  • Late-Onset Bone Metastasis 46 Years After Initial Surgery for Pheochromocytoma: A Case Report. 国際誌

    INTRODUCTION: Pheochromocytoma is a rare catecholamine-producing tumor with metastatic potential. Recurrence after more than 40 years is exceptionally rare. CASE PRESENTATION: During evaluation for ischemic colitis, a 71-year-old woman was found to have multiple bone metastases, possibly linked to catecholamine excess. She had undergone left adrenalectomy for pheochromocytoma at age 25. Bone biopsy confirmed metastatic pheochromocytoma, and immunohistochemical findings were similar to the original tumor. Urinary metanephrine and normetanephrine were markedly elevated. She declined systemic therapy and has remained clinically stable for 6 years, with her blood pressure well controlled on doxazosin. CONCLUSION: This case illustrates a recurrence 46 years after adrenalectomy, potentially representing the longest reported interval to date. It highlights the silent and indolent nature of some metastatic pheochromocytomas and underscores the necessity of lifelong follow-up. The patient's stable course also emphasizes the clinical heterogeneity of metastatic pheochromocytoma and supports the need for individualized follow-up and treatment strategies.

    We found an extremely rare case of upper gastrointestinal involvement in trisomy 8-positive myelodysplastic syndrome. This is the first reported case of trisomy 8-positive myelodysplastic syndrome with both upper and lower gastrointestinal involvement, which was successfully treated with bone marrow transplantation.

    This study aimed to elucidate the morphological and proliferative progression of dysplasia in sessile serrated lesions (SSLs) to submucosal invasive carcinoma (SIC). Seventy-six SSLs with dysplasia, including 20 with SIC, were analysed. Each lesion was systematically partitioned into morphological units (MUs) based on cytological atypia (low/high) and gland pattern (serrated/tubular), resulting in 200 MUs: serrated-low (n = 76), serrated-high (n = 34), tubular-low (n = 26), and tubular-high (n = 64). These MUs were further categorised as SIC-related (n = 21) or -unrelated (n = 179). Ki67 immunostaining defined proliferative zones within each MU: lower (Ki67-L, n = 70), upper (Ki67-U, n = 40), diffuse (Ki67-D, n = 58), and other (n = 32, excluded from statistical analyses). Fisher's exact test was used in the following specific analyses. Significant associations between serrated-low MUs and SIC-unrelated MUs, and between tubular-high MUs and SIC-related MUs (p < 0.001, respectively) were observed. Moreover, Ki67-L patterns were absent in SIC-related MUs (0/70, 0%, p < 0.001), predominantly in serrated-low MUs (45/70, 64%, p < 0.001). Conversely, Ki67-D patterns were enriched in SIC-related (20/58, 34%, p < 0.001) and tubular-high MUs (39/58, 67%, p < 0.001). Furthermore, Ki67-U patterns exhibited intermediate frequencies between Ki67-L and Ki67-D patterns, with one (2.5%) in SIC-related MUs, six (15%) in serrated-low MUs, and 16 (40%) in tubular-high MUs. This study revealed a significant correlation between Ki67 expression patterns and morphological progression of dysplasia in SSLs. Additionally, the observed Ki67 redistribution, from lower to upper mucosa, followed by diffuse downward spread, mirrors the top-down growth pattern observed in conventional adenomas. Further molecular studies focusing on these proliferative zones are crucial for elucidating the underlying mechanisms of dysplasia progression in SSLs.

    Rectal implantation cysts can occur at anastomotic sites after low anterior resection (LAR) for rectal cancer. Herein, we report a case of primary adenocarcinoma arising from a rectal implantation cyst after LAR for rectal cancer. A 70-year-old woman was referred to our hospital for diagnosis and treatment of a growing cystic lesion. She had LAR performed for rectal cancer 29 years previously and had a rectal implantation cyst detected 13 years previously. On the first visit to our hospital, serum CEA and CA19-9 levels were elevated, and computed tomography (CT) scans revealed a cystic lesion near the anastomosis. CT-guided biopsy revealed no cancer tissue in the cystic lesion. After that, the cystic lesion naturally shrank, and serum CEA and CA19-9 levels became normal. Follow-up included 3 monthly serum CEA and CA19-9 testing and 6 monthly CT scans. Two years later, serum CEA and CA19-9 levels were elevated again. Colonoscopy revealed an ulcerative lesion at the anastomotic site, in which adenocarcinoma was confirmed. Abdominoperineal resection with sacral resection was performed, and postoperative histopathological examination revealed a primary adenocarcinoma with mucinous component at the implantation cyst. Since rectal implantation cysts can become malignant after extended periods, clinicians need to be aware of this disease.

    A 64-year-old woman was receiving oral methotrexate (MTX) for rheumatoid arthritis (RA) for 15 years. She underwent esophagogastroduodenoscopy because of discomfort in the chest. Endoscopic findings revealed an ulcer in the lower esophagus extending to the gastroesophageal junction (EGJ). The ulcer occupied half of the esophageal lumen and had a sharp and clear margin. Magnifying narrow-band imaging endoscopy revealed the deposition of white plaque, and there were few microvessels in the edge and bottom of the ulcer. Histologic examination of the biopsy specimens from the oral edge of the lesion revealed proliferation of atypical lymphoid cells (immunophenotype results: CD20 [+], CD3 [partially +], CD5 [-], and BCL-2 [-]]. The patient was diagnosed with methotrexate-associated lymphoproliferative disorder (MTX-LPD) and was advised to stop MTX intake. After 2 months of stopping MTX, the ulcer was found to be almost regressed and showed signs of healing. MTX-LPD in the lower esophagus extending to the EGJ is extremely rare. This case can help in expanding the understanding of esophageal MTX-LPD.

    Severe hepatotoxicity from combination chemotherapy with cisplatin and 5-fluorouracil is a rare adverse effect. In this case report, we present a case with severe hepatotoxicity immediately following chemotherapy with cisplatin and 5-fluorouracil. This female patient had previously been treated with this combination with no hepatotoxicity. The elevated liver enzymes quickly normalized after chemotherapy was stopped. There were no specific changes in liver imaging. As hepatotoxicity occurred after repeated administration of cisplatin, we suggest that this hepatotoxicity might represent a case of allergic hepatitis caused by cisplatin. Severe hepatotoxicity should be watched for with repeated administration of cisplatin.

    BACKGROUND: Pulmonary tumor thrombotic microangiopathy (PTTM) is a rare condition in patients with hepatocellular carcinoma (HCC); to date, few cases have been reported. While hepatic dysfunction has been focused on the later stages of HCC, the management of symptoms in PTTM is important for supportive care of the cases. For the better understanding of PTTM in HCC, the information of our recent case and reported cases have been summarized. CASE SUMMARY: A patient with HCC exhibited acute and severe respiratory failure. Radiography and computed tomography of the chest revealed the multiple metastatic tumors and a frosted glass-like shadow with no evidence of infectious pneumonia. We diagnosed his condition as acute respiratory distress syndrome caused by the lung metastases and involvement of the pulmonary vessels by tumor thrombus. Administration of prednisolone to alleviate the diffuse alveolar damages including edematous changes of alveolar wall caused by the tumor cell infiltration and ischemia showed mild improvement in his symptoms and imaging findings. An autopsy showed the typical pattern of PTTM in the lung with multiple metastases. CONCLUSION: PTTM is caused by tumor thrombi in the arteries and thickening of the pulmonary arterial endothelium leading to the symptoms of dyspnea in terminal staged patients. Therefore, supportive management of symptoms is necessary in the cases with PTTM and hence we believe that the information presented here is of great significance for the diagnosis and management of symptoms of PTTM with HCC.
