    PURPOSE: To investigate the association between plasma antibody levels against Porphyromonas gingivalis (PG) and atrial fibrillation (AF) history in community-dwelling older individuals in Japan. MATERIALS AND METHODS: This study was a subset of the Uonuma cohort study, including 3091 participants aged 60-79 years. Data were collected, including AF history as a dependent variable, plasma immunoglobulin G antibody levels against PG as an independent variable, and previously reported AF risk factors and demographic information as covariates. RESULTS: The median age of the participants was 69 years. Of the 3091 participants, 1411 (45.6%) were men, and 56 (1.8%) had an AF history. AF prevalence was significantly higher in participants with higher antibody levels against PG than in those with lower antibody levels (3.0% vs 1.4%; p = 0.005). Multivariable logistic regression analysis showed that participants with higher antibody levels against PG had twofold higher odds of having AF (odds ratio = 2.13; 95% confidence interval = 1.23-3.69). Restricted cubic spline analysis indicated a nonlinear relationship between antibody levels against PG and AF history. CONCLUSION: Plasma antibody levels against PG were associated with AF history in community-dwelling older individuals in Japan.

  • Prevalence and Cause of Enamel Hypoplasia in Primary Teeth among 1-year-old Japanese Children Reviewed

    Objective: In this study, the prevalence and underlying cause of enamel hypoplasia in one-year-old children were examined. Additionally, to determine the association between enamel hypoplasia and exposure to sunlight based on the frequency of the mother’s outings during pregnancy. Methods: A total of 273 kids (139 boys) were involved in this study. Based on questionnaires surveys to mothers, oral examinations, and photographs of children's mouths, we looked at the connection between the circumstances of pregnancy and the occurrence of enamel hypoplasia. The presence of enamel hypoplasia was assessed using logistic regression analysis differed among the results of the questionnaire. Results: The prevalence of enamel hypoplasia was 38.5%. In the medium and low groups, enamel hypoplasia was more common than in the high group at the birth month of the child (adjusted odds ratio: 3.36, 95% confidence interval: 1.65–6.85, p = 0.001 vs. adjusted odds ratio: 3.76, 95% confidence interval: 1.89–7.48, p < 0.001, respectively). Enamel hypoplasia was knowingly higher among the children whose mothers went out less than 1–2 times a week during pregnancy (adjusted odds ratio: 2.41, 95% confidence interval: 1.04–5.61, p = 0.041). Enamel hypoplasia was significantly more prevalent when mothers used painkillers during pregnancy (adjusted odds ratio: 5.45, 95% Confidence interval: 1.74–17.08, p = 0.004). Conclusion: It was suggested that the use of antipyretic analgesics during pregnancy and frequency of outings and variations in sunshine hours according to the child's birth month could affect the occurrence of enamel hypoplasia.

    Objectives: The present study aimed to examine the association between periodontal condition based on the plasma antibody levels to periodontopathic bacteria and a history of atrial fibrillation (AF).
    Methods: A cross-sectional study was conducted on 1,740 participants (1,199 men and 541 women) aged 60–79 years using the baseline health survey data in the Uonuma region of Japan from 2012 to 2014. Data were collected from medical examinations, self-administered questionnaires that enquired about a history of AF, and plasma IgG antibody levels to Porphyromonas gingivalis gingipain. A history of AF (0: absence, 1: presence) was used as a dependent variable while antibody levels (0: ≤third quartile, 1: >third quartile) and a history of angina pectoris were considered independent variables. Age and sex were used as covariates. Multivariate logistic regression analysis was conducted to determine the association between antibody levels to P. gingivalis and a history of AF.
    Results: Among 1,740 participants, 39 (2.2%) had AF. Participants with higher antibody levels to P. gingivalis showed a significantly higher prevalence of AF than those with lower antibody levels (4.1% vs. 1.6%; p < 0.05). Multivariate logistic regression analysis showed that participants with higher antibody levels to P. gingivalis had an approximately 3-fold higher odds of having AF (OR = 2.71; 95% CI = 1.42–5.14).
    Conclusions: A correlation was observed between antibody levels to P. gingivalis and a history of AF in Japanese adults aged 60–79 years. These results may suggest that periodontal condition contributes to AF onset.

  • <Report and Essay>Practice of Field Medicine Related to Health Maintenance for Community-Dwelling Elderly People in 2017

    During the week of August 6th, 2017, we conducted "Practice of Field Medicine related to health maintenance for community-dwelling elderly people" in Tosa town. In this program, students participate into health checkups for the community-dwelling elderly as staff members. The program was started in 2004 for the purpose of enabling the elderly to live in their home land cheerfully. During the last 14 years, the life expectancy of Tosa town became the longest for women and the 3rd longest for men in Kochi Prefecture. Before the program, the premium for nursing-care insurance of Tosa town was the highest in the prefecture. But now, it became suppressed compared with other towns in the prefecture. Based on the activities in Tosa town, the Bhutanese government is now trying to introduce health checkup system for the elderly on a national scale. In 2017, students came to Tosa town to participate into the program from various universities such as Kyoto University, Tokyo Women's Medical University, Kochi University, Niigata University, Osaka University, Kyushu Dental University, University of Debrecen, and University of Kochi. In this article, we would like to introduce what made the students come to Tosa town and what they got through the experiences in Tosa town with their own words.

    Objectives: To examine the association between plasma antibody levels against Porphyromonas gingivalis (PG) and isolated systolic hypertension (ISH) among community-dwelling older adults in Japan.
    Methods: A cross-sectional study was conducted on 1,942 participants aged 60–79 years using the baseline data of the Uonuma cohort study. Data on systolic and diastolic blood pressure were collected through medical examinations and plasma immunoglobulin G anti-PG antibody levels were measured. The other data were collected through medical examinations or a self-administered questionnaire. ISH was defined as a systolic blood pressure of ≥140 mmHg with a diastolic counterpart of <90 mmHg. Participants were divided into quartiles according to their anti-PG antibody levels (quartile 1, lowest; quartile 4, highest). The Cochran−Armitage test was used to look for the trend toward a higher prevalence of ISH with higher anti-PG antibody levels. A multivariable logistic regression analysis was performed to assess their association.
    Results: The prevalence of ISH was 24.0%, and it increased with higher anti-PG antibody levels (19.3%, 24.5%, 26.1%, and 26.0% for quartiles 1, 2, 3, and 4, respectively; p for trend = 0.013). After adjusting for previously reported ISH risk factors, participants with the highest quartile of anti-PG antibody levels were more likely to have ISH (adjusted odds ratio = 1.43; 95% confidence interval = 1.05–1.96) than those with the lowest quartile.
    Conclusions: A positive association was found between plasma anti-PG antibody levels and ISH among community-dwelling older adults in Japan.

    Clinical dental examinations are a resource-intensive method in terms of personnel, facilities, time, and cost. Many studies have reported that self-reported number of remaining teeth was a valid reflection of the clinically-examined number of teeth. However, it remains unknown whether this validity is similarly applicable to patients who performed different oral health behavior or had different health conditions of gingiva. Accordingly, the aim of this cross-sectional study was to assess the validation of self-reported number of remaining teeth in different oral health behavior or health conditions of gingiva. Self-administered questionnaires and clinical examinations were conducted on 9,560 new patients over 20 years old (3,324 men, 6,236 females) in 1,215 dental clinics in Japan. After dividing them into two groups according to the oral health behavior or health conditions of gingiva, we compared the number of remaining teeth from self-reported questionnaires and clinical value. Overall, the 78% of patients showed the difference within one tooth between the two score. In addition, the differences in the number of remaining teeth between 2 average score in the different oral health behavior or different health conditions of gingiva were 0.1-0.3 teeth. In conclusion, self-reported questionnaires can give sufficiently reliable data about remaining teeth even if patients performed different oral health behavior or had different health conditions of gingiva.
