    Abstract

    Background

    Masticatory muscle tendon-aponeurosis hyperplasia (MMTAH) is a relatively newly identified clinical condition that manifests as trismus with a square-shaped mandible. Herein, we report a case of MMATH that was initially misdiagnosed for polymyositis due to trismus and simultaneous lower limb pain, with literature review.

    Case presentation

    A 30-year-old woman had a history of lower limb pain after exertion for 2 years. Initial physical examination had been performed at the Department of General Medicine in our hospital. There was also redness in the hands and fingers. Although polymyositis was suspected, it was denied. The patient visited our department for right maxillary wisdom tooth extraction.

    Clinical examination revealed that the patient had a square-shaped mandible. The maximal mouth opening was 22 mm. There was no temporomandibular joint pain at the time of opening. Furthermore, there was awareness of clenching while working. Panoramic radiography revealed developed square mandibular angles with flattened condyles. Computed tomography showed enlarged masseter muscles with high-density areas around the anterior and lateral fascia. Magnetic resonance imaging also showed thickened tendons and aponeuroses on the anterior surface and inside bilateral masseter muscles. Finally, the patient was diagnosed with MMTAH. Bilateral aponeurectomy of the masseter muscles with coronoidectomy and masseter muscle myotomy was performed under general anesthesia. The maximum opening during surgery was 48 mm. Mouth opening training was started on day 3 after surgery. Histopathological examination of the surgical specimen showed that the muscle fibers were enlarged to 60 μm. Immunohistochemistry testing for calcineurin, which was associated with muscle hypertrophy due to overload in some case reports, showed positive results. Twelve months after surgery, the mouth self-opening and forced opening were over 35 mm and 44 mm, respectively.

    Conclusions

    Herein, we report a case of MMATH. Lower limb pain due to prolonged standing at work and overload due to clenching were considered risk factors for symptoms onset of MMATH.

    BACKGROUND: Lip closing functions have never been evaluated from the viewpoint of elastic properties. OBJECTIVES: The purpose of the present study was to investigate the behavior of the lower orbicularis oris muscle during a button-pull exercise to measure lip closing force and quantitatively evaluate its elastic properties using sonographic elastography. METHODS: Appropriate compression loads for elastography were randomly measured on one of three types of acoustic couplers on three examiners. Compression tests were performed on three types of acoustic couplers within the appropriate compression load. Using the acoustic coupler with the lowest elastic modulus, the strain ratio of the lower orbicularis oris muscle during lip closing was measured, and elastography was performed on six males under tension loads of 0-8 N. RESULTS: The intraclass correlation coefficient (1, 3) for the tension load of 0 N was 0.81. Elastography showed that the strain ratio values increased significantly (p < 0.05) as the tension load increased. CONCLUSIONS: Combining the data obtained from lip closing test devices and sonographic elastography enabled the muscle performance to be evaluated objectively and accurately.

    OBJECTIVES: The present study was performed to investigate the mineral density distribution in enamel and dentin for both permanent and primary teeth and to establish the standard density per tooth type using micro-computed tomography (CT). METHODS: Fifty-seven extracted human teeth (37 permanent, 20 primary) were evaluated in the present study. The enamel and dentin mineral densities in the extracted teeth were measured using micro-CT. Cubic regression curves were used to determine the mineral density distribution in the enamel and dentin for each tooth type. RESULTS: The mean values, distributions, and regression equations of the mineral densities were obtained. The mean mineral density values for permanent enamel and dentin were significantly higher than those for their primary counterparts for each tooth type. CONCLUSIONS: In the present study, we demonstrated the distribution of mineral density in sound enamel and dentin and attempted to determine the standard mineral density for each tooth type using micro-CT. The mineral density distributions found in this study contribute to our understanding of the mechanical properties of enamel and dentin. A positive correlation suggests that the systemic bone mineral density could be predicted based on the analysis of exfoliated teeth, such as in patients with hypophosphatasia. The present results may be useful in establishing a numerical standard for the mechanism involved in root fracture and for early detection of root fracture risk.

    <p> The causes of xerostomia are diverse and include Sjӧgren's syndrome, psychological stress, and side effects of medications. In some patients, the cause may be multifactorial (e.g., psychological stress and side effects of medications, or irradiation and mouth breathing). No unified diagnostic criteria for xerostomia have been established; therefore, the criteria used in clinical practice differ among dentists. Furthermore, it is difficult to diagnose multiple causes using a flow chart-type diagnostic sheet. Therefore, we created a new diagnostic sheet that is useful for multiple causes of xerostomia. In the present study, we used this sheet to diagnose new patients in our xerostomia clinic. The cause of xerostomia was an autonomic nerve disturbance (61.4%); other causes included the side effects of medications (57.3%), mouth breathing (36.8%), Sjӧgren's syndrome (15.9%), dental psychosomatic disorder (14.5%). The degree of coincidence of diagnosis was very high (κ: 0.99, <i>p</i>< 0.001). A total of 164 patients (74.5%) had plural diagnosis. The results of this study indicate that multiple factors are likely related to xerostomia. Therefore, the use of a diagnostic sheet that can facilitate multiple diagnoses in patients with xerostomia seems necessary. </p>

    <b>Objective</b>: In order to establish a response indicator for predicting the treatment effects of oral appliances in patients with obstructive sleep apnea syndrome, we examine the utility of synchronous real-time dynamic ultrasound monitoring of the motion of the hyoid bone and the mandibular condyle during anterior mandibular movement.<br><b>Materials and methods</b>: Eight healthy volunteers, including three females and five males, were recruited for this study. None of the subjects had a history of sleep-related disorders. The synchronous real-time dynamic monitoring of mandibular protrusion was performed using two ultrasound systems with dedicated intraoperative probes. We measured five consecutive movements of the lateral portion of the mandibular condyle as an indicator of the anterior movement of the mandible and five consecutive movements of the lateral edge of the hyoid body as an indicator of the caudal movement of the hyoid bone.<br><b>Results</b>: The variation coefficient of the response ratio, which was defined as the ratio between hyoid caudal movement and mandibular anterior movement, was smaller than the amount of hyoid caudal movement in every subject. The mean amount of hyoid movement ranged from 2.0mm to 9.0mm, and the mean response ratio ranged from 0.28 to 0.87. The mean variation coefficient ranged from 5.2% to 19.5% for hyoid movement and from 4.6% to 16.1% for the response ratio.<br><b>Conclusion</b>: It was speculated that interpersonal comparisons could be performed using representative individual response ratio values as an indicator of the treatment effects of oral appliances.

    Most cases of hypophosphatasia (HPP) exhibit early loss of primary teeth. Results of microcomputed tomography (micro-CT) analysis of teeth with HPP have rarely been reported. The purpose of the present study was to describe the mineral density distribution and mapping of exfoliated teeth from an HPP patient using micro-CT. Four exfoliated teeth were obtained from a patient with HPP. Enamel and dentin mineral densities of exfoliated teeth were measured on micro-CT. The mean values of enamel and dentin mineral densities in mandibular primary central incisors with HPP were 1.61 and 0.98 g/cm3, respectively. The corresponding values in the mandibular primary lateral incisors were 1.60 and 0.98 g/cm3, respectively. Enamel hypoplasia was seen in the remaining teeth, both maxillary and mandibular primary canines and first and second molars. Micro-CT enables nondestructive, noninvasive evaluation and is useful for studying human hard tissues obtained from patients.

    The aim of this study was to determine changes in overnight respiratory function and craniofacial and pharyngeal airway morphology following orthognathic surgery. The subjects were 40 patients in whom mandibular prognathism was corrected by orthognathic surgery: a one-jaw operation in 22 patients and a two-jaw operation in 18 patients. Morphological changes were studied using cone beam computed tomography immediately before surgery and at more than 6 months after surgery, and the apnoea-hypopnoea index (AHI) was measured with a portable polysomnography system. Pharyngeal airway volume was decreased significantly after surgery, especially in the one-jaw operation group. AHI was not changed significantly after surgery in either group, although AHI in one patient in the one-jaw operation group was increased to 19 events/h. There was no significant change in pharyngeal airway morphology in that patient, but he was obesity class 1 and was 54 years old. In conclusion, some patients who are obese, have a large amount of mandibular setback, and/or are of relatively advanced age may develop sleep-disordered breathing after mandibular setback; a two-jaw operation should therefore be considered in skeletal class III patients who have such risks because it decreases the amount of pharyngeal airway space reduction caused by mandibular setback surgery.

    OBJECTIVE: The aim of this study was to evaluate the usefulness of hypothetical monoenergetic images after dual-energy computed tomography (DECT) for assessment of the bone encircling dental implant bodies. STUDY DESIGN: Seventy-two axial images of implantation sites clipped out from image data scanned using DECT in dual-energy mode were used. Subjective assessment on reduction of dark-band-like artifacts (R-DBAs) and diagnosability of adjacent bone condition (D-ABC) in 3 sets of DECT images-a fused image set (DE120) and 2 sets of hypothetical monoenergetic images (ME100, ME190)-was performed and the results were statistically analyzed. RESULTS: With regards to R-DBAs and D-ABC, significant differences among DE120, ME100, and ME190 were observed. The ME100 and ME190 images revealed more artifact reduction and diagnosability than those of DE120. CONCLUSIONS: DECT imaging followed by hypothetical monoenergetic image construction can cause R-DBAs and increase D-ABC and may be potentially used for the evaluation of postoperative changes in the bone encircling implant bodies.

    Objectives The aims of this study were to confirm the course of the anterior superior alveolar nerve (ASAN) canal in maxillary bone on CT images and to clarify the components of its contents to provide new evidence for neurovascularization of the anterior jaw bones.Methods The heads and two jaw bone specimens (maxillae) of three formalin-perfused cadavers were examined. The ASAN canal course was verified on cone-beam computed tomography (CBCT) images of the heads. Subsequently, the canal structures branching from the inferior orbital canal were dissected macroanatomically and compared with the CBCT images. Microanatomically, the ASAN canal was visualized in two bone specimens from the infraorbital region using micro-computed tomography (micro-CT). To verify the micro-CT findings, each specimen was sectioned for comparison with the histological observations.Results The gross anatomy revealed close correspondence between the course of the ASAN canal on CBCT images and that of the neurovascular bundle dissected from the canal structures branching from the inferior orbital canal. Microscopically, it was verified on micro-CT images that the ASAN canal contained neurovascular bundles including nerve bundles, arteries, and veins.Conclusions We confirmed that the canal-like structure in the anterior maxillary bone on CT images is the ASAN canal. It should be noted that the ASAN canal is filled with neurovascular structures. The present findings may provide useful information for clinicians assessing potential risks prior to anterior jaw bone surgical procedures.

    本研究は、Hellmanの歯齢の各段階において、下顎骨における下顎管の位置関係とその変化を画像解析から検討した。実験には、Hellmanの歯齢各期のヒト乾燥下顎骨各5個体、各個体を左右側計測し、計100個体について画像解析を行った。撮影には歯科用コーンビームCT撮影装置ファインキューブを使用した。画像解析には高速3次元画像解析装置Virtual Place Advance PLUSを使用し、下顎孔からオトガイ孔にかけて1.0mm間隔のパラアキシャル像を取得。これを再構成して前頭断像を得た。得られた各前頭断像について、解析・検討を行った。(著者抄録)

    The present study was performed to investigate the muscle mass and muscle quality of the tongue and suprahyoid using ultrasonography and to examine the association between these and oral function and nutritional status. The subjects were 10 elderly female and 8 healthy young females. Physical characteristics and ultrasonography measurements were compared. The group of elderly females had a significantly higher geniohyoid muscle brightness and masseter muscle brightness than that observed in the group of the young females. These results may explain the association between the geniohyoid muscle and the swallowing function. Our study suggests that the muscle quality assessment with ultrasonography is useful as an observation of sarcopenia of the oral region. Further research is needed to determine the sarcopenia index.

    The aim was imaging of the microvascular distribution in the mandibular bone marrow using Dual Energy CT Imaging (DEI). Histopathological specimens of the mandibular bone and pre-operative CT images from the patients who underwent the resection of the mandibular disease were used. The extent of the microvascular distribution within the bone marrow on the histological specimen was subjectively assessed. Multi-Planar Reconstruction Images generated from the pre-operative CT images were compared with the histological findings. Creation of optimal CT images for analysis of the microvascularization in the bone marrow of the mandible was unsatisfactory. The bone marrow area was extremely small to visualize on CT images even with DEI. The area of adipose tissue was predominantly larger than that of microvessels distribution on histological images. It was conceivable that such microvascularization could not make the clear density variations on CT images.
