昨年度、本学の研究倫理審査に研究計画書および書類を申請し、研究実施について承認された。その後、研究分担者の協力を得て、研究実施について４施設より内諾を得た。現在、施設に研究説明を行い、インタビュー実施について調整中である。
申請者は、看護師がチームワークにおいて倫理的ジレンマを経験した場合、個人対応が多くジレンマを解決することが困難な場合がある状況を明らかにした。しかしながら、こうした状況に対する看護管理者や周囲がどのように支援してしているのか明らかにした調査は数少ない。そのため、看護チームで経験する倫理的ジレンマに対して行動を起こすために必要な因子についての看護管理者の認識を明らかにすることで、看護師が倫理的ジレンマを経験した際の組織的支援のあり方、チームワークで倫理的ジレンマに対応するための行動指針作成および倫理的ジレンマを解決に導くための環境整備に向けた基礎資料となり得る。看護師が組織的支援を得て倫理的ジレンマを解決するための行動がとれることにより、患者に提供するケアの質向上にも貢献できる可能性がある。また、看護師個々の倫理的ジレンマ対応能力の向上にも貢献できるか可能性もある。
今年度は、研究実施施設および研究参加者（３０～４０名を予定）とインタビュー日時の調整を行い、フォーカスグループインタビューを行う。随時、逐語録作成を行い、研究分担者と協力してデータ分析を実施する。また、論文執筆の準備を行う。

本研究の目的は、当事者視点のQOL評価を導入したハイリスク妊婦の支援モデルを開発することである。本研究においては、助産師を対象としたフォーカス・グループインタビュー調査を実施する計画であったが、COVID-19の感染拡大による緊急事態宣言の発令、所属機関の感染対策取り組み通知などから県を跨いだ移動について制限するよう指示があったため、計画修正の必要性が生じた。
2021年度は、COVID-19の感染拡大の影響に伴い、webを用いた個別のインタビュー調査の実施方法の修正を行い、所属機関の倫理審査委員会に研究計画の変更を行った。その後、マーケティングリサーチ・市場調査を行っている業者に対象者の募集について依頼を行い、助産師5名を対象にweb上でインタビューを実施した。インタビューデータを逐語録に起こし、質的記述的に分析を行った。その結果、対象者はハイリスク妊婦の入院における課題について、<思い描いていた妊娠生活や出産、産後の生活との差異による戸惑い>、<体調や症状に目が行ってしまいがちで、妊婦自身が大にしていることに目がいきにくい状況>、<コミュニケーションに時間をかけられないため家族関係や本人の価値観に踏み込めない>、<栄養面やカロリー重視で食べる楽しみまで追及されてない食事>、<突然の入院のため家族の調整ができず不安が大きい>が挙げられた。また、課題に対し、<不安を和らげるような声掛けを行う>、<意識的に家族の話題や世間話をする>、<大事にしていることや入院生活の要望、出産・育児に向けた要望を聞き出す>、<妊婦の希望に沿った食事変更を細目に行う>が抽出された。

A total of 13 couples were interviewed: a couple who had a child by egg donation, a couple who planned to donate eggs, and three wives. Some of the couples who did not tell their children felt guilty because they had undergone treatment that was not permissible in Japan. The reason for not telling was that Japan is not an environment that accepts diverse families, and they were worried that their children would suffer when they learned the truth. Telling couples had no small amount of anxiety about how they would accept the truth that their child had been born by egg donation as he or she grew up. The couple was concerned about how to communicate that they did not have enough information about the donor and that the child could not meet with the donor even if they wanted to. To tell or not to tell, the couple had the same idea.

The purpose of this study was to systematically construct theories that provide a philosophical foundation for the new development of narrative theory in medicine, and three hypotheses were obtained as outcomes. (1) Evidence-based medicine is a naturalism, while narrative practice is a constructionism. Both aim to solve the health problems of individual patients, but the former is based on the ethical principle of fairness and the latter on that of equity, with code of conduct providing standardized care and providing individualized care respectively. (2) The areas of interest in health care include physical functioning, life functioning, and life (or life history) integrity, and the priority given to these areas determines care provision is based on whether realism or constructionism. (3) Narrative practices can be categorized as interpretation, mediation, and intervention.

In the narrative 'place' description of interaction and dialogue, I considered from the making of a zinsei-kamishibai and the observation at a fieldwork at a Foster Home (hospice) performance,ethics workshop,nursing cafe etc.
From two workshops of clinical ethics education using narrative archives,it is necessary to create a dialogue with diverse positions and participants with values and to create a culture for advanced care planning it was thought.

The objective of this research was to clarify the status of stakeholders as the participant in discourse and the conditions of a procedurally valid discourse in nuclear policy makings for the ethical foundation of nuclear power generation by means of discourse ethics. We analyzed various documents such as investigation reports about the accident of Fukushima Daiichi Nuclear Power Plant, books, academic papers, media coverage, and public-opinion polls regarding the accident or nuclear power policies, and results of various elections in which those issues were one of the points in dispute. We clarified the nature of the technological and ethical dilemmas related to the operation of nuclear power plants. We also pointed out that the climate of opinion demanding the ethical basis of nuclear power generation by means of discourse ethics or other dialogical mothodologies for policy-makings has been diminishing from Japanese society within the six to eight years since the Fukushima accident.

There are two unique results of this research.
（1）The resilience scale of parents of children with PIMD is comprised of 7 sub-scales, which is thought to be an adaptive scale. The results have suggested the possibility that the concept of resilience can be an index to measure elements for supporting the adaptation process of the parents of the children for supporting the children with PIMD in the future. This study aimed to construction of a nursing care model for improvement of QOL and maternal role attainment of pregnant women in MFICUs.We evaluated them with using the SEIQoL-DW. Pregnant women in MFICUs revealed that “relationship with family” was the most crucial QOL component. Women in the low-score group exhibited a low level of “relationship with family,” and they tended not to list “growth and health of the fetus” as a QOL component. Moreover, QOL components that suggested the difference between the routine life the women led in the MFICU and the daily life they desired were “diet” and “ability to move without restriction.”

This study was performed on couples who received children by assisted reproductive technology (ART) such as in vitro fertilization. As a result, we discovered the relationship between wives and their husbands/males, which was taken from the standpoint of males, and the relationship between the males and the fetus grown from fertilized eggs for the first time in our country. Also, the characteristics of the pregnancy period in the couple pregnant with ART were disclosed.
It was clarified that pregnancy after ART showed marked differences from natural pregnancy in early pregnancy period and the care programs for couples specialized in pregnancy after ART require a supporting care for the transition from infertility treatment to pregnancy and the preparation of parent role attainment.

The objectives of this study are to reveal various factors which affect safety,comfort,and QOL of pregnant women during their stay at MFICU from their point of view,and to develop a comprehensive care pregnant for maternity patients staying at MFICU.Firstly,we assessed actual living situation of pregnant women and obtained their own QOL evaluations to identify factors which may affect QOL.According to the result, however,there was dissociation between MFICU,where they are obliged to spend their maternity lives,and actual care provided by medical professionals.Hence,forcus-group interviews were performed in MFICU midwives while showing them QOL obtained from patients.As we recognized the necessity of providing supports which improves the patients'QOL at MFICU in addition to safety, we discussed supporting methods which can be more effective.

The purpose of this study was to develop a childcare support program focused on perspectives of mothers (or those who are in charge of) raising children with the most severely impaired physical and mental conditions at home. According to the results, it was revealed that there were 4 elements: “children”, “mothers”, “families” and “supporters” and that they required supports according to the shift timing in the local life and to the life stage of children. In addition, it was found important for the childcare support program to offer clinical decision-making skills for medical social workers, teamwork,partnership and the mentoring system.

Our research project aimed at (1) to construct an integrated method of healthcare ethics, (2) to test the practicability of the method, and (3) to test the legal compatibility of the method. As the five-years outcome of the project, we developed a modified “four quadrants” that had been proposed by Jonsen et al to fit Japanese clinical context, and a “narrative protocol” that requires healthcare providers to carefully analyze how the case is experienced by individuals in different positions by means of collecting narratives. We also published research papers on clinical and legal analysis of ethical issues regarding Hansen’s disease (leprosy), terminal care, and genetic medicine.

This study aimed at the development of the evaluation index of palliative care for the elderly with both cancer and dementia. Our research methods were literature documentation, a nation-wide questionnaire survey of palliative care unit, interview with expert nurses, observations at palliative care centers in Sweden and at daycare centers for the elderly with dementia in Japan. The results suggested seven major elements that affect the quality of care, a conceptual framework for the quality evaluation, and some ethical issues that should be taken into account in palliative care for the elderly with both cancer and dementia. As a product of the study, we established a provisional version of the index.

The purpose of this study is to clarify the problems and the positive aspects of child care aiming at promoting mothers' well-being. I focused on mothers who assumed the responsibility of caring for children with the most severely impaired physical and mental conditions. Under this study I have obtained following findings (1)The problems of child care were divided into "particular problems at different stages of life," and "recurring problems." (2)The positive aspects of child care were divided into "the elements of the child" and "the elements of the mother." (3)As these elements are largely linked to regional support systems, the consideration of an intervention program for regional services and the needs which they serve is an important endeavor.

ハンセン病問題を「物語的正義」論の観点から再検討することが、本研究の目的である。そのために、(A)資料・文献研究と(B)聞き取り調査を併用しながら、以下の点について理論構築を行った。1)パターナリズムとしての絶対隔離政策、2)患者団体による患者の権利運動の展開とその社会的受容、3)<語り手>としての患者と<聞き手>としての知識人の乖離と責任の所在、4)<伝え手>としてのマスメディアの責任、5)病者に対する社会的な<無関心>と<偏見>の形成と存続、6)医療についての<正義>に求められるべき特質(<語り手><伝え手><聞き手>それぞれの立場の責任、および公正な意思決定の手続きとはいかなるものか)。
本年度の実績としては、理論研究と資料研究の双方で順調な成果を見た。研究代表者の宮坂は、単著『ハンセン病重監房の記録』を来年度初め(平成18年4月14日予定)に刊行する予定である。これは、本研究で行った聞き取り調査、資料調査などの結果を盛り込み、特にハンセン病療養所に設けられていた懲罰施設(とりわけ収監者が多数死亡した過酷な懲罰施設であった栗生楽泉園の「重監房」)に焦点を当てて概説したものである。そのような施設において、患者に保障されるべき裁判を受ける機会が与えられず、療養所職員の裁量罰として不当な監禁が行われた実態を明らかにし、さらには、それらがメディアや国会等でいかに論じられ、廃止に至ったかを検証した。患者らは、こうした不当な人権侵害について、戦前から訴え続けていたのだが、保健医療などの専門職、マスメディア、国会議員、および一般国民が、それを聞き入れるまでに長い時間がかかった。
共同研究者の藤野は、『近現代日本ハンセン病問題資料集成 補巻』(全8,9巻)を刊行した。これは、『近現代日本ハンセン病問題資料集成 戦前編』(全8巻)および『同 戦後編』(全10巻)に引き続き、ハンセン病問題研究の一次資料の集成であり、この問題を研究する上での重要な基礎的資料となるものと考えられる。

医療の公共政策という文脈の中で正義論を再検討し、生命倫理学における正義論を批判的に再構築することが、本研究の目的であった。そのために、1)利益と危害の明確化、2)私益と公益の内容、および両者の対立構造、3)正義に求められるべき特質を中心に検討してきた。研究方法として、(1)文献研究、(2)事例収集、(3)論理研究を、3年度にわたって平行して行ってきた。本年度は、これらの集大成として単著『医療倫理学の方法-原則・手順・ナラティヴ』を刊行した。以下にその構成を示す。
第1部 医療倫理の歴史(古代から近代の医療倫理の変遷、現代-患者の権利の時代へ)
第2部 医療倫理学の方法、基本的な概念と構造、三つの方法論-原則・手順・ナラティヴ)
第3部 死と喪失(死と喪失についてのレビュー、告知-深刻な診断を知る,それを伝えるということ、尊厳死-最後まで生きる,その人にかかわるということ)
第4部 性と生殖(性=セクシュアリティについて、生殖について、障害児の出生を「防ぐ」ということ)
第5部 患者の権利と公共の福祉(患者と第三者の利害の対立、自己危害と他者危害)
第6部 医学研究と医療資源(生体と医療資源、医療資源の配分と医療情報)
これによって、歴史的なレビューを行った上で、領域横断的な系統的な方法論を構築し、それに沿って各領域における事例と、これまでに提出されている議論を収集・整理し、個々の医療的文脈の中での指針を示すという結果を得ることができた。この成果を基礎として、正義論、物語論など、今後の生命倫理学の方法論の研究をさらに推進したい。

研究に取り組んでいた3年間の間に,看護における倫理的問題のとりあげられ方は,生命倫理学で頻用される倫理原則の適用を目的とした,どちらかと言えば観念的なものから,次第に,看護独自の立場や使命をふまえた現実的なものへと,広がりが見られるようになってきた.施設で働く看護者が実際に日々経験する思いを,倫理的視点から説明しようとする研究も増え,新たな用語も使用されるようになってきた.
しかし,わが国における看護倫理の普及は,倫理的感受性を身につけさせることを意図した看護の基礎教育の分野と,組織経営の視点から看護をとらえる必要のある看護管理の分野ではさかんであるものの,多くの臨床看護者一般にとっては依然として観念的でいかめしいイメージのままである.また,看護倫理に関する多くの研究はなされているものの,精神科看護領域における看護倫理の研究は,ほとんどないと言ってよいほどに数少ないこともわかった.
1.そこで,今年度はまず,以前に看護学生を対象に行った調査研究を再分析し,さらに調査内容を検討して,新たに調査を行った.その結果,学生はたとえ医療における倫理の学習を終えていたとしても,改めて倫理を問われると構えてしまうこと,しかし実は多くの状況を複合的に考え合わせる能力を持つことが明らかになった.これは精神科領域においても同様であり,精神科看護者の反応は「むずかしい」と口を閉ざすか,あるいは「患者の人権」「自律か安全か」といった,従来からの精神科看護での典型的なテーマを語るかのいずれであった.
2.そこで,「看護における倫理」を,平成15年度に日本看護協会から出された「看護者の倫理綱領」をふまえて広くとらえ,調査協力者の語る悩みやストレスの中から,精神科領域における倫理的なテーマを探索する記述的方法による研究を行い,精神科看護での傾向や特徴を考察した.
精神科看護における倫理的問題は,解決に即,結びつくものでは少ないが,そこで無力感におちいることなく前向きに取り組むことにより,やがては解決に至る.そのためのエンパワメントのためにも,現実的な倫理の教育が必要であることが示唆された.

本研究では、生命倫理上の事例を、複数の当事者の物語の総体として把握した上で、一定の判断が導かれる過程を検証することを目標とし、(A)生殖医療(不妊治療、障害児の中絶等)および(B)終末期医療(癌告知、延命治療の可否等)の二つの領域を中心に、当事者の「語りnarrative」を収集した。次いでこれらの「語り」を、(a)各当事者(患者、家族・近親者、医療従事者)にとってその事例がどのような経験であったか、(b)当事者は過去にどのような経験を持ち、人生の中でその事例をどのように位置づけているか、(c)当事者は選択可能な将来の物語をどのように予見している(いた)か、またどのような選択が可能である(あった)か、(d)当事者は実際の判断に際してどのような推論のプロセスをたどったか、ということに焦点を当てながら整理した。こうして得られた「物語」の集成を事例横断的に比較しながら、(a)当事者間での「物語」の固有性、(b)「物語」の選択可能性、および(c)その選択を支えた論理構造を中心に、判断プロセスを分析した。その結果、(1)経験と語りの「位置」(その経験を予見する位置、その経験の最中にある位置、その経験を通り抜けた位置、など)によって同一の経験の意味づけが変化する、(2)経験の意味づけにはいくつかの共通した構造がある(他者の物語を参照する、自己の物語の推移によって状況は「改善」されていると見なす、現在を注視して、過去や将来の経験を倭小化する、など)、(3)このような物語的構造が自己と他者(家族、支援者、医療関係者など)とのあいだで乖離している場合に、現実の行動の選択・意思決定をめぐってジレンマにつながる、といった点が示唆された。
