    対象:過去5年間の当院ICUのCPUにデータが残っている5日以上の機械的人工呼吸管理を受けた170症例(人工呼吸からの離脱成功145例,失敗25例)を対象とした。人工呼吸からの離脱成功は初回の試行のみでの評価とし,気管切開を試行した症例は除外した。
    Input & Output data:体重,身長,BMI,性別,入院時検査の呼吸機能(%VC,%FEV_<1.0>),人工呼吸開始前のPaO_2/FIO_2(PF ratio),ICU入室時のAPACE II score,人工呼吸離脱時の血液ガス(PaO_2,PaCO_2,pH)PaO_2/FIO_2(PF ratio),呼吸数,1回換気量,分時換気量(1分間の平均値),平均血圧,心拍数,鎮静の有無の18項目をinput, outputを人工呼吸からの離脱の成否として,ニューラルネットワーク(ANN)を構築した。ANNはNeuroSolutionsTM4.0(NeuroDimension, USA)を使用,Multiple perception layerで構築,170症例のデータでtrainingを行った。ただし,人工呼吸離脱時の呼吸モードはCPAP,PEEP3cmH_2O,Pressure Support 5cmH_2O,血液ガスは抜管直前,呼吸数,1回換気量,分時換気量,平均血圧,心拍数は抜管前30分前の値とした。次に,15症例で,構築したANNを用いて人工呼吸からの離脱の成否について前向きに予測した。
    結果:Training:1000回のtrainingで,linear correlation coefficient(成功0.9998;不成功0.9996),%Correct(成功100%;不成功100%)の好結果を得た。離脱予測:15例中2例の離脱失敗があり,失敗予測(0.999,0.997),成功予測(0.06,0.00)で,正確に予測可能であった。
    結論:ANNを用いて,人工呼吸器からの離脱の成否の予測は可能である。
