    新潟大学医学部保健学科看護学専攻助産学コースは、2000年に短期大学部専攻科から移行後、修了生を対象とした評価を実施していない。本研究の目的は、本学助産学コース修了者の就業の動向と助産学教育プログラムの成果と課題を明らかにすることである。対象者は、本コースを履修した学生全員(2004年〜2015年卒業、207名)である。ウェブ調査(質問紙調査)と、助産学プログラムへの評価についてはグループインタビュー法を組み合わせて実施した(2015年12月〜2016年3月)。記述統計と質的分析を用いて解析した。新潟大学医学部倫理委員会の承認を受けた(受付番号2390)。助産師としての就職は約8割で、高度医療施設が多数を占め、約半数は新潟県外であった。助産学プログラムに対し、主に実習で専門職者としての基本的な技術や取り組み姿勢を学び、就職後直ちに実践が期待される乳房ケア・母乳育児等への要望があった。(著者抄録)

    Many nursing students are interested in pregnancy, labor, and delivery because it is one of their possible future life events. However, with the severe decline in birthrate in recent decades, very few students have an opportunity to experience an actual delivery. The concepts of maternity nursing are difficult to understand through lectures alone without any practical experience or training. However, students need to understand the subject matter, characteristics, and\nrelated concepts in the adaptation process of starting a new family for women, children, and their family members, and the necessary support they require. We thought that it would be effective to promote students'understanding through a case study and classroom discussion, and through having them observe, participate in, and experience actual maternity care training in a hospital after the lectures. We included these themes in our educational program. We also tried to teach\nnursing students by active learning in the course'Maternal Nursing and Health Care'. We used a movie and a written case study to help students imagine the experience of a woman in a maternity ward. In the classes, students asked questions about the movie and case study, had discussions with other group members, searched for information, studied related questions by themselves, gave presentations of their learning content, and then received evaluations by other groups. Here we report an overview of our educational program.

    本研究は、身体の不調が引き起こすメンタルの不調(産後うつ等)が児童虐待、養育不全(ネグレクト)のリスク(健やか親子21(第二次))であることから、子育て期においてそれらを予防/健康増進することにより健やかな子どもの成長を支える方策を検討する。心身の不調を回復するために必要不可欠な睡眠が、両親と児の三者が相互に作用している状態を捉え、「睡眠負債」を生じさせない方略を検討することを目的としている。子育て期は、子どもの出生直後の、新たな親役割を果たすべく試行錯誤する時期から、その後の長期にわたり、子どもの発達とともに求められる親役割の変化、それにあわせて生活状態・睡眠状態の変化を試行錯誤していく。その中で、「睡眠」にはどのような相互作用があるのか/母親の睡眠不足と父親の睡眠不足はどのように解消されているのか、疲労回復が不十分な状態がどのくらい継続しているのか/「睡眠負債」がどのような影響を及ぼしているのか、どのような状況が睡眠の充足につながるかについて、検討する。
    当初、乳児期の子どもをもつ両親を対象とした睡眠状態に関する縦断的観察研究(研究1)と子育て期を終えた女性の横断的健康調査(研究2)を計画したが、研究協力者を確保するために考えていた各施設と研究代表者/研究分担者所属機関が、コロナ感染症拡大への対応がとられる中、研究1について研究方法の見直しが随時に迫られ、計画通りの遂行が困難になった。遅れながらも、倫理委員会受審、承認を得ることができた。対象者募集まで進めた。研究2については、調査内容を見直し、倫理委員会の受審の準備が整った。

    A summary of research results for each of the ST1-5 subthemes is provided below.
    ST1;The effects of neonicotinoid pesticides on rice paddy communities and the life histories of rice paddy organisms were investigated using paddy mesocosms. ST2;The effects of neonicotinoid pesticides on soil microbial communities and microbial degradation of clothianidin under submerged condition were examined. ST3;The dynamics of Clothianidin in paddy fields and drainage channels were investigated by observation, experiment, and numerical modeling. ST4;It was detected the existence of secondary human exposure and tertiary exposure from breast milk to infants. ST5;We found that fetal exposure of mice to a neonicotinoid pesticide, clothianidin, caused impaired memory formation and altered gene expression in the brain after growing.

    To improve the midwifery practice skills of new midwives’, gaze analysis and interview data were collected in advanced, knowledgeable, and experienced midwives to later create educational and visual materials in the clinical setting. A problem-solving educational program (educational materials and educational program) was developed to explore a time series of gazes, perform a perspective analysis on the focus of gazes, and qualitatively analyze the process of observation, judgment, and action contributing to situational awareness, decision-making, and actions in situations where emergency decisions in labor are necessary.
    To assess the effects of the educational program, (1) eye gaze measurements ; (2) interviews regarding observations, thoughts, and behaviors; (3) viewing of teaching materials; (4) debriefing of observations, thoughts, and behaviors; (5) eye gaze measurements; and (6) evaluation of educational material (questionnaires) were conducted on new midwives.

    This study aimed to construction of a nursing care model for improvement of QOL and maternal role attainment of pregnant women in MFICUs.We evaluated them with using the SEIQoL-DW. Pregnant women in MFICUs revealed that “relationship with family” was the most crucial QOL component. Women in the low-score group exhibited a low level of “relationship with family,” and they tended not to list “growth and health of the fetus” as a QOL component. Moreover, QOL components that suggested the difference between the routine life the women led in the MFICU and the daily life they desired were “diet” and “ability to move without restriction.”

    The low birth rate has been pointed out and wrestled as a serious social health problem for a long time. There might be the difference between actual childcare situation and the social recognition. So, we aimed to be clarified hormonal conditions the physical health among mothers before and after childbirth, and sleep disturbance among mothers and her partners objectively and subjectively.
    As the results, majority of mothers unique hormonal conditions which had not recovered before pregnant state yet after six month of childbirth, much disturbed sleep had kept to after three month even in their partners at one month after childbirth. The need including the menstruation recovery of the woman after childbirth and the health condition of the partner became clear for a health index among early child rearing period that had been overlooked until now.

    This study notes that the sleep influences especially on the women in menopause with aging. We made the sleep intervening program using the Qi-gong Chinese relax technique. The program is to estimate the change of quality in sleep by Qi-gong. We made DVD as teaching material entitled ‘Wi-gong Brings calm sleep’ (47min). The subjects are divided into experimental group and control group. For experimental group we instruct Qi-gong and Sleep-hygiene simultaneously, for control group we instructed first sleep-hygiene and 2 weeks later Qi-gong.
    As the result of data analysis there was no clear improvement of quality in sleep by sleep intervening program, but we could guess rapid going to sleep and deep sleep by Qi-gong.

    Mothers soon after childbirth are recovering physically and also taking on multiple roles. Early postpartum is an especially stressful and sensitive time for mothers as they focus on incorporating a new member into the family, lack of sleep, sleep deprivation and fatigue. However, their physical health has been relatively neglected both in research and practice. This research described the general health status of mothers in early parenthood longitudinally.
    This study revealed subjectively and objectively that mothers in early parenthood were poor physical health, disturbed sleep frequently. Getting good quality sleep should important to maintain good health conditions and to accomplish better childcare for women who are in one or more years after childbirth.

    Traditional oriental medicine is concerned with maintaining balance between internal energies. Data was collected from 328 women to measure the function of female Organ Meridians. Findings indicated a wide range of individual differences. Quantitative analysis was problematic. In many of the women in our study, the measured value of Organ Meridians of lung, spleen and liver were high. Although findings of patterns of dysfunction were low value, the difference between left and right, or greater variation. The findings of meridians of kidney and bladder in Infertile women showed low levels of energy. The reproduction regions of women undergoing infertility treatment by hormone therapy, showed indications of imbalance with the upper body. Results suggest the possibility that the flow of internal energy may be increased by appropriate care. More research is needed.

    We support women's health from the position of gathering evidence for the wellbeing of women who have cold constitution. Therefore we examined the effect of alternating hot and cold water baths. Alternating hot and cold water baths were conducted in this study, no significant changes in skin blood flow were found in the cold constitution and non-cold constitution group's circulation. As the skin temperature decreases, the recovery rate increased markedly when compared with the starting temperature before the alternating hot and cold water baths. However, by giving stimulus of alternating hot and cold water baths to the skin, irritation of the skin vessels were thought to occur. Alternating hot and cold water baths are used in private regimen or alternative medicine for cold constitution. However, their effectiveness may need further confirmation of scientific evidence and further research is needed.

    喫煙は、多くの生活習慣病の予防可能な単一因子とされる。看護職者が「禁煙支援」を行い、現在の喫煙関連疾患患者の重症化予防だけではなく、顕在化していない喫煙関連疾患の発症予防を目指していくことは重要であり、その実現に向けた検討をすることが、本研究の目的である。
    前年度までの2年間の調査では、看護学生、看護師とも「喫煙関連教育や禁煙支援スキルの不足」「保健医療専門職者として自身の喫煙行動に対する意識の低さ」が看護職者(看護学生を含む)自身の喫煙率の高さや禁煙支援未実施率と関連していることが明らかになり、日々更新される喫煙の健康影響や禁煙支援方法等に関する専門知識をどのように得ていけるか、それらを活用した実践ができるかが重要であると考えた。
    本研究の最終年度である平成17年度は、多数の看護職者が活動する医療機関での禁煙支援の実現を目指し、具体的方策を明らかにすることと、それらを実践していくことを本研究の目的と定めた。
    看護学生には喫煙関連教育と保健医療分野の専門職者となる自身の行動選択の意義の情報提供を行い、看護師には臨床看護実践へ取り込むことができる各科の特徴をふまえた対象者(患者)への禁煙支援方法の提案と実践可能かつ効果的な支援に関する議論を重ねて行うことを、実施した。さらに、我が国の権威的研究者と交流を持ち禁煙支援方法の最新情報の収集に努め情報提供に活用すること、臨床看護師や看護教育機関教員と現状の困難点について意見交換を行い、実際的な禁煙支援の提供方法の検討を深めた。これらの研究活動に関する論文を作成した。
    今後、保健医療専門職者の最大数を占める看護職者による禁煙支援のさらなる発展は必須であり、個々の看護師が高い意識をもって効果的な支援を提供していくために、看護学教育(あるいはその前の時期)からの一貫した教育・研修内容の検討が課題になっている。
