Bone fragility and atypical femoral fractures in SLE: role of disease activity, infection, and treatment.

INTRODUCTION: To determine the incidence and risk factors of fragility fractures and atypical femoral fracture (AFF)-related events in patients with systemic lupus erythematosus (SLE) receiving long-term glucocorticoid (GC) therapy. MATERIALS AND METHODS: A retrospective analysis was conducted of 170 SLE patients followed from 2016 to 2023. Data on GC use, bisphosphonate (BP) therapy, bone-related events, and clinical characteristics were collected. Risk factors for fragility fractures and AFF-related events, including localized periosteal thickening (LPT), were analyzed using multivariate logistic regression. RESULTS: Although the median daily dose of prednisolone decreased over time, 82.9% of patients still met the criteria for pharmacologic intervention for GC-induced osteoporosis in 2023, and most continued to receive > 5 mg/day of GC. The median duration of BP therapy was 10.4 years, with 69 patients maintaining BP treatment throughout the observation period (median, 12.5 years). Fragility fractures and AFF-related events occurred in 7.6% and 5.8% of patients, respectively. Fragility fractures were independently associated with SLE flares and infection-related hospitalizations, whereas AFF-related events were significantly associated with prolonged BP use. CONCLUSION: Despite a gradual reduction in GC dosage, many patients with longstanding SLE remain at elevated risk for fractures. The comparable frequencies of fragility fractures and AFF-related events highlight the clinical relevance of both complications. The prevention of SLE flares and infections may contribute to lowering the risk of fragility fractures. In addition, careful monitoring for AFF/LPT is warranted in patients receiving long-term BP and GC therapy.

Calcium and magnesium both work together and against each other by sharing homeostatic regulatory systems in the kidneys. Previous studies suggested that these minerals and their intake ratio were associated with chronic health conditions such as cardiovascular disease. Our objective was to investigate the association of calcium and magnesium intake and their intake ratio with albuminuria. This cross-sectional study analyzed data from a Japanese community-based cohort comprising 6,849 individuals aged ≥40 years (mean age 68.8 years, 51.3% women). Energy-adjusted dietary intake of calcium and magnesium and the calcium-to-magnesium intake ratio were derived from a validated food frequency questionnaire. The outcome was spot urinary albumin-to-creatinine ratio (ACR, mg/g). Linear and logistic regression analyses were performed with adjustments for potential confounders. The analytic population had 1.65 and 73.5 mL/min/1.73 m2 as median intake ratio and estimated glomerular filtration rate, respectively. In multivariable linear regression analysis, lower intakes of calcium and magnesium were associated with the log-transformed ACR but, after mutual adjustment, only calcium intake was independently associated with the ACR (regression coefficient -0.084; 95%CI, -0.149, -0.019). A lower intake ratio was associated with the log-transformed ACR (regression coefficient, -0.085; 95%CI, -0.150, -0.021). These associations were evident overall and in male participants. Although magnesium intake was associated with albuminuria only before accounting for calcium intake, calcium intake and calcium-to-magnesium intake ratio demonstrated robust associations. Our findings support that the importance of calcium intake and its balancing with magnesium might extend to kidney health, especially albuminuria.

Comprehensive Bibliometric Analysis of Uremic Toxin Research 国際誌

Uremic toxins accumulate as kidney disease progresses, contributing to diverse systemic disorders. Despite numerous studies, no comprehensive mapping has been performed. In this study, keywords related to uremic toxins were quantitatively analyzed using bibliometric methods to clarify research trends, key molecules, and unresolved challenges. Literature and molecular data on uremic toxins and chronic kidney disease were retrieved from the Web of Science and the European Uremic Toxin Work Group (EUTox) databases. Network, citation burst, and keyword frequency analyses were performed using KeyWords Plus. In total, 3302 articles were identified, showing an increasing trend. Citation burst analysis revealed a growing interest in gut microbiota-related topics (burst strength = 15.21), whereas keyword frequency analysis indicated that indoxyl sulfate (566 articles) and p-cresyl sulfate (537 articles) were the most studied toxins. Toxins such as trimethylamine-N-oxide have gained attention over the past 5 years. Analysis of the EUTox database identified 24 protein-bound uremic toxins; among the 94 toxins with unreported clinical toxicity, 15 molecules, including osteocalcin and quinolinic acid, were investigated in <5 studies. These findings suggest that the gut microbiota and related uremic toxins are current research focuses; however, further investigation of underreported uremic toxins is required to define their clinical significance.

BackgroundInfections can make it difficult to continue peritoneal dialysis (PD). Nontuberculous mycobacteria-associated PD (NTM-PD) infections, while rare, frequently pose a treatment challenge due to their intractable nature and the lack of established therapeutic guidelines. As a result, we aimed to investigate the clinical characteristics of NTM infections in patients undergoing PD.MethodsWe retrospectively examined consecutive patients with NTM-PD infections from 2012 to 2022. The cases were identified through microbiological records. The primary outcomes were all-cause mortality and transition to hemodialysis. Secondary outcomes included treatment duration and antimicrobial regimens. Outcomes were compared across different NTM species and between cases with and without infectious disease (ID) consultation.ResultsAmong 177 patients undergoing PD, we identified 22 NTM infections in 20 patients. The predominant species were M. chelonae (36%), M. fortuitum (36%), and M. abscessus (23%). Twelve patients were transitioned to hemodialysis, with no mortality. All M. abscessus infections (n = 5) required transition to hemodialysis, compared to 46% in other species. ID consultation (n = 15) was linked to more frequent antimicrobial susceptibility testing (60% vs. 0%, p < .05), longer treatment duration (5.7 vs. 1.2 months, p < .05), and increased use of combination therapies (100% vs. 43%, p < .05). However, ID consultation did not affect the frequency of transition to hemodialysis.ConclusionEarly identification of NTM species and timely ID consultation can help optimize management strategies for these challenging infections.

Estimating the impact of pill burden on health utilities in hemodialysis therapy. 国際誌

Patients undergoing hemodialysis often require numerous medications because of comorbidities including hyperphosphatemia, diabetes, and anemia, coupled with water restrictions. Reducing pill burden may alleviate psychological stress and improve quality of life (QOL). However, standardized instruments such as the EuroQOL 5 Dimensions inadequately measure these effects. We investigated the impact of pill burden on utility scores using a vignette-based time trade-off (TTO) method. Members of the general population were interviewed in person using vignettes representing seven health states with varying daily pill burdens (2, 3, 6, 9, 12, 24, and 36 tablets) in the context of hemodialysis patients, including disease characteristics, symptoms, and daily life restriction. Utility scores were evaluated using the TTO method and the impact of pill burden on QOL was estimated. The mean age (44.5 years) and sex distribution (50.5% male) of the 107 participants closely matched the general population of Japan. Utility scores decreased with higher pill burdens, ranging from 0.373 (0.579) [mean (standard deviation)] for 2 tablets/day to 0.239 (0.568) for 36 tablets/day. Compared with 2 tablets/day, the utility score for 6 tablets/day was significantly lower [mean (standard deviation) difference: 0.046 (0.223), P = 0.034, paired t-test)], with the difference showing a more significant as the number of tablets increased. Subgroup analyses by sex and age showed consistent trends with no significant differences between groups. A higher pill burden was associated with lower utility scores, highlighting its negative impact on QOL in patients undergoing hemodialysis. Reducing pill burden may improve QOL and address challenges related to water restrictions.

Dual Renal and Cardiac Phenotypes Associated with Rare Variants Inherited from Both Parents.

We herein report a woman with autosomal dominant Alport syndrome (ADAS) with a family history of left ventricular noncompaction cardiomyopathy (LVNC). Exome sequencing identified a rare heterozygous variant in COL4A4, NM_000092.5: c.2510G>C (p.G837A), and a novel heterozygous variant in ACTC1, NM_005159.5: c.922T>C (p.Y308H), as causes of ADAS and LVNC, respectively. The cardiac phenotype was presumed to have been inherited paternally, whereas the renal phenotype was assumed to be maternally inherited. These two variants independently contributed to clinical phenotypes. This case highlights the clinical significance of comprehensive genetic testing for facilitating the precise diagnosis of rare and complex hereditary disorders.

Impact of upacicalcet on bone metabolism in hemodialysis patients with secondary hyperparathyroidism: a post-hoc analysis. 国際誌

Secondary hyperparathyroidism (SHPT) is a complication prevalent among patients undergoing hemodialysis (HD). Upacicalcet, a novel intravenous calcimimetic agent, has demonstrated efficacy in improving bone turnover by suppressing PTH production. However, the influence of baseline bone metabolism on the efficacy of calcimimetics remains unclear. Therefore, we aimed to evaluate the efficacy of upacicalcet on PTH suppression and changes in bone turnover based on bone-specific alkaline phosphatase (BAP) levels. This study involved a post-hoc analysis of data from a phase 3, placebo-controlled, double-blind trial evaluating the effect of upacicalcet in HD patients with SHPT. Patients were categorized into 3 groups based on tertiles of baseline serum BAP levels. Key biomarkers, including serum levels of intact PTH (iPTH), BAP, tartrate-resistant acid phosphatase-5b (TRACP-5b), and BAP/TRACP-5b ratio, were measured. Percentage changes from baseline in these parameters were assessed using a mixed-effects model for repeated measures. Additionally, cases of increased serum BAP levels following upacicalcet administration were investigated. A total of 103 HD patients with SHPT treated with upacicalcet were included in the analysis. Patients were categorized into low BAP (<12.8 μg/L), medium BAP (12.8-18.8 μg/L), and high BAP (>18.8 μg/L) groups. After 24 wk of upacicalcet intervention, iPTH levels decreased across all baseline BAP groups. Serum BAP and TRACP-5b levels decreased, whereas the BAP/TRACP-5b ratio increased across all groups. However, 26 (27.4%) patients exhibited increased BAP levels at week 24 relative to the levels at baseline despite the significant reduction in PTH levels. Upacicalcet treatment reduced PTH levels in HD patients with SHPT, regardless of baseline BAP levels. The concurrent increase in the BAP/TRACP-5b ratio with upacicalcet suggests that this agent may exert direct effects on bone metabolism, in addition to its role in suppressing parathyroid activity.
