    Dysfunction of primary cilia leads to genetic disorder, ciliopathies, which shows various malformations in many vital organs such as brain. Multiple tongue deformities including cleft, hamartoma, and ankyloglossia are also seen in ciliopathies, which yield difficulties in fundamental functions such as mastication and vocalization. Here, we found these tongue anomalies in mice with mutation of ciliary protein. Abnormal cranial neural crest-derived cells (CNCC) failed to evoke Hh signal for differentiation of mesoderm-derived cells into myoblasts, which resulted in abnormal differentiation of mesoderm-derived cells into adipocytes. The ectopic adipose subsequently arrested tongue swelling formation. Ankyloglossia was caused by aberrant cell migration due to lack of non-canonical Wnt signaling. In addition to ciliopathies, these tongue anomalies are often observed as non-familial condition in human. We found that these tongue deformities could be reproduced in wild-type mice by simple mechanical manipulations to disturb cellular processes which were disrupted in mutant mice. Our results provide hints for possible future treatment in ciliopathies.

    Abstract

    Background

    Coordination among lip, cheek and tongue movements during swallowing in patients with mandibular prognathism remains unclear.

    Objectives

    This study aimed to identify the temporal sequences of tongue pressure and maxillofacial muscle activities during swallowing in patients with mandibular prognathism and compared characteristics with those of healthy volunteers.

    Methods

    Seven patients with mandibular prognathism (mandibular prognathism group) and 25 healthy volunteers with individual normal occlusion (control group) were recruited. Tongue pressures and masseter, orbicularis oris, mentalis and supra‐ and infrahyoid muscle activities while swallowing gel were measured simultaneously using a sensor sheet system with five measurement points and surface electromyography, respectively. Onset time, offset time and durations of tongue pressure and muscle activities were analysed.

    Results

    In the mandibular prognathism group, tongue pressure was often produced first in more peripheral parts of the palate. Offset of tongue pressure in the posteromedian and peripheral parts of the palate and maxillofacial muscle activities except for orbicularis oris were delayed. Duration of tongue pressure in the anteromedian part of the palate was significantly shorter and durations of masseter, mentalis and suprahyoid muscle activities were significantly longer. Times to onset of orbicularis oris and suprahyoid muscle activities based on first onset of tongue pressure were significantly shorter.

    Conclusion

    These results suggest that patients with mandibular prognathism may exhibit specific patterns of tongue pressure production and maxillofacial muscle activities during swallowing.

    PURPOSE: The aim of this retrospective study was to determine orthognathic surgery indicators for Japanese patients with jaw deformities using both Index of Orthognathic Functional Treatment Needs (IOFTN) and maxillofacial morphometric analysis. SUBJECTS AND METHODS: The subjects were 89 patients treated with orthognathic surgery and 92 patients treated with orthodontic treatment alone, and were classified as class I, II, or III according to the ANB angle. Based on the results for IOFTN and the results of cephalometric analysis, the indication criteria for orthognathic surgery were examined. RESULTS: In IOFTN analysis, none of patients in the orthognathic surgery group were classified as category 1 or 2, while 48% of the patients in the orthodontic treatment group were classified as category 4 or 5. The results of the cephalometric analysis of patients in classified categories 4 and 5 showed that the orthognathic surgery group had significantly greater lateral mandibular deviation in Class I cases, significantly more severe degree of mandibular retrusion in Class II cases, and significantly more severe degree of mandibular prognathism in Class III cases. The results of the logistic regression analysis showed that IOFTN was a common variable as an indication criterion for orthognathic surgery, and several different variables were also selected from the cephalometric measurements in each group. CONCLUSION: IOFTN is a highly sensitive and useful indicator as a criterion for orthognathic surgery. However, in the choice of treatment strategy, maxillofacial morphometric analyses and the patient's desired goal are important.

    BACKGROUND: Craniofacial disharmony in cases of jaw deformity associated with abnormal lateral deviation of the jaw mostly involves both the maxilla and mandible. However, it has been still difficult to capture the jaw deviation aspect in a 3-dimensional and quantitative techniques. In this study, we focused on 3-dimensional mandibular morphology and position of the condylar head in relation to the base of the skull in patients with mandibular prognathism, one of the most common jaw deformities. We used cluster analysis to quantify and classify deviation and clarified its characteristics. We also investigated the degree of correlation between those findings and menton (Me) deviation measured on frontal cephalograms, which is a conventional indicator of jaw deformity. RESULTS: Findings obtained from 100 patients (35 men, 65 women) were classified into the following three groups based on mandibular morphology and condylar position relative to the skull base. Then, reclassification using these parameters enabled classification of cluster analysis findings into seven groups based on abnormal jaw deviation characteristics. Comparison among these seven groups showed that the classification criteria were ramus height, mandibular body length, distance from the gonion to the apex of the coronoid process, and the lateral and vertical positions of the mandible. Weak correlation was also found between Me deviation on frontal cephalograms and each of the above parameters measured on 3D images. CONCLUSIONS: Focusing on mandibular morphology and condylar position relative to the skull base in patients with mandibular prognathism, we used cluster analysis to quantify and classify jaw deviation. The present results showed that the 3D characteristics of the mandible based on mandibular morphology and condylar position relative to the skull base can be classified into seven groups. Further, we clarified that Me deviation on frontal cephalograms, which has been used to date, is inadequate for capturing jaw deviation characteristics.

    OBJECTIVE: This study aimed to determine if the change in technique of soft palate closure or timing of hard palatal repair induced occlusal changes in patients with complete unilateral cleft lip and palate (CUCLP). DESIGN: Retrospective study. SETTINGS: A medical and dental hospital in Japan. SUBJECTS: A total of 96 patients with CUCLP treated with 2-stage palatoplasty were included in the study and categorized into 3 groups (G1, G2, and G3) according to the protocol used. INTERVENTIONS: G1 underwent soft palate repair using Perko method at 1.5 years of age and hard palate repair using vomer flap procedure at 5.5 years of age. Furlow method was used for soft palate repair in G2 at 1.5 years of age and hard palate repair using vomer flap procedure at 5.5 years of age. The Furlow method was used to repair the soft palate in G3 at 1.5 years of age and vomer flap procedure was used to repair the hard palate at 4 years of age. MAIN OUTCOME MEASURES: Two evaluators assessed the dental arch relationship using the modified Huddart/Bodenham (mHB) index on 2 separate occasions. RESULTS: Intra- (intraclass correlation coefficient [ICC]: 0.962) and inter-examiner (ICC: 0.950) reliability showed very good agreement. The frequency of crossbite present in the major and minor segments gradually decreased with each change in protocol. Mean segmental scores showed no significant difference between 3 protocols (P > .05). Good inter-arch alignment occurred with all 3 surgical protocols (G1:82.6%, G2:89.8%, and G3:91.7%). CONCLUSIONS: There was no significant difference in the dental arch relationship outcomes between the 3 surgical protocols. The dentition status was comparable with all surgical protocols, even after the changes.

    OBJECTIVES: The ciliopathies are a wide spectrum of human diseases, which are caused by perturbations in the function of primary cilia. Tooth enamel anomalies are often seen in ciliopathy patients; however, the role of primary cilia in enamel formation remains unclear. MATERIALS AND METHODS: We examined mice with epithelial conditional deletion of the ciliary protein, Ift88, (Ift88fl / fl ;K14Cre). RESULTS: Ift88fl / fl ;K14Cre mice showed premature abrasion in molars. A pattern of enamel rods which is determined at secretory stage, was disorganized in Ift88 mutant molars. Many amelogenesis-related molecules expressing at the secretory stage, including amelogenin and ameloblastin, enamelin, showed significant downregulation in Ift88 mutant molar tooth germs. Shh signaling is essential for amelogenesis, which was found to be downregulated in Ift88 mutant molar at the secretory stage. Application of Shh signaling agonist at the secretory stage partially rescued enamel anomalies in Ift88 mutant mice. CONCLUSION: Findings in the present study indicate that the function of the primary cilia via Ift88 is critical for the secretory stage of amelogenesis through involving Shh signaling.

    Quality of life （QOL） is now known to be one of the important prognostic factors in many clinical fields. Disease-specific quality of life is an important index as well as health related quality of life （HRQOL） to validate outcomes of medical treatments. The Orthognathic Quality of Life （OQLQ） index has been developed to measure QOL of dentofacial deformity. The aim of the present study was to develop a Japanese version of the OQLQ and examine its reliability and validity.
    <br>Materials & methods: With the permission of the OQLQ developer, the original English version of the OQLQ was translated into Japanese with a forward-backward translation method. A pilot study was carried out on a sample of 22 patients, and a validity study was done on a sample of 25 patients. The reliability was verified with Cronbach’s coefficient alpha and item-total correlation. The validity study was performed after confirming the reliability of the questionnaire, using the test-retest method. HRQOL was also measured to verify the validity of the questionnaire with SF-8, and correlation was verified with Spearman’s correlation coefficient. All of the patients who participated in this study were new patients who were diagnosed with dentofacial deformity.
    <br>Results: In the pilot study, the correlation coefficients of the item-total correlation score were −0.011 to 0.068. Some questions on oral function component showed a weak correlation, but Cronbach’s coefficient alpha of each component was 0.79-0.85, which revealed the reliability of the questionnaire. The paired t-test in the validity study showed no significant difference for most of the questions, confirming repeatability in the same patient and the validity of the questionnaire. Spearman’s correlation coefficient showed, for all four components of the OQLQ, moderate to strong correlation with some items of mental scales and MCS of SF-8.
    <br>Conclusion: We concluded that the Japanese version of the OQLQ we developed is a reliable and valid questionnaire.

    Hh signaling has been shown to be activated in intact and injured peripheral nerve. However, the role of Hh signaling in peripheral nerve is not fully understood. In the present study, we observed that Hh signaling responsive cells [Gli1(+) cells] in both the perineurium and endoneurium. In the endoneurium, Gli1(+) cells were classified as blood vessel associated or non-associated. After injury, Gli1(+) cells around blood vessels mainly proliferated to then accumulate into the injury site along with endothelial cells. Hh signaling activity was retained in Gli1(+) cells during nerve regeneration. To understand the role of Hedgehog signaling in Gli1(+) cells during nerve regeneration, we examined mice with Gli1(+) cells-specific inactivation of Hh signaling (Smo cKO). After injury, Smo cKO mice showed significantly reduced numbers of accumulated Gli1(+) cells along with disorganized vascularization at an early stage of nerve regeneration, which subsequently led to an abnormal extension of the axon. Thus, Hh signaling in Gli1(+) cells appears to be involved in nerve regeneration through controlling new blood vessel formation at an early stage.

    Wnt signaling plays a critical role in the development of many organs, including the major movable craniofacial organs tongue, lip, and eyelid. Four members of the R-spondin family (Rspo1-4) bind to Lgr4/5/6 to regulate the activation of Wnt signaling. However, it is not fully understood how Rspos/Lgrs regulate Wnt signaling during the development of movable craniofacial organs. To address this question, we examined the expression of Rspos, Lgrs, and Axin2 (major mediator of canonical Wnt signaling) during tongue, lip, and eyelid development. The expression of Axin2, Rspos and Lgrs was observed in many similar regions, suggesting that Rspos likely activate canonical Wnt signaling through the Lgr-dependent pathway in these regions. Lgr expression was not detected in regions where Axin2 and Rspos were expressed, suggesting that Rspos might activate canonical Wnt signaling through the Lgr-independent pathway in these regions. In addition, the expression of Rspos and Lgrs were observed in some other regions where Axin2 was not expressed, suggesting the possibility that Rspos and/or Lgrs are involved in non-canonical Wnt signaling or the Wnt-independent pathway. Thus, we identified a dynamic spatiotemporal expression pattern of Rspos and Lgrs during the development of the eyelid, tongue, and lip.

    <jats:title>Abstract</jats:title><jats:sec><jats:title>Purpose</jats:title><jats:p>Obstructive sleep apnea (OSA) is generally more common in men than in women, so many studies have only involved male OSA patients as subjects. However, a certain proportion of OSA patients are female, and few studies have addressed this population. Maxillofacial morphology and obesity are now being shown to be intimately connected with OSA, but the details remain unclear. The objective of this retrospective study was to identify the clinical features of OSA in men and women of different ages, with a focus on maxillofacial morphology and body mass index (BMI).</jats:p></jats:sec><jats:sec><jats:title>Methods</jats:title><jats:p>The subjects comprised 112 patients (56 men and 56 women) randomly selected from adult patients in the Niigata University Medical and Dental Hospital. We investigated the results of lateral cephalogram measurements and polysomnography (PSG) conducted before the start of OSA treatment. We then divided the patients by age (with 50 years as the cutoff value) and sex, and performed statistical comparisons among these four groups.</jats:p></jats:sec><jats:sec><jats:title>Results</jats:title><jats:p>The findings showed that the hyoid bone was lower in men aged ≥50 compared with <50, and the cross‐sectional area of the tongue was larger. This suggested that aging changes in maxillofacial morphology may be a risk factor in men. In women, however, only slight morphological differences were found between the two age groups.</jats:p></jats:sec><jats:sec><jats:title>Conclusions</jats:title><jats:p>This comparative study of sex and age groups in OSA patients suggests that the different groups have different clinical characteristics and may have different risk factors for developing OSA.</jats:p></jats:sec>

    Bisphosphonate (BP) related osteonecrosis of the jaw (BRONJ) has become widely known, but there are few reports of orthognathic surgery for jaw deformity receiving BP. We report a case of jaw deformity taking oral BP treated by orthognathic surgery. A 16-year-old man visited our department with the complaint of abnormal occlusion. He was diagnosed as having mandibular protrusion, and orthognathic surgery was planned. However, he was diagnosed multiple sclerosis at the end of presurgical orthodontic treatment, so oral steroids therapy was started and oral BP was began for prevention of glucocorticoid-induced osteoporosis. The symptom of multiple sclerosis did not appear by the therapy, and the steroids were reduced in dose. At 19 years of age, orthognathic surgery was performed by Le Fort I osteotomy and sagittal splitting ramus osteotomy after discontinuance of BP for 6 months. The bone-fixed plates were removed 9 months later and then BP was restarted. During the perioperative period, care was given to avoid infection, and BRONJ did not occur. He has been followed up for 2 years after orthognathic surgery, and no abnormality has been observed.

    The aim of this study was to clarify the characteristics of each group classified by cluster analysis based on the measurement of frontal soft tissue in patients with mandibular prongathism. The subjects were consisted with 100 patients (40 males, 60 females) diagnosed as skeletal mandibular prongathism. Frontal soft tissue was classified by cluster analysis based on these measurement results and performed multi groups comparison about the result of the measurement item in each group. The measurements were performed using the points of facial structure and facial outline and all subjects were applied the cluster analysis based on the measurements results. We got 4 clusters; group A (N=22): Patients with asymmetry of the face outline and little asymmetry of the position of facial structure ; group B (N=9): Patients with asymmetry in both position of facial parts and facial outline; group C (N=29): Patients with slight asymmetry of the facial outline and the severe asymmetry around nose and upper lip regions in facial structure; group D (N=40): Patients with symmetric facial outline and the position of facial structure. Those results suggested that each group may have a morphological characteristics. Group A has the patients with severe mandibular deviation with little maxillary deviation. In contrast, group B has the patients with severe bimaxillary deviation. Group C may have partially maxillary deviation with little mandibular deviation and group D has little face asymmetry. This classification must be helpful for treatment planning of surgical orthodontics for mandibular prognathisms with facial asymmetry.

    BACKGROUND: Craniofacial disharmony in cases of jaw deformity associated with abnormal lateral deviation of the jaw mostly involves both the maxilla and mandible. However, it has been still difficult to capture the jaw deviation aspect in a 3-dimensional and quantitative techniques. In this study, we focused on 3-dimensional mandibular morphology and position of the condylar head in relation to the base of the skull in patients with mandibular prognathism, one of the most common jaw deformities. We used cluster analysis to quantify and classify deviation and clarified its characteristics. We also investigated the degree of correlation between those findings and menton (Me) deviation measured on frontal cephalograms, which is a conventional indicator of jaw deformity. RESULTS: Findings obtained from 100 patients (35 men, 65 women) were classified into the following three groups based on mandibular morphology and condylar position relative to the skull base. Then, reclassification using these parameters enabled classification of cluster analysis findings into seven groups based on abnormal jaw deviation characteristics. Comparison among these seven groups showed that the classification criteria were ramus height, mandibular body length, distance from the gonion to the apex of the coronoid process, and the lateral and vertical positions of the mandible. Weak correlation was also found between Me deviation on frontal cephalograms and each of the above parameters measured on 3D images. CONCLUSIONS: Focusing on mandibular morphology and condylar position relative to the skull base in patients with mandibular prognathism, we used cluster analysis to quantify and classify jaw deviation. The present results showed that the 3D characteristics of the mandible based on mandibular morphology and condylar position relative to the skull base can be classified into seven groups. Further, we clarified that Me deviation on frontal cephalograms, which has been used to date, is inadequate for capturing jaw deviation characteristics.

    The tongue is an indispensable organ for good quality of life as it has various critical functions such as feeding, swallowing, pronunciation, and taste. On the other hand, the tongue is most commonly surgical removed organ in the oral cavity, since it is the most common region for cancer in dentistry. Therefore, it is expected that tongue regenerative therapy is established. However, a mechanism of tongue development remains unclear, which is essential for inducing stem cells in tongue regeneration. This study elucidated the mechanism of tongue development by analyzing microRNA-deficient mice, in which the tongue lost. Results suggest the possibility that the tongue was absent due to the overactivity of multiple signaling pathways in microRNA-deficient mice. Thus, appropriate activity levels of the signaling pathway is likely important for tongue development.

    Through theoretical research based on a literature review, we proposed a "three-fold model of competency" and categorized genericity into four types: genericity transcending subject-specificity, genericity bypassing subject-specificity, genericity rooted in subject-specificity, and meta-subject genericity.
    Alongside this, development research was conducted mainly at Niigata University Faculty of Dentistry, Tokyo City University, and Takatsuki Junior and Senior High Schools, and the effectiveness of PEPA (Pivotal Embedded Performance Assessment) and dialogical argumentation as methods of fostering and assessing competencies including knowledge and skills was examined. In addition, we conducted a qualitative research at Minerva University as an advanced case study of the formation and assessment of generic competencies transcending subject-specifity, and clarified its inner workings.

    The purpose of this study is to reproduce jaw morphology by a method called a generative design method that seeks the most suitable morphology in structural mechanics by large-scale computer calculation for jaw deformity cases (face asymmetry/jaw deviation). By applying it and identifying the unbalanced masticatory muscle action that causes the jaw deviation, we clarify the mechanical background that causes the jaw deviation. As a result, the patterns of jaw displacement were classified into seven patterns, and it became clear that the jaw displacement, which had been impossible to quantitatively analyze up to now, was essential for understanding the displacement. On the other hand, it was revealed that the generative design method, which is a large-scale analysis, is difficult to use for predicting the morphological change of the maxillofacial morphology, and further advancement of ICT is awaited.

    In the present study, we analyzed the clinical characteristics of OSA in Japanese adult OSA patients by sex and age with a focus on maxillofacial morphology and BMI. The results indicated that the clinical features of OSA vary between men and women and between different age groups, and suggest that factors such as the vertical position of the hyoid bone and the cross-sectional area of the tongue may have a greater effect than the position of the jawbone on the severity of OSA.
    In addition, our results suggest that OSA in female patients is possibly affected by factors other than maxillofacial morphology and BMI, and that female hormone tests should be included in OSA-related parameters in the case of females.

    The aim of this project was to establish the effective treatment program for orthognathic patients with skeletal maxillo-mandibular deviation to predict and reduce the jaw deformities in adolescence period. The results showed that jaw deviation was probably caused by imbalanced stress distribution on the maxilla and mandible. Especially, the presence of occlusal cant seemed to play a main role in an increase of jaw deviation. To prevent exacerbation of jaw deviation, the early correction of vertical problems on molar region will be effective in pre-adolescence patients.

    The aim of this study was to clarify the postoperative changes after orthognathic surgery from biomechanical point of view. We tried to evaluate the postoperative displacement of bony segment using finite element analysis under the situation of mastication force loading. As a result of this study, the postoperative changes of bony segment after orthognathic surgery which was observed in the clinical situation could be reproduced and the postoperative prediction using finite element analysis must be useful for diagnosis and planning for surgical orthodontic treatment.
