    Lip vermilion is unique and can be distinguished from the adjacent skin and oral mucosa. However, because of the lack of appropriate evaluation tools, skin and/or oral mucosa substitutes such as in vitro vermilion epithelial models have been used for lip product testing. We aimed to develop and characterize a lip vermilion epithelium reconstruction model (LVERM) using skin and oral keratinocytes. LVERM was manufactured by co-culturing primary skin and oral keratinocytes, using a device that allowed the separation of cell seeding, and created an intercalated cell-free zone, referred to as the vermilion part. After removing the device, LVERM construction was completed in 8 days, in a submerged condition. Subsequently, they were placed in an air-liquid interface for 7 days. To determine the epithelial characteristics of LVERM, keratin 2e (KRT2) and small proline-rich protein 3 (SPRR3) expression patterns were examined. The in vivo expression profiles of KRT2 and SPRR3 genes in vermilion were also examined. We found that a continuous multi-layered epithelium was generated in the LVERM that exhibited ortho- and para-keratinization in the skin and oral mucosa parts, respectively. Although an intermediate keratinization pattern was observed in the vermilion part, KRT2 and SPRR3 were co-expressed in the suprabasal layer, consistent with the expression pattern of a single vermilion epithelial model. Clustering analysis revealed that KRT2 and SPRR3 gene expression in vermilion was location-dependent within the sample. Therefore, LVERM can be used as an evaluation tool for lip products and has great importance in innovative approaches for cosmetic testing.

    We previously reported that the cell and colony motion of oral keratinocytes are correlated with proliferative capacity, and speculated that this may be a specific index for monitoring cell quality. However, how cell motility and proliferation are regulated by signaling pathways remains unelucidated. Here, we found that the regulation of cell motility and proliferative capacity of oral keratinocytes can be attributed to the epidermal growth factor/epidermal growth factor receptor (EGF/EGFR) axis. The EGFR downstream cascade involving the Src/PI3K/Akt/mTOR signaling pathway showed a major effect on cell motility and proliferative capacity in oral keratinocytes. Furthermore, both EGFR and Src attenuated E-cadherin expression. Taken together, these findings provide a potential basis for future quality control of cells for therapeutic use.

    BACKGROUND: We previously reported a novel technique for fabricating dermo-epidermal junction (DEJ)-like micropatterned collagen scaffolds to manufacture an ex vivo produced oral mucosa equivalent (EVPOME) for clinical translation; however, more biomimetic micropatterns are required to promote oral keratinocyte-based tissue engineering/regenerative medicine. In addition, in-process monitoring for quality control of tissue-engineered products is key to successful clinical outcomes. However, evaluating three-dimensional tissue-engineered constructs such as EVPOME is challenging. This study aimed to update our technique to fabricate a more biomimetic DEJ structure of oral mucosa and to investigate the efficacy of optical coherence tomography (OCT) in combination with deep learning for non-invasive EVPOME monitoring. METHODS: A picosecond laser-textured microstructure mimicking DEJ on stainless steel was used as a negative mould to fabricate the micropatterned collagen scaffold. During EVPOME manufacturing, OCT was applied twice to monitor the EVPOME and evaluate its epithelial thickness. FINDINGS: Our moulding system resulted in successful micropattern replication on the curved collagen scaffold. OCT imaging visualised the epithelial layer and the underlying micropatterned scaffold in EVPOME, enabling to non-invasively detect specific defects not found before the histological examination. Additionally, a gradual increase in epithelial thickness was observed over time. CONCLUSION: These findings demonstrate the feasibility of using a stainless-steel negative mould to create a more biomimetic micropattern on collagen scaffolds and the potential of OCT imaging for quality control in oral keratinocyte-based tissue engineering/regenerative medicine.

    OBJECTIVE: This study aimed to determine if the change in technique of soft palate closure or timing of hard palatal repair induced occlusal changes in patients with complete unilateral cleft lip and palate (CUCLP). DESIGN: Retrospective study. SETTINGS: A medical and dental hospital in Japan. SUBJECTS: A total of 96 patients with CUCLP treated with 2-stage palatoplasty were included in the study and categorized into 3 groups (G1, G2, and G3) according to the protocol used. INTERVENTIONS: G1 underwent soft palate repair using Perko method at 1.5 years of age and hard palate repair using vomer flap procedure at 5.5 years of age. Furlow method was used for soft palate repair in G2 at 1.5 years of age and hard palate repair using vomer flap procedure at 5.5 years of age. The Furlow method was used to repair the soft palate in G3 at 1.5 years of age and vomer flap procedure was used to repair the hard palate at 4 years of age. MAIN OUTCOME MEASURES: Two evaluators assessed the dental arch relationship using the modified Huddart/Bodenham (mHB) index on 2 separate occasions. RESULTS: Intra- (intraclass correlation coefficient [ICC]: 0.962) and inter-examiner (ICC: 0.950) reliability showed very good agreement. The frequency of crossbite present in the major and minor segments gradually decreased with each change in protocol. Mean segmental scores showed no significant difference between 3 protocols (P > .05). Good inter-arch alignment occurred with all 3 surgical protocols (G1:82.6%, G2:89.8%, and G3:91.7%). CONCLUSIONS: There was no significant difference in the dental arch relationship outcomes between the 3 surgical protocols. The dentition status was comparable with all surgical protocols, even after the changes.

    Cells/colony motion determined by non-invasive, quantitative measurements using the optical flow (OF) algorithm can indicate the oral keratinocyte proliferative capacity in early-phase primary cultures. This study aimed to determine a threshold for the cells/colony motion index to detect substandard cell populations in a subsequent subculture before manufacturing a tissue-engineered oral mucosa graft and to investigate the correlation with the epithelial regenerative capacity. The distinctive proliferating pattern of first-passage [passage 1 (p1)] cells reveals the motion of p1 cells/colonies, which can be measured in a non-invasive, quantitative manner using OF with fewer full-screen imaging analyses and cell segmentations. Our results demonstrate that the motion index lower than 40 μm/h reflects cellular damages by experimental metabolic challenges although this value shall only apply in case of our culture system. Nonetheless, the motion index can be used as the threshold to determine the quality of cultured cells while it may be affected by any different culture conditions. Because the p1 cells/colony motion index is correlated with epithelial regenerative capacity, it is a reliable index for quality control of oral keratinocytes.

    新潟大学医歯学総合病院顎顔面口腔外科(以下,当科)では,1974年の開設以来,チームアプローチによる一貫した管理体制で口唇裂・口蓋裂治療にあたってきた。1983年からHotz床併用二段階口蓋形成手術法(以下,二段階法)を採用しているが,過去に2度,言語成績の向上を目的に手術プロトコールを変更してきた。これまで,言語評価と上顎歯列模型の形態分析から治療成績の向上を報告してきたものの,咬合という観点からは検討されていなかった。そこで,今回5-Year-Olds' Indexを用いて咬合関係を評価し,2度の手術プロトコール変更の妥当性を検証した。対象は,当科で治療した片側性唇顎口蓋裂の一次症例のうち,資料の整った97例とした。97例は,軟口蓋形成術の術式と硬口蓋閉鎖術の時期により,(1)P+5群:1983~1995年に軟口蓋形成術を1歳半にPerko法で施行し,硬口蓋閉鎖術を5歳半に鋤骨弁法で施行した23例,(2)F+5群:1996~2009年に軟口蓋形成術を1歳半にFurlow変法で施行し,硬口蓋閉鎖術を5歳半に鋤骨弁法で施行した49例,(3)F+4群:2010~2017年に軟口蓋形成術を1歳半にFurlow変法で施行し,硬口蓋閉鎖術を4歳に鋤骨弁法で施行した25例の3群に分類し,比較検討を行った。5-Year-Olds' Indexを用いた咬合評価の結果,各群のスコアは,P+5群:2.65,F+5群:2.77,F+4群:2.80であり,t検定においてP+5群とF+5群間,F+5群とF+4群間に有意差はなかった。一方,度数分布ではF+4群は他の群に比べ,G1の比率が有意に低かった。なお,いずれの群においてもGroup 5はなかった。以上のことから,当科で行った手術プロトコールは言語成績の向上にはたらくものの,硬口蓋閉鎖時期の早期化が上顎歯列の頬舌的な成長に抑制的な影響を与えている可能性も示唆され,咬合関係に留意すべき症例もあると考えられた。(著者抄録)

    当院の二段階口蓋形成手術法では、軟口蓋形成術をFurlow法により1歳半に、硬口蓋閉鎖術を鋤骨弁により4歳に施行している。軟口蓋形成術後の硬口蓋残遺裂には硬口蓋閉鎖床を装着しているが、この間の言語機能の獲得には否定的な影響が懸念されやすい。本研究では、当院の治療体系における口蓋化構音の発現要因を明らかにし、口蓋化構音の発現を予防することを目的として、まず、軟口蓋形成術後の1歳半から硬口蓋閉鎖術前の4歳までを幼児期前期とし、片側完全唇顎口蓋裂16例の幼児期前期の言語発達過程でみられた言語症状を回顧的に評価し、良好群、口蓋化構音群(以下、口蓋化群)、不良群の3群に類型化した。そして、類型化した群ごとに、言語機能の獲得状況と硬口蓋残遺裂を含めた上顎歯槽弓形態の関連性について検討した。その結果、口蓋化群の2歳の硬口蓋残遺裂後端の位置に関連性が示され、硬口蓋残遺裂後端や軟口蓋からの漏れを補おうとする舌の誤学習が、口蓋化構音の発現に影響を与えている可能性が推察された。また、得られた結果にもとづき、言語機能をできる限り向上させるため、幼児期前期の言語管理の進め方について再考した。(著者抄録)

    かかりつけ歯科医院での長期メインテナンス中、下顎埋伏智歯の歯冠部に関連して生じたと考えられる高齢者の原発性骨内癌を経験したので報告する。症例は74歳の男性、右側下唇の知覚異常と右側下顎臼歯部の咬合痛を主訴に当科紹介初診となった。右側下顎第二大臼歯は挺出し、動揺度2、頬側歯肉に軽度の腫脹を認めたが排膿はなかった。パノラマX線写真で右側下顎智歯は骨性に逆生埋伏しており、単純CTで埋伏智歯の歯冠部に連続した境界不明瞭で辺縁不整な35×25mm大の腫瘤性病変を認めた。生検にて扁平上皮癌(原発性骨内癌疑い)の診断を得て、顎下部郭清術、下顎骨区域切除術、金属プレートによる顎骨再建術を行った。一般的に原発性骨内癌は嚢胞様病変から悪性転化するとされるが、本症例は当科初診の8ヵ月前に紹介元で撮影されたパノラマX線写真では異常所見は観察されず、明白な嚢胞様変化を辿ることなく悪性転化し、急速に増大したと推察された。症状のない下顎埋伏智歯は経過観察されることが多いが、発育性嚢胞や歯周炎などのリスクが低いと判断される高齢者の下顎埋伏智歯においても、多様な変化の可能性も念頭に置いた経過観察が肝要である。(著者抄録)
