    BACKGROUND: Seasonal influenza continues to pose a significant global public health challenge owing to its considerable disease burden, making it imperative to elucidate its potential driving factors. Although the roles of meteorological factors such as temperature and absolute humidity have been extensively investigated, research quantifying the burden of ambient ozone (O₃) as an environmental determinant of influenza, particularly among vulnerable populations, remains limited. The aims of this study were to assess short-term effects of photochemical oxidants (Ox, parts per billion [ppb]) in a temperate Japanese city, quantify the associated attributable burden, and evaluate variations across subgroups to enhance understanding and identify high-risk populations. METHODS: Daily time-series data on influenza cases, photochemical Ox levels, and meteorological factors were compiled for Kawasaki City, Japan, spanning 2015 to 2019. Using time-stratified case-crossover analyses and a distributed lag non-linear model, Ox-influenza associations were evaluated, accounting for long-term trends, seasonality, and day-of-the-week effects, and capturing both non-linear and delayed impacts. The overall attributable fractions (AFs) and those corresponding to low and high Ox levels were subsequently calculated. Furthermore, subgroup analyses were conducted according to influenza subtype, sex, and age group. RESULTS: Data of 163,120 influenza cases were analyzed. Overall, a U-shaped exposure-response curve was observed between Ox concentrations and influenza incidence. The overall minimum morbidity Ox (MMOx) level was 48.4 ppb (94.0%), with cumulative relative risks of 2.06 (95% confidence interval [CI]: 1.50, 2.82) and 1.47 (95% CI: 0.78, 1.51) observed at low and high Ox levels, respectively. The burden of influenza incidence attributable to non-optimal MMOx level was estimated at 36.6% (95% empirical CI: 17.1, 50.9), with the majority of this burden arising from low Ox concentrations. Furthermore, patients with influenza A exhibited higher risks and AFs than did patients with influenza B, although the differences by sex and age group were not statistically significant. CONCLUSIONS: Overall, non-optimal photochemical Ox levels are associated with a substantial burden of influenza morbidity. These findings may inform local public health initiatives by aiding the development of targeted preparedness and prevention strategies to mitigate short-term effects of air pollution.

    BACKGROUND: Influenza remains a significant public health challenge worldwide, necessitating robust forecasting models to facilitate timely interventions and resource allocation. The aim of this study was to develop a long short-term memory (LSTM)-based short-term forecasting model to accurately predict weekly influenza case counts in Tokyo, Japan. METHOD: By using weekly time-series data on influenza incidence in Tokyo from 2000 to 2019, along with meteorological variables, we developed four distinct models to evaluate the impact of the external variables of mean temperature, relative humidity, and national public holidays. After model training, we assessed the predictive performance on an independent test dataset, using mean square error (MSE), root mean square error (RMSE), mean absolute error (MAE), and Pearson's correlation coefficient. RESULTS: During the study period, 1,445,944 influenza cases were analyzed. The model incorporating all three external variables demonstrated superior predictive accuracy, with an MSE of 3,646,084, RMSE of 1,909, MAE of 849, and Pearson's correlation coefficient of 0.924. These findings underscore the substantial contribution of these external factors to improving the prediction performance. CONCLUSION: This study highlighted the efficacy of LSTM-based models for short-term influenza forecasting and reinforces the importance of integrating meteorological variables and national public holidays into predictive frameworks. Our optimal model provided more precise forecasts of influenza activity in Tokyo, Japan.

    Introduction Rotavirus is the principal pathogen responsible for acute gastroenteritis and severe diarrhea in children worldwide and remains a significant public health threat. However, studies on the association between rotavirus gastroenteritis epidemics and meteorological factors in Japan are still scarce. In this study, we aimed to quantify the short-term effects of meteorological factors on the incidence of rotavirus gastroenteritis in Japan using advanced time-series modeling approaches. Methods We conducted a two-stage time-series analysis. In the first stage, a time-stratified case-crossover study was performed using conditional quasi-Poisson regression in conjunction with a distributed lag nonlinear model to assess the association between meteorological factors and the incidence of rotavirus gastroenteritis across all 47 Japanese prefectures from 2014 to 2019. A multivariate meta-analysis was subsequently used to pool associations at the national level. Results A total of 26,549 cases of rotavirus gastroenteritis were included in the study. Our analysis revealed that low mean temperature and reduced relative humidity were significantly associated with an increased incidence of rotavirus gastroenteritis. More specifically, the overall lag-cumulative relative risk (RR) of rotavirus gastroenteritis peaked at approximately 13.0°C (RR: 2.2, 95% confidence interval (CI): 1.9-2.6) relative to a reference mean temperature of 25°C. Similarly, the RR was maximized at a relative humidity of approximately 55% (RR = 1.7, 95% CI: 1.4-2.2) compared with 77%. Conclusions This nationwide study showed that cold and dry environmental conditions were associated with an elevated risk of rotavirus gastroenteritis, which varied markedly based on region.

    BACKGROUND: Baloxavir marboxil is used for influenza treatment in Japan. However, the clinical and virologic significance of baloxavir-associated PA variants in children remains unclear. METHODS: We conducted a four-season, multicenter study from 2018-2019 to 2023-2024 in pediatric patients under 19 years old with influenza A who were treated with baloxavir. Clinical data were collected at the first visit; diary cards were maintained for up to 8 days. Clinical specimens were collected twice, with a median interval of 4 days. PA substitutions were screened by real-time RT-PCR and confirmed by next-generation sequencing and Sanger methods. RESULTS: Among 247 patients, 24 (9.7%) harbored PA variants, one pre-treatment and 23 post-treatment. PA/I38T was the most common substitution. A(H3N2) patients had a higher frequency of PA variants than A(H1N1)pdm09 (13.8% vs. 5.1%). Although PA variants were associated with slower viral RNA decline, no significant differences were observed in fever duration (median 22.6 vs. 15.6 h) or symptom duration (median 105.0 vs. 113.3 h) compared to wild-type virus infections. CONCLUSION: PA variants, especially PA/I38T, emerged more frequently in A(H3N2) infections but had minimal clinical impact. The rarity of pre-treatment variants supports limited community transmission. Continued surveillance is warranted, particularly in younger children.

    OBJECTIVE: While the short-term effects of ambient temperature on heatstroke mortality have been studied across various countries, the impact on morbidity and its spatial distribution remains insufficiently examined. We quantified the association between maximum temperature and heatstroke-related ambulance dispatch (HSAD) cases in Japan using daily national data. METHODS: Daily HSAD counts and meteorological data (daily maximum temperature (°C), relative humidity (%), wind speed (m/s), and sunshine duration (h)) were analyzed for June-September (summer), 2015-2019, across 47 Japanese prefectures. A time-stratified case-crossover study with conditional quasi-Poisson regression combined with a distributed lag non-linear model was used to estimate maximum temperature-HSAD associations in Japan between 2015 and 2019. A random-effects meta-analysis pooled country-level association. RESULTS: A total of 300,528 HSAD cases were analyzed. Higher temperatures were associated with an increased risk of HSAD cases. The overall lag-cumulative relative risk at the 95th percentile of maximum temperature (35.3 °C) compared to the minimum morbidity temperature (18.2 °C) was 19.23, with a 95 % confidence interval of 13.95 to 26.51. Lagged effects of higher temperatures persisted for up to 2 days. Significant geographical variation in HSAD cases was observed (Cochran Q test, p < 0.001; I 2 = 84.0 %). The association between maximum temperature and HSAD cases was significantly modified by the use of air conditioning (likelihood ratio test, p = 0.01). CONCLUSIONS: Our study demonstrated the influence of heat on HSAD cases in Japan, emphasizing the necessity of preventive strategies to reduce the impact of temperature on heatstroke morbidity.

    This study aims to quantify the impact of ambient temperature on the risk of herpangina in Japan. We compiled weekly time-series data on the incidence of herpangina and meteorological factors in all 47 Japanese prefectures (2005-2019). A space-time-stratified case-crossover design, combined with a distributed lag nonlinear model, was employed to estimate the relative risk (RR) of heat exposure across prefectures. A multivariate meta-analysis was conducted to obtain pooled estimates at the national level. Our analysis revealed a nonlinear, inverted U-shaped association between ambient temperature and the incidence of herpangina at the national level in Japan. Peak risk occurred at 24.3 °C, with an RR of 8.65 (95% confidence interval: 6.32-11.84) relative to the minimal risk. This association was predominantly driven by the immediate effects of high temperatures, with the risk persisting for up to four weeks. Our assessment demonstrates that high temperatures increase the risk of herpangina, highlighting the need for targeted preventive measures to mitigate the health impacts of global warming.

    This study explored the distribution and genetic characteristics of respiratory pathogens in outpatients with acute respiratory infections (ARIs) in Yangon, Myanmar, during the 2023 rainy season. Among 267 patients who tested negative for influenza, RSV, and SARS-CoV-2 using rapid diagnostic tests, 84.6% were positive for at least one pathogen according to a multiplex polymerase chain reaction (PCR) assay, the BioFire® FilmArray® Respiratory Panel 2.1. The most common viruses detected were rhinovirus/enterovirus (RV/EV) at 37.8%, respiratory syncytial virus (RSV) at 22.4%, and human metapneumovirus (hMPV) at 10.0%. These pathogens co-circulated mainly from July to September, with RV/EV consistently predominant. Symptom comparison among RV/EV-, RSV-, and hMPV-infected patients showed similar clinical features, though fever was more common in hMPV cases. Among RV/EV-positive patients, 59.3% had single infections, while 40.7% experienced co-infections, especially with RSV and adenovirus. Genotyping identified 28 types from five species, primarily RV-A and RV-C, which were genetically diverse. One EV-D68 case was also found, emphasizing its potential risk. This study underscores the genetic diversity and clinical impact of RV/EV and stresses the importance of ongoing molecular surveillance in Myanmar's post-COVID-19 context to inform effective public health responses.

    To evaluate genetic changes in respiratory syncytial virus (RSV) between 2019 and 2023, we analyzed RSV strains from Myanmar before and after the COVID- 19 pandemic. Real-time polymerase chain reaction (RT-PCR) positive samples from children presenting with acute respiratory infections at outpatient clinics in Yangon were sequenced to determine the genotype. Phylogenetic and molecular evolutionary analyses were conducted using the Bayesian Markov Chain Monte Carlo method to construct the time-scale Maximum Clade Credibility tree. Of 1127 samples, 104 (9.2%) RSV-A and 233 (20.7%) RSV-B were positive by RT-PCR. There was an absence of a notable epidemic in 2020, a temporal shift with a surge of RSV-A in the 2021 outbreak, a lack of expected cases in 2022 and a substantial resurgence of RSV-B in 2023. The genotype of RSV-A was mainly A.D.3 lineage through the study period, while RSV-B were B.D.4.1.1 and B.D.E.1. RSV-A showed that the same lineage persisted within Myanmar throughout the pandemic, leading to a large outbreak post-COVID. In contrast, RSV-B strains appear to have temporarily disappeared during the pandemic, but subsequently, globally circulating strains likely entered Myanmar, resulting in a major outbreak in 2023. The estimated evolutionary rate at the G-ectodomain for RSV-A was 7.76 × 10⁻³ and RSV-B was 5.67 × 10⁻³ substitutions/site/year. Strengthening genomic surveillance will likely support comparisons of circulating strains with those in other countries and facilitate the introduction of vaccines and other interventions.

    This study provides a comprehensive collection of open-access daily time-series datasets designed to quantify the association between short-term exposure to maximum temperature and heatstroke-related ambulance dispatches (HSAD) utilizing a two-stage time-stratified case-crossover design. The datasets include daily records of HSAD, maximum temperature (°C), and relative humidity (%) for the summer months (i.e., June to September) from 2015 to 2019, spanning all 47 Japanese prefectures and covering the entire country. These datasets are well-suited for time-series regression analyses to estimate location-specific maximum temperature-HSAD associations in Japan. The availability of data from multiple locations also facilitates the of regional differences by aggregating location-specific associations within broader geographical areas.     Varicella, commonly referred to as chickenpox, is an airborne infectious disease that continues to pose an increasing threat to public health. Despite previous investigations, the global accumulation of epidemiological studies exploring the association between varicella epidemics and meteorological factors remains insufficient. This study aimed to quantify the short-term effect of meteorological factors, such as mean temperature and relative humidity, on the incidence of varicella across all 47 prefectures of Japan over a decade (2010-2019). Using a two-stage time-series modelling design, we first applied distributed lag non-linear models to estimate the exposure-response relationships for each prefecture, followed by a meta-regression to pool these results for a nationwide estimate. The analysis covered 1,315,616 varicella cases. Our findings indicate a significant increase in varicella risk associated with lower temperatures and reduced humidity. Specifically, the cumulative relative risk (RR) at the 5th temperature percentile, compared to the 99th, was 2.45 (95% confidence interval [CI]: 2.15, 2.79). The cumulative RR at the 5th humidity percentile, compared to the 90th, was 1.11 (95% CI: 1.03, 1.09). These results suggest that public health strategies targeting temperature- and humidity-related varicella morbidity could be more effective if adapted to local climatic conditions. Understanding the influence of meteorological factors on varicella transmission may further elucidate the mechanisms driving the disease's seasonal trends.

    OBJECTIVES: Mumps (infectious parotitis) is an acute respiratory illness caused by the mumps virus in humans. While numerous studies have explored the impact of climate variability on mumps incidence in specific cities or regions, few have analyzed nationwide associations across multiple locations. This study aims to systematically assess the short-term effects of meteorological factors on mumps incidence across Japan over a 15-year period. STUDY DESIGN: Weekly time-series of mumps cases and meteorological factors in the period 2005-2019 were collated from the 47 Japanese prefectures. METHODS: A two-stage time-series design was employed. The exposure-response relationships between incidence of mumps, mean temperature, and relative humidity were quantified using a quasi-Poisson regression with a distributed lag non-linear model for each prefecture and the estimates from all the prefectures were then pooled using a multivariate mete-regression model to derive nationwide average associations. RESULTS: Between 2005 and 2019, 1,455,583 mumps cases were reported. Our results indicate that increased mean temperature and relative humidity are positively associated with increased mumps incidence in Japan. Specifically, using -1.3 °C as the reference temperature, the relative risk (RR) of mumps peaked at 19.4 °C, with an RR of 1.16 (95 % confidence interval [CI]: 1.08-1.24). For humidity, using 45.5 % as the reference, the RR peaked at 68.3 %, with an RR of 1.05 (95 % CI: 1.02-1.09). CONCLUSIONS: Our findings demonstrate the influence of heat and humidity on mumps incidence in Japan and underscore the need for preventive measures to mitigate the impact of climate variability on mumps transmission.

    BACKGROUND: Acute respiratory infection (ARI) and diarrhea remain critical public health concerns. In Indonesia, various interventions have been implemented to reduce the prevalence of ARIs and diarrhea among children in low- and middle-income households. Hence, the absence of detailed data on the prevalence of ARIs and diarrhea among children under five in low- and middle-income households in Indonesia restricts the formulation of targeted health interventions and policies. This study sought to evaluate the prevalence of ARI and diarrhea while examining modifiable risk factors related to malnutrition, sanitation, and social protection characteristics in Indonesia. METHODS: This study utilized a cross-sectional design based on data from the Nutrition Status Survey 2022 covering 514 districts/cities in Indonesia. It analyzed 289,631 children under five years out of 334,848 households with low and middle wealth indices. Multivariable binary logistic regression analysis was employed to calculate the risk associated with cases of ARI and diarrhea. RESULTS: The prevalence of ARI and diarrhea among low- and middle-wealth households were 5.7% and 6.0%, respectively, with infants under six months being the most vulnerable group to these infections, including malnutrition. The most significant risk factors for ARI and diarrhea are unclean cooking fuel [adjusted odds ratio (aOR) = 1.53, 95% CI 1.47-1.60] and shared toilet facilities (aOR = 1.45, 95% CI 1.38-1.51), with households using shared toilets having 1.45 times higher risk of diarrhea (aOR = 1.45, 95% CI 1.38-1.51) compared to those with private access. Additionally, households lacking social protection support are also at increased risk for these infections and malnutrition issues. CONCLUSIONS: This study revealed a notable prevalence of ARI and diarrhea among low- and middle-wealth households, particularly affecting infants under six months. Vulnerable children, especially those who were stunted or underweight, and households lacking sanitation and social protection faced heightened risks for these health issues.

    BACKGROUND: This study aimed to assess the antibody response to SARS-CoV-2 vaccines among healthcare workers (HCWs) from multiple outpatient clinics in Japan, examining the effects of baseline characteristics (e.g., sex, age, underlying condition, smoking history, occupation) and prior infections. METHODS: A total of 101 HCWs provided serum at four time points between October 2020 and July 2023. HCWs received two to six doses of mRNA vaccine (BNT162b2 or mRNA-1273). Anti-nucleocapsid (N) and anti-spike (S) IgG antibodies against the ancestral Wuhan strain were measured using the Abbott Architect™ SARS-CoV-2 IgG assay. Univariate and regression analysis evaluated factors such as past infections, age, sex, smoking, underlying condition, and occupation. RESULTS: After four to six doses, the median anti-S IgG titer in uninfected HCWs was 1807.30 BAU/mL, compared to 1899.89 BAU/mL in HCWs with prior infections. The median anti-N IgG titer was 0.10 index S/C in uninfected HCWs and 0.39 index S/C in infected HCWs. HCWs with prior infection had anti-S IgG titers 1.1 to 5.8 times higher than those without. Univariate and multivariate analyses indicated infection and vaccination significantly increased anti-S and anti-N IgG titers. Age, sex, smoking history and occupation did not influence antibody titers while underlying conditions were associated with lower anti-N IgG titers. CONCLUSIONS: Infection and vaccination were strongly associated with an increase in anti-S and anti-N IgG titers; however, the impact of hybrid immunity appeared to be limited and varied depending on the timing of the sampling. These findings provide valuable insights for developing personalized vaccination strategies and future vaccine development.

    Introduction Climate change is a decisive factor affecting human health. While many epidemiological studies have investigated the acute impacts of ambient temperature on mortality and morbidity, the global burden of infectious gastroenteritis linked to temperature changes remains largely unexplored. Therefore, we aimed to examine the exposure-response associations between ambient temperature and infectious gastroenteritis incidence throughout Japan and quantify the temperature-related morbidity burden. Methods Weekly time-series data from 2000 to 2019, encompassing meteorological factors and infectious gastroenteritis cases, were collected from all 47 Japanese prefectures. A two-stage time-series design was employed. In the first stage, quasi-Poisson regression models combined with distributed lag non-linear models were used for each prefecture. In the second stage, a multivariate meta-analysis was conducted to derive national estimates. The attributable fractions were determined for both low and high temperatures, categorized as temperatures below or above the minimum risk temperature, respectively. Results The analysis included 19,571,939 infectious gastroenteritis cases. The exposure-response association between temperature and infectious gastroenteritis cases was non-linear, exhibiting an approximate M-shaped relationship. Overall, 51.7% (95% empirical confidence interval (eCI): 42.6, 58.4) of infectious gastroenteritis cases were attributable to non-optimal temperatures in Japan. The attributable fraction to low temperatures was 47.6% (95% eCI: 38.5, 54.2), whereas that of high temperatures was 4.1% (95% eCI: 2.4, 5.5). Conclusion The majority of the temperature-related infectious gastroenteritis burden in Japan was attributable to lower temperatures. Our findings indicate that public health strategies aimed at mitigating the burden of infectious gastroenteritis should take temperature levels into account.

    1. COVID-19と気候変動との非線形遅延関連構造の解析
    COVID-19は世界の公衆衛生に多大な影響を及ぼしているにもかかわらず、このウイルスが人から人へどのように感染するのか、また、どのような環境・気象条件がこのプロセスを促進するのかについてはほとんど知られていなかった。本研究では、分布ラグ非線形モデルを用いて、2021年に欧州を中心に開発された、ケース時系列設計（Case Time Series Design）というモデリング方法を応用し、全国47都道府県における気象因子（気温と絶対湿度）と実効再生産数（一人の人が何人に感染させるか）との間の関連構造を体系的に解析した。モデリング解析の結果から、気温の上昇と絶対湿度の上昇は共に実効再生産数の低下と非線形に関連することが明らかになった。 加えて、効果修飾の検討から、気温の実効再生産数への影響は絶対湿度が高いほどより増大する可能性が示唆された。この知見は、環境・気象条件が伝播に及ぼす限られた役割を浮き彫りにし、地域的な流行の不均一性の形成メカニズムに新たな示唆を与えた。

    2. COVID-19流行前後のインフルエンザ及びRSウイルス感染症の疫学
    COVID-19流行後の2023年夏に始まったインフルエンザの大規模な流行に着目し、疫学像の変化をLetterとして報告した。日本では、ワクチン供給量は十分であるものの、より脆弱な小児や高齢者においては疾病負荷は未だ大きく、今後のCOVID-19以降の流行予測が重要であることを改めて強調した。加えて、RSウイルス感染症に関しては、COVID-19以前における気象因子（気温、相対湿度、降水量、風速）との関連を改めて精査を行なった。その結果、気温、相対湿度、降水量の上昇や、風速の減少は、RSウイルス感染症の発生の増加と関連している事が明らかになった。この結果は、COVID-19流行以前やF蛋白ワクチン接種以前の時代におけるRSウイルス感染症の基礎的な疫学情報を提示する。
