via the OF-5 may be useful in preventing the progression of overall frailty in older adults.

STATEMENT OF THE PROBLEM: Previous longitudinal studies have found that patients with shortened dental arches (SDA) have a higher risk of long-term tooth loss than complete dental arch (CDA). However, the difference between SDA and CDA concerning oral function has yet to be demonstrated. PURPOSE: We aimed to clarify the influence of molar occlusal support on oral function in SDA and CDA participants cross-sectionally and longitudinally. MATERIALS AND METHODS: We enrolled independent older adults (≥65 years) living in rural areas of Japan. Of these, 257 had CDA, and 21 had SDA without removable prostheses. We assessed the oral function (number of functional teeth, masticatory performance using gummy jelly, occlusal force, oral cleanliness, oral dryness, and maximum tongue pressure) during baseline and two years after baseline (follow-up). Comparison of oral function between SDA and CDA and changes over time were examined using repeated measures analysis of variance and post-hoc test. RESULTS: Cross-sectional analysis showed that both masticatory performance and occlusal force were significantly lower in SDA than in CDA. The functional teeth tended to decrease over time more in SDA than in CDA (p < 0.001). No longitudinal differences were found between SDA and CDA regarding other factors. CONCLUSIONS: Participants with SDA had fewer teeth over time than CDAs, but no apparent difference was found in the decline of oral function. CLINICAL IMPLICATIONS: It is notable that prosthetic treatment for patients with SDA, whose main options are either implant or crown prosthesis, removable partial dentures, or no treatment, does not result in a loss of function over time. Prosthetic treatment should be conducted after careful consultation with the SDA patients to determine specific goals for each.

Predicting cognitive function changes from oral health status: a longitudinal cohort study International journal

Recent studies indicate a potential link between oral health and cognitive function; however, long-term associations remain unclear. This study aimed to identify oral health factors that predict changes in cognitive function among older adults over time. The study included 583 independent older adults (201 male, 382 female) with a mean age of 72.7 years. Cognitive function was assessed using Mini-Mental State Examination (MMSE) at baseline and follow-up over two years, with relative change in MMSE (rMMSE) calculated. Baseline oral health variables included number of remaining teeth, masticatory performance, occlusal force, oral diadochokinesis and tongue pressure. Physical performance and confounding factors were also considered. The relationship between rMMSE and the variables was analyzed using Pearson's correlation, Mann-Whitney U test, and multiple linear regression analysis. Cognitive function declined in 196 subjects, with rMMSE significantly correlated with oral diadochokinesis. Regression analysis revealed significant associations between cognitive changes and oral diadochokinesis (p = 0.020) and knee extension strength as a physical performance (p = 0.047). Our findings suggest that cognitive decline may be indicated by declines in both physical and oral motor performance. Incorporating oral diadochokinesis testing into health screenings could aid early detection of cognitive decline, improving outcomes and reducing healthcare burdens.

This narrative review provides an overview of current knowledge on the impact of nutritional strategies on chronic craniofacial pain associated with temporomandibular disorders (TMDs). Individuals experiencing painful TMDs alter their dietary habits, avoiding certain foods, possibly due to chewing difficulties, which might lead to nutrient deficiencies. Our literature investigation revealed that the causal links between nutritional changes and craniofacial pain remain unclear. However, clinical and preclinical studies suggest that nutraceuticals, including vitamins, minerals, polyphenols, omega-3 fatty acids, isoprenoids, carotenoids, lectins, polysaccharides, glucosamines, and palmitoylethanolamides, could have beneficial effects on managing TMDs. This is described in 12 clinical and 38 preclinical articles since 2000. Clinical articles discussed the roles of vitamins, minerals, glucosamine, and palmitoylethanolamides. The other nutraceuticals were assessed solely in preclinical studies, using TMD models, mostly craniofacial inflammatory rodents, with 36 of the 38 articles published since 2013. Our investigation indicates that current evidence is insufficient to assess the efficacy of these nutraceuticals. However, the existing data suggest potential for therapeutic intervention in TMDs. Further support from longitudinal and randomized controlled studies and well-designed preclinical investigations is necessary to evaluate the efficacy of each nutraceutical intervention and understand their underlying mechanisms in TMDs.

This study aimed to develop the Food Acceptance Questionnaire (FAQ) to assess the masticatory ability of Thai older adults (≥60 years). Fifty participants were interviewed using open-ended questions about food they regularly consumed and avoided due to difficulty chewing. From a list of 140 items, 100 were recruited for a trial version of the FAQ. A total of 154 participants responded to the 5-point Likert scale on their chewing perception of each food item (1, impossible to chew; 5, most easily eaten). The average response of each food item was used as the chewing index (CI). The 100 food items were ranked and divided into five grades based on their CIs. Masticatory performance (MP) was objectively assessed by a visual scoring method using gummy jelly (UHA Mikakuto). Two foods from each grade that demonstrated the highest correlation with MP were selected to form the final 10-item FAQ. The FAQ score was calculated by summarizing the responses of 10 items. MP correlated strongly with the FAQ score (r = 0.57, p < 0.001), indicating its predictive validity. Furthermore, the FAQ indicates strong internal consistency (Cronbach's alpha coefficient = 0.90), indicating high reliability. In conclusion, this newly developed 10-item FAQ is valid and reliable for assessing the masticatory ability of Thai older adults.

Effect of soft denture liners on complete denture treatments: A systematic review.

PURPOSE: This systematic review examined the effectiveness of soft denture relining (SDR) materials. STUDY SELECTION: A comprehensive search of MEDLINE, Cochrane Library, and ICHUSHI was conducted up to July 26, 2020. Target outcomes were patient satisfaction, oral health-related quality of life (OHRQOL), masticatory ability (MA), denture functional duration, residual ridge resorption (RRR), and microbial contamination. An organization specializing in literature searches performed the reference searches, and two reviewers independently selected the literature sources, extracted the data, and assessed the risk of bias. The reviewers resolved any disagreements concerning the assortment of literature sources through discussion. SDR included acrylic- and silicone-based materials, which were evaluated separately. RESULTS: Reviewers selected 7, 5, 11, 1, 4, and 6 studies to assess patient satisfaction, OHRQOL, MA, functional duration, RRR, and microbial contamination, respectively. The results confirmed that SDR improved patient satisfaction, OHRQOL, MA, and RRR. However, the functional duration of SDR material is shorter than that of hard denture relining (HDR) or acrylic resin material. Furthermore, SDR material is more susceptible to microbial contamination in the long term. The risk of bias for the included studies tended to be high because of specific issues (difficulty in blinding SDR versus HDR). CONCLUSIONS: For patients who wear complete dentures, SDR often provides beneficial outcomes such as pain reduction and recovery from MA. However, caution should be exercised regarding their use owing to insufficient functional duration and the possibility of microbial contamination during long-term use.

Comparison of T2 values of the displaced unilateral disc and retrodiscal tissue of temporomandibular joints and their implications. International journal

Unilateral anterior disc displacement (uADD) has been shown to affect the contralateral joints qualitatively. This study aims to assess the quantitative T2 values of the articular disc and retrodiscal tissue of patients with uADD at 1.5 Tesla (T). The study included 65 uADD patients and 17 volunteers. The regions of interest on T2 maps were evaluated. The affected joints demonstrated significantly higher articular disc T2 values (31.5 ± 3.8 ms) than those of the unaffected joints (28.9 ± 4.5 ms) (P < 0.001). For retrodiscal tissue, T2 values of the unaffected (37.8 ± 5.8 ms) and affected joints (41.6 ± 7.1 ms) were significantly longer than those of normal volunteers (34.4 ± 3.2 ms) (P < 0.001). Furthermore, uADD without reduction (WOR) joints (43.3 ± 6.8 ms) showed statistically higher T2 values than the unaffected joints of both uADD with reduction (WR) (33.9 ± 3.8 ms) and uADDWOR (38.9 ± 5.8 ms), and the affected joints of uADDWR (35.8 ± 4.4 ms). The mean T2 value of the unaffected joints of uADDWOR was significantly longer than that of healthy volunteers (P < 0.001). These results provided quantitative evidence for the influence of the affected joints on the contralateral joints.

PURPOSE: This study aimed to elucidate the relationship between diurnal masseter muscle activity and awareness of diurnal awake bruxism (d-AB) by conducting a comparative analysis of electromyographic (EMG) data from individuals with and without awareness of diurnal awake bruxism (d-AB), utilizing EMG data gathered from multiple subjects. METHODS: Unilateral masseter electromyography (EMG) recordings were performed during the daytime using an ultraminiature wearable EMG device. A total of 119 participants (59 with awareness of diurnal tooth clenching [d-TC] and 60 without awareness of d-TC) were included. Waveforms longer than 0.25 s with the two amplitude conditions, exceeding twice the baseline and >5% of maximum voluntary clenching, were extracted. In addition, the number of bursts and episodes (groups of bursts), burst duration, and burst peak amplitude were calculated for each participant. RESULTS: There were no significant differences in the EMG parameters between the groups with and without awareness of d-TC. Additionally, the frequency distribution of the number of EMG waveforms exhibited wide ranges and substantial overlap between the two groups. CONCLUSIONS: The variability in the number of bursts and episodes, burst peak amplitude, and burst duration among subjects suggests the need for an objective classification of d-AB severity based on EMG values. The absence of significant differences and large overlap in frequency distributions between the groups with and without awareness of d-TC indicate difficulty in predicting muscle activity solely based on awareness of d-AB.

Masticatory function such as chewing is expected to modify human cognitive function, and/or the possibility of improving cognitive function is also predicted. This systematic review investigated whether masticatory function affects cognitive function for older/young adults. Full articles written in English from January 2000 to April 2022 were collected using PubMed and Cochrane Library. Target outcomes were cognitive function test scores, cognitive processing speed (reaction time), and masticatory function. For each research question, two independent reviewers conducted the search and screening, data extraction, quality assessment, and risk of bias assessment. The reviewers resolved any disagreements by discussion. From 226 articles retrieved, 20 were included in this review. Older adults with lower scores on the cognitive function test had lower masticatory performance, lower chewing ability, chewing difficulty, and decreased number of teeth. An increased risk of cognitive impairment was found in older adults with masticatory dysfunction. For young adults, gum chewing significantly reduced the processing speed of cognitive tasks compared to no gum chewing. Although most of the evidence included had a low level of evidence and a high risk of bias because of the research designs, the results still suggest that mastication may be a factor in improving cognitive function.
