    Background: Cognitive impairment, including dementia, is a growing global health challenge, particularly as populations age. Previous studies have identified periodontitis and diabetes mellitus as modifiable risk factors for dementia, potentially linked through systemic inflammation. We hypothesize that systemic inflammation induced by periodontitis may contribute to an increased risk of cognitive impairment. Therefore, this study aims to explore the correlation between periodontal inflammation and the risk of Mild Cognitive Impairment (MCI) in type II diabetes mellitus. Materials and Methods: Baseline data analysis was performed as an analytical cross-sectional study among diabetic patients aged 40 and older who met the inclusion criteria from a randomized controlled trial (RCT) from November 2020 to April 2023. Periodontal inflammation was measured using the Periodontal Inflamed Surface Area (PISA) score. The MCI risk score was calculated using blood samples to assess eight protein markers correlated to MCI (ApoA1, TTR, C3, Albumin, ApoC1, A1BG, A2AP, and HPX). Fisher's exact test and Spearman's correlation analysis were performed. Results: 29 T2DM patients were included in the study. There was a significant difference in MCI risk score between the low and high PISA levels group (p < 0.05). Patients with low PISA scores tend to have a lower risk of MCI (p < 0.00). Variables correlated with MCI risk are PISA (ρ = 0.37; p < 0.05) and TTR levels (ρ = -0.51; p < 0.01). ApoA1 has a correlation with CRP (ρ = 0.42; p < 0.05) and IL-6 (ρ = 0.43; p < 0.05), and C3 (ρ = 0.42; p < 0.05) was correlated with CRP. Conclusions: This study found that periodontal inflammation status has a potential correlation to the risk of MCI.

    Dental checkups are integral to maintaining oral function in older adults and are a critical determinant of overall health and quality of life. However, their impact on oral function parameters remains understudied. This study investigated the association between dental checkup attendance and oral function in older adults. This cross-sectional study recruited a convenience sample of 235 older adults (75.1 ± 5.6 years, 125 males). Participants were categorized based on biannual dental checkup attendance. Seven oral parameters were assessed: oral cleanliness, oral dryness, occlusal support, oral motor function, tongue pressure, masticatory performance, and swallowing function. Statistical analyses included the Mann‒Whitney U test, chi-squared test, and linear regression analysis. Significant differences between attendees and non-attendees were found in sex, body mass index, and oral hygiene practices. Oral cleanliness (p = 0.005), masticatory performance (p < 0.001), and swallowing function (p = 0.001) were significantly better in checkup attendees. Non-attendance at dental checkups was consistently associated with the extent of declined oral function in crude (B:0.45; 95%CI:0.12,0.77) and fully adjusted models (B:0.56; 95%CI:0.16,0.97). Given the positive association between dental checkup non-adherence and declined oral function, encouraging community-based awareness campaigns with accessible dental care could promote checkup attendance. Consequently, it contributes to maintaining oral function in the older adult population.

    OBJECTIVES:  This article investigates the changes in blood-based biomarkers associated with mild cognitive impairment (MCI) risk in type 2 diabetic patients following local antibiotic periodontal treatment. MATERIALS AND METHODS:  A secondary analysis of data from a 24-week randomized controlled trial was conducted, involving 27 patients with type 2 diabetes mellitus and periodontitis. Participants received periodontal treatment biweekly from baseline until the 6th week of the study. Sixteen patients were assigned to an intervention group and received local antibiotic periodontal treatment (Periofol 2%). The outcomes were periodontal inflammation score, which was measured using periodontal inflamed surface area, the inflammation markers levels (tumor necrosis factor-α, C-reactive protein, and interleukin [IL]-6), and MCI risk score, which was assessed using protein plasma analysis through blood test. The evaluations were performed at baseline and week 24th in both groups. The changes in periodontal inflammation scores, inflammation parameters, and MCI risk in baseline and week 24th were analyzed. STATISTICAL ANALYSIS:  The Wilcoxon signed-rank test was used for within-group analysis and the Mann-Whitney U test was utilized for between-group analysis. RESULTS:  Periodontal parameters were improved in both groups (p < 0.05). IL-6, complement C3, and alpha-2-antiplasmin levels were significantly decreased in the intervention group (p < 0.05). In between-group comparisons, there was a significant difference between the control and intervention groups in apolipoprotein A1, apolipoprotein C1, and alpha-1-B glycoprotein levels in week 24th (p < 0.05). CONCLUSION:  Even though the periodontal status showed significant improvement after being given local antibiotic periodontal treatment, the changes in MCI risk proteins plasma remained unclear.

    BACKGROUND AND OBJECTIVES: Weight loss is a critical health issue among the older population. This study aimed to explore the association between weight loss and oral health in older adults. METHODS: We used cross-sectional data from the Uonuma Chronic Kidney Disease Cohort Study. The binary logistic regression models were fitted to estimate the relationship between weight loss and independent variables including the number of teeth present. RESULTS: The participants included 1656 females and 1625 males. There were 1217 females (73.4%) and 1236 males (76.0%) with normal BMI (18.5 ≤ BMI < 25) in the previous year. Weight loss was determined over a 12-month period. Individuals with normal BMI in previous year, and those with weight loss (≥ 5%) or those whom maintained a stable weight were further analysed. The regression analysis (model 3, fully adjusted) showed that females with 0 and 1-10 teeth present had a higher likelihood of losing weight (OR = 4.07; 95% CI = 1.85-8.93, OR = 2.33; 95% CI = 1.24-4.39) compared with those with 20 or more teeth. Current smokers had a higher likelihood of losing weight (OR = 3.15; 95% CI = 1.01-9.76) than lifelong abstainers. Among males, both current and previous smokers showed a higher likelihood of losing weight (OR = 4.94; 95%CI = 1.76-13.8, OR = 3.35; 95% CI = 1.29-8.68) than those who never smoked. CONCLUSIONS: This study indicates that the number of teeth and smoking are both linked to weight loss in females. However, in males, the association between the number of teeth and weight loss may be compromised due to the high prevalence of smoking in this group.

    The association between adipokines and inflammatory periodontal diseases has been studied over the last two decades. This review was intended to explore the observation that periodontal therapy may lead to an improvement of adipokines in diabetic patients. In summary, substantial evidence suggests that diabetes is associated with increased prevalence, extent and severity of periodontitis. Numerous mechanisms have been elucidated to explain the impact of diabetes on the periodontium. However, current knowledge concerning the role of major adipokines indicates only some of their associations with the pathogenesis of periodontitis in type 2 diabetes. Conversely, treatment of periodontal disease and reduction of oral inflammation may have positive effects on the diabetic condition, although evidence for this remains somewhat equivocal.

    本研究の目的は、2型糖尿病患者への抗菌的歯周治療が、アディポサイトカインの変化を介して、心筋梗塞リスクに及ぼす影響をランダム化比較試験により検証することである。
    2023年度は、倫理審査申請を行い、許諾を受けた。（2023-0048）
    当初の予定より倫理審査許諾に時間を要し、またコロナやインフルエンザによる臨床研究遂行の環境整備に時間を要したため、研究開始が遅れている。2024年2月に代謝内科にて研究打ち合わせを行い、2024年6月より患者スクリーニングを行う予定である。
    なお、2023年5月に日本口腔衛生学会、2024年3月にIADR国際歯科研究学会に参加し、研究に関する情報収集を実施した。

    研究課題「糖尿病患者の脳梗塞リスクに対する抗菌的歯周治療の有用性に対するランダム化比較試験」は、令和2年8月に新潟大学倫理審査委員会にて倫理審査承認（2020-0126）を得て開始した。本研究はアディポサイトカイン、とくにアディポネクチンの濃度変化に着目して、血管障害の改善に対する抗菌的歯周治療の効果を検証し、脳梗塞リスクへの抗菌的歯周治療の有用性に関するエビデンスを構築することを目的としている。
    令和3年度は、新たに25名の研究対象者のリクルート（総数50目標）を計画したが、令和4年3月末時点で研究参加者総数は30名で、うち21名が研究完了となっている。新型コロナウイルス感染拡大による歯科受診控えはだいぶ少なくなったが、研究参加期間が長いこと、すでに歯科受診をしているなどの理由で参加に同意を得られない場合が多く、参加者確保に時間を要している。すでに内科医と相談し、令和4年度にも対象者のスクリーニングを継続する予定である。また状況によっては、科研研究そのものの延長も視野に入れている。
    なお、研究参加者への口腔内処置は、新潟大学医歯学総合病院の感染対策マニュアルに準じて適切な感染対策下で実施しており、問題は発生していない。また、抗菌剤の局所投与についても、日本歯周病学会の抗菌療法診療ガイドラインに準じて適切な薬剤を適切な量と期間使用を遵守しており、必要最小限度の投与を行っている。薬剤耐性菌等の発生はない。
    令和4年度も万全な感染対策の下、対象者に対して研究計画の目的、内容ならびに個人情報の保護等について書面にて十分な説明を行い、さらなる研究参加者の確保（新規に20名）を目指す。

    アルツハイマー型認知症を含む大半の認知症の初期症状が抑うつや不安であると言われている。そのため、我々は認知症発症あるいは認知機能低下のサロゲート指標として対象者の抑うつ感を評価指標に用いることとした。
    本研究では、新潟県魚沼地区に居住する65歳以上の高齢者を対象に、現在歯数と心の健康状態の関連についての検討を行った。メタボリックシンドローム該当者あるいはメタボリックシンドローム予備軍において、現在歯数19本以下の者は、20本以上の者と比較して、心と体の健康状態が悪い者の割合が有意に高いという結果が得られた。
