People-Centered Care(PCC)とは,市民がケアの主体となり,保健医療専門職(以下:専門職)とパートナーを組んで行う,個人や地域社会における健康問題の改善に向けた取り組みのことをいう。研究者らは,我が国の少子超高齢社会の課題をはじめ,グローバルな健康課題の改善に向けた市民と専門職とのパートナーシップを目指し,市民と専門職が共に考える「PCCパートナーシップ」の教材開発に取り組んでいる。今回は,その教材作成の取り組みとその成果を報告する。教材作成にあたって,PCCの研究成果を集約し,PCCに精通する専門職と当事者で教材内容と方法について検討を重ねた。その結果,PCCの定義とPCCに欠かせない市民と専門職のパートナーシップの8要素を軸に,「パート1:PCCとは(アニメーション動画)」「パート2:PCCの実践例(実写動画)」「パート3:PCCの活動事例(実写動画)」のeラーニングとDVD教材を作成した。今後,本教材の活用可能性について調査を行う予定である。(著者抄録)

AIM: We assessed the feasibility of the Birth and Parenting Planning (BPPing) program we developed to support women diagnosed with fetal anomalies. METHODS: We conducted a feasibility study using a quasi-experimental, post-test only, non-equivalent groups design. We assessed feasibility from four aspects: (1) Demand: birth plan submitting rate (primary outcome); (2) Acceptability: woman's satisfaction of care scale (SATISFACTION) and Visual Analog Scale for care satisfaction; (3) Preliminary efficacy: women's recognition of being able to express and share her hopes scale (HOPES); and (4) Safety: obstetric/neonatal outcomes and frequency of women's referral to a psychological counselor. Fisher's exact test and Mann-Whitney U-test were used for data analysis. RESULTS: We enrolled 62 women with fetal anomalies attending a perinatal medical center for antenatal check-ups. Of these, 51 women (median age: 34 years) received either the BPPing program (intervention group: n = 25) or the usual care (control group: n = 26). The birth plan submitting rates were 96% (intervention group, 24/25) and 7.7% (control group, 2/26). The intervention group had a significantly higher total score on HOPES at postpartum (U = 441.5, p = 0.027). Upon subgroup analysis based on parity, in the intervention group only, the multiparas had a higher score on the SATISFACTION item, "I would recommend the care I received to someone in a similar situation". There were no adverse events in either group. CONCLUSION: The BPPing program was feasible in supporting women diagnosed with fetal anomalies in terms of demand, acceptability, preliminary efficacy, and safety.

目的:産科医療者(助産師・看護師・医師)による妊産婦に対する共有意思決定(以下,SDM)の実態を明らかにする.方法:2021年2月,調査会社にモニター登録している医療職で産科または婦人科で女性に意思決定支援を行ったことがある者にウェブ調査を行い,意思決定支援を行った1ケースのSDMの程度を9項目のSDM-Q-Docを用いてたずねた.女性の意思決定へのかかわり方の希望と実際について,「女性は,自分だけで方針を決定したいと思っていた」等5段階でたずねた.群間差の検定にt検定および一元配置の分散分析を用いた.実施に際しては研究倫理審査の承認を得た.結果:回答者52人は,30歳代が22人(42.3%),女性が42人(80.8%),助産師が26人(50.0%)だった.SDM-Q-Docの合計は45点満点中30.4±8.7(range 3-44)で,項目別では「SDM-Q-Doc-6:私は女性に,どの選択肢を希望するのか尋ねた」が5点満点中3.60±1.36と平均値が最も高かった.SDM-Q-Doc得点は医療者の年齢(r=0.325,p=0.019)および意思決定支援の経験年数(r=0.290,p=0.039)との間に相関が認められた.女性の意思決定へのかかわり方(実際)とSDM-Q-Docの関係では,「医師と一緒に方針を決定した」場合のSDM-Q-Doc得点が最も高かった.また,SDM-Q-Docの9項目合計の素点は100点満点に換算すると67.5点であった.結論:本研究は,妊産婦にSDMを実施している産科医療者による1ケースに対する評価として,先行研究で示されている得点の上限程度であった.医療者は,年齢が高く,経験年数が長いほどSDMの実践をしており,経験を積むことがSDMの実践を可能にしていた.今後は,医療者と患者がペアになり同一の意思決定支援の場面を想定して双方から評価する研究が必要である.(著者抄録)

目的:健康課題に共に取り組む市民と保健医療専門職(専門職)とのPeople-Centered Care(PCC)パートナーシップ-16尺度を開発し,信頼性と妥当性を検討した.方法:PCCパートナーシップ尺度原案37項目を用いて市民と専門職を対象に調査し,内容妥当性の検討後,天井効果とI-T相関分析,主成分分析および確認的因子分析により,8因子構造の信頼性と妥当性を分析し,モデルの適合度の確認を行った.結果:有効回答は329部であり,尺度は16項目に精錬された.PCCの構成要素8つによる分析の結果,8下位尺度では負荷量が.88以上,寄与率は77.0%以上,Cronbach's αは.70以上が確認された.PCCパートナーシップ尺度得点は「協同効用因子」得点と正の相関が認められた.また8因子16項目の仮説モデルの適合度を確認的因子分析で検討し,整合性を認めた.結論:PCCパートナーシップ尺度16項目8因子構造を開発し,信頼性と妥当性が確保できた.(著者抄録)

Objective: this study developed a people-centered care (PCC) partnership model for the aging society to address the challenges of social changes affecting people's health and the new role of advanced practice nurses to sustain universal health coverage. Method: a people-centered care partnership model was developed on the basis of qualitative meta-synthesis of the literature and assessment of 14 related projects. The ongoing projects resulted in individual and social transformation by improving community health literacy and behaviors using people-centered care and enhancing partnership between healthcare providers and community members through advanced practice nurses. Results: people-centered care starts when community members and healthcare providers foreground health and social issues among community members and families. This model tackles these issues, creating new values concerning health and forming a social system that improves quality of life and social support to sustain universal health care through the process of building partnership with communities. Conclusion: a PCC partnership model addresses the challenges of social changes affecting general health and the new role of advanced practice nurses in sustaining UHC.

Prenatal testing has been provided in Japan over the past several decades. However, it is difficult to assess the clinical status of amniocentesis (AC) and maternal serum markers (MSM) because obstetricians can perform these tests without registration. This study aims to investigate the current clinical status of AC and MSM in Japan. We conducted a questionnaire study that was intended for a total of 5622 Japanese obstetrics/gynecology facilities during October 2013 to January 2014. The response rate was 40.8% (2295/5622). Of the 2295 facilities, 864 performed MSM (37.7%), 619 performed AC (27.0%) and 412 performed both (18.0%). The average number of MSM tests was 2.0 per month (range 0-52), and the average number of AC tests was 2.4 per month (range 0-30). Involvement of genetic professionals, such as clinical geneticists (CGs) and certified genetic counselors (CGCs), contribute to a content-rich explanation and management of difficult issues and lengthened the explanation time. Nevertheless, relatively few facilities employed these specialists (MSM: 96/864 and AC: 128/619). This is the first study to highlight the current clinical status of AC and MSM tests in Japan. Active involvement of CGs and CGCs can provide more appropriate genetic counseling for prenatal tests.

Many nursing students are interested in pregnancy, labor, and delivery because it is one of their possible future life events. However, with the severe decline in birthrate in recent decades, very few students have an opportunity to experience an actual delivery. The concepts of maternity nursing are difficult to understand through lectures alone without any practical experience or training. However, students need to understand the subject matter, characteristics, and\nrelated concepts in the adaptation process of starting a new family for women, children, and their family members, and the necessary support they require. We thought that it would be effective to promote students'understanding through a case study and classroom discussion, and through having them observe, participate in, and experience actual maternity care training in a hospital after the lectures. We included these themes in our educational program. We also tried to teach\nnursing students by active learning in the course'Maternal Nursing and Health Care'. We used a movie and a written case study to help students imagine the experience of a woman in a maternity ward. In the classes, students asked questions about the movie and case study, had discussions with other group members, searched for information, studied related questions by themselves, gave presentations of their learning content, and then received evaluations by other groups. Here we report an overview of our educational program.

Purpose: The aim of this article is to explore the interaction between the integration of genetics-genomics competencies into nursing curricula and regulatory standards. By taking a global perspective of activity in this field, we aim to develop a framework that can inform strategic planning in relation to international genetics-genomics and nursing education.
Methods: We focus our exploration around a small-scale international survey on the progress, achievements, and critical success factors of 10 countries in relation to the integration of genetics-genomics into nursing education, with exemplars from three of those countries.
Findings: Analysis of the data generated 10 themes, each with several subthemes that play a critical role in the development of genetics-genomics in nursing education and practice. The themes were organized into three overarching themes: nursing in genetics, genetics in nursing, and recognition and support. Genetics-genomics competence is not fully integrated into nursing education at an appropriate level in any country, nor was it reflected robustly in current standards for registration and licensure.
Conclusion: Strong leadership from the specialist genetics community plays a critical role in defining genetics-genomics competence but the engagement of nursing professionals at senior levels in both government and regulatory institutions is essential if nurses are to be active participants in the innovations offered by genomic healthcare.
Clinical Relevance: Safe and effective nursing practice must incorporate the needs of those with, at risk for, or susceptible to genetic-genomic conditions, as well as those who might benefit from the application of genomic technologies in the diagnosis and management of common conditions such as cancer and heart disease. The scope of such practice can be articulated though competence statements. Professional regulation defines the standard of competence that practicing nurses should demonstrate at initial registration and licensure.

自分の健康の基本となるからだに関する知識を子供に提供するためのプログラムを開発し、研究手法としてのCBPRの有用性を検討した。数名の大学教員のはじめたプロジェクトが、人づてにネットワークとして広がり、そこに定期的会合、議事録作成、連絡方法が確立し、資金支援を受け、教育プログラムの作成が進んだ。また、組織としてのシステムが定まったプロジェクトは、地域などへ対外的に活動をすることができ、内部では役割分担も起こった。

The prevalence of BRCA1/2 germline mutations in Japanese patients suspected to have hereditary breast/ovarian cancer was examined by a multi-institutional study, aiming at the clinical application of total sequencing analysis and validation of assay sensitivity in Japanese people using a cross-sectional approach based on genetic factors estimated from personal and family histories. One hundred and thirty-five subjects were referred to the genetic counseling clinics and enrolled in the study. Full sequencing analysis of the BRCA1/2 gene showed 28 types of deleterious mutations in 36 subjects (26.7%), including 13 types of BRCA1 mutations in 17 subjects (12.6%) and 15 types of BRCA2 mutations in 19 subjects (14.1%). Subjects were classified into five groups and 22 subgroups according to their personal and family history of breast and/or ovarian cancer, and the prevalence of deleterious mutations was compared with previously reported data in non-Ashkenazi individuals. Statistical analysis using the Mantel-Haenszel test for groups I through IV revealed that the prevalence of Japanese subjects was significantly higher than that of non-Ashkenazi individuals (P = 0.005, odds ratio 1.87, 95% confidence interval 1.22-2.88). Family history of the probands suffering from breast cancer indicated risk factors for the presence of deleterious mutations of BRCA1/2 as follows: (1) families with breast cancer before age 40 within second degree relatives (P = 0.0265, odds ratio 2.833, 95% confidence interval 1.165-7.136) and (2) families with bilateral breast cancer and/or ovarian cancer within second degree relatives (P = 0.0151, odds ratio 2.88, 95% confidence interval 1.25-6.64). Since the 1960s, Japanese nurses have been working as international nursing collaborators with or for the people in developing countries in order to promote health through transferring nursing knowledge and skill. However a number of nurses reported that they did not have enough competencies as international nursing collaborators. The objectives of this study were: a) to describe the experiences of Japanese international nursing collaborators whose mission was transferring nursing knowledge and skill and b) to determine their learning needs as international collaborators, and to design educational programs (graduate program) based on the results of survey. The participants of this study were twenty-seven nurses who had had an international collaborative mission and stayed in a developing country for more than one year. Semi-structured group or individual interviews were used in order to describe activities process and leaning needs as international collaborators. The purpose of this study was to clarify health information needs and information seeking behavior of working women. The participants were seven women working at three companies based in Tokyo. Two focus group interviews were conducted to collect data: one with three women and the other with four women. Results of our qualitative data analysis, three categories of characteristics of health information seeking behavior were identified : 1) awareness of and caring for one's physical and psychological well-being, 2) collecting information, and 3) utilization of information. These findings suggest that support for working women should help them become aware of and understand their physical and psychological well-being, and improve their capacity as knowledgeable users of health information.

The purpose of this study was to identify, through an integrative literature review, the challenges to and strategies for international nursing collaboration/cooperation among organizations seeking to promote the education of nurses and enhance the capacities of nursing personnel. We searched for relevant articles using CINAHL and PubMed, without time restriction. We found 31 articles with the term INTERNATIONAL and COLLABORATION or INTERNATIONAL and COOPERATION in their titles, and NURSING in their texts. Of these, 23 articles were concerned with collaborations for education and human resource development. We classified these articles into two categories focusing on their collaborative processes, outcomes, challenges and strategies. Nine articles published between 1997 and 2005 were classified in the first category: international nursing collaboration/cooperation between organizations in two developed countries. Major contents of collaboration in this category were nursing student exchange programs and human resource development for practitioners. Common challenges observed here were budget management, language barrier, and evaluation of collaborative programs. Internet was considered as a useful tool of communication. 14 articles published between 1971 and 2005, of which 11 published after 1999, were classified in the second category: international nursing collaboration/cooperation between organizations in developed countries and organizations in developing countries. Programs in this category were aimed at developing undergraduate or graduate courses/programs, training practitioners, and providing community education in the developing countries. Common challenges observed here were communication difficulties, which included not only language barrier but also lack of reliable means for communication, and understanding different cultural backgrounds. The key is to respect each culture and its beliefs and to understand roles/functions of nursing in respective country. Our conclusion is that mutual understanding between cultures is essential for successful international collaboration. Differences between cultures should be well recognized, and various ways of communication should be established.

Aim: The aim of this research study was to clarify the effect that counseling using the Ottawa Personal Decision Guide had on pregnant women in helping them to decide whether or not to undergo prenatal testing. It compared the "decisional conflict" of a group of women interviewed by a nurse using the guide with that of a second group who were only given standard genetic counseling. Methods: A group of pregnant women attending the hospital for genetic counseling was randomly divided into an intervention group and a control group. The women in the intervention group were given standard genetic counseling, followed by an interview based on the decision aid guide. The women in the control group were given only standard genetic counseling. Both groups were followed up. The primary outcome of the study was to determine the level of decisional conflict, as measured by the Japanese version of the Decisional Conflict Scale. The secondary outcome was self-esteem, based on the Rosenburg Self-Esteem Scale. The outcome indicators were obtained from responses given in patient questionnaires conducted after standard genetic counseling (pre-intervention) and after the conclusion of the decision-making process (postintervention). Results: The pre-intervention and postintervention changes in decisional conflict scores (primary outcome) indicated no significant difference between the two groups. However, with regard to postintervention decisional conflict, the rate of the response "mild to high decisional conflict" was lower for the intervention group than the control group. No significant difference between the two groups was detected for the secondary outcome of self-esteem. Conclusions: The reasons for the lack of a clear effect on the level of decisional conflict are that the pre-intervention responses for both groups were obtained after completion of the standard genetic counseling program and because follow-up conducted by the nurses took the form of care given in response to patients' needs. The use of decision aids to assist in the decision-making process had no effect on the level of self-esteem in the two groups. As a result, there was a similar level of intervention for both groups. This paper reports the development of specialist genetic nursing in the UK and Japan. Between 1970 and 1990, significant technical advances in genetic testing occurred. These advances offered many families affected by genetic conditions more choices in diagnostic, predictive, and prenatal testing. It was recognized that genetic testing could significantly alter the life of the family and that supportive health services were required to accompany such testing. Nurses began to offer specialist services in this setting, but considerable professional development was required to ensure practitioners were competent. In the UK, the profession grew slowly over several decades. Concern to establish minimum standards of practice in this new professional setting culminated in the establishment of a set of competencies and a formal registration process. By comparison, in Japan, developments in genetic nursing have occurred over a shorter period of time, but minimum standards of competence already have been agreed upon. © 2006 The Authors

This article describes the January 24-25, 2004 international workshop proceedings held at St. Luke's College of Nursing. The purpose of the workshop was to share the 2002 and 2003 findings, from the research study titled 'Design of Educational Programs for Japanese Nurses whose Mission is to Transfer Nursing Knowledge in Developing Countries' with three international advisors. The focus of the research was to develop graduate, continuing and postgraduate programs in order to prepare those Japanese international nursing collaborators who engage in technical transfer activities in developing countries. Three groups conducted this research. Groups one and two focused on 'Personn el development Program' and 'Basic Nursing Educational Program'. Group three focused on the Process of Technical Transfer. Each of the groups presented their findings and proposed preliminary educational guidelines or educational programs. The members of the workshop provided additional suggestions and ideas for the final year of this study in 2004.

The purpose of this study was to survey experienced genetic health care provider's perceptions of the level of practice competency required for Basic Level and Advanced Level Genetic Nursing. Basic level refers to the general nurse and the advanced level refers to the genetic nurse. The respondents who agreed to participate in the study were 491 nurses and physicians who were involved in genetic care in 113 institutions of 40 prefectures. The questionnaire we developed was comprised of 89 items in seven areas of competency for genetic nursing. The respondents were asked to assign the competency items to one of the following nursing levels: (a) General Nurse; (b) nurse who specializes in Genetic Nursing; or (c) not appropriate for the nurse. Data were collected from January to March 2001. The Delphi Method was used to explore areas of agreement regarding competency fit among the respondents. Three iterations of the survey, resulting in a response rate of 60%(295 respondents), provided sufficient clarity for assigning levels of competence. <BR>Respondents selected the following areas of competence for the General Nurse: living support, psychological support and identifying client's needs. For the Genetic Nurse they selected: provision and exchange of appropriate genetic information among health care providers, reference and collaboration with other institutions and helping clients to understand their conditions and characteristics. As a result, competency of the Basic Level and the Advanced Level were clarified in Genetic Nursing. The results help provide directions for the furtherance of genetic education in nursing in Japan.

The Bioethics Study Group was formed in the year of 2000 by a group of graduate students of this college, T. Nishimura the instructor of the bioethics course being an advisor. The Group met once a month and each participant took his/her turn presenting a case study relating one's own field of nursing. This report, after a brief sketch of the work up to 2003, summarizes the discussions on Search for Spiritual Care the Group devoted in 2003. In the beginning is described a view on "Spirituality" and "Spiritual Care" in general and follow the discussions based on the cases six members introduced of each one's special area. The following are the topics of the discussions ; (1) advanced medicine and spiritual care in the area related to assisted reproduction, (2) in child health nursing area, about spiritual care for a mother and her child based on literature reviews related to the growth of children, (3) in adult nursing area, about spiritual care in finding the meaning of life through diet, (4) in psychiatric nursing area, about spiritual care for a patient to regain eagerness for life, (5) in home nursing area, about a difficulty of spiritual care for a patient who lost his wife, and (6) in palliative care area, about spiritual care of a dying patient. It is the wish of the Group to continue the study and inquiry keeping clinical developments in mind and reaffirming the importance of spiritual care to uphold the "Dignity of life".

The purpose of this study was to describe characteristics of masters' theses and doctoral dissertations during the first twenty years of the Graduate School to make that a basis from which to discuss the future of the Graduate School. A total of 278 masters' theses and 24 doctoral dissertations were studied. Major analysis items included: student specialty, year of submission, source of data, study design, method of data gathering, type of quantitative analysis, type of qualitative analysis and inclusion of a statement describing ethical considerations. Care receivers (child and adult patients) were the source of data for more than half the masters' theses. Of all studies, 70% were descriptive. Interview was the most frequently used data gathering method, followed by questionnaire and observation. Regarding data analysis, 205 used qualitative analysis, 128 used quantitative analysis and 55 used both. 80% of the theses included a statement describing ethical considerations. Care receivers (child and adult patients) were the source of data for 80% of doctoral dissertations. For the preliminary studies, qualitative descriptive design was most frequently used (6 studies). For the design of the major study, correlational designs were used most frequently (8 studies), followed by methodological and qualitative descriptive designs (6 of each), subexperimental designs (2) and experimental designs (1). Combining preliminary and major studies, 21 used qualitative analysis, 42 quantitative analysis and 5 used both. For their dissertations, 7 students adopted qualitative analysis for their preliminary study and quantitative analysis for the major study. All dissertations included a statement describing ethical considerations. On the basis of this analysis and our current understandings, we suggest that studies to measure the direct effects of nursing care be more emphasized. After taking into consideration various points of view, it is recommended that courses of study and support systems be reviewed in light of expectations for graduate student research.

The aim of this research was to identify major sources of stress and coping mechanisms of nurses caring for patients undergoing treatment for infertility. All 963 nurses (registered nurses, public health nurses, and nurse-midwives) working in centers providing treatment for infertility in Japan received questionnaires; 409 of the 963 identified and describes sources of stress; of these, 365 described their coping mechanisms.<BR>Sources of stress were classified into five categories and 19 sub-categories. The most frequently identified stressor was the system for providing infertility treatment. Other frequently mentioned stressors were problems in the immaturity of the field, and ethical problems.<BR>Six coping strategies with 28 sub-categories were identified. Fifty-one nurses said that they simply denied their stress or were unable to cope with it. From the responses it seems that nurses appear to cope best with natural treatments for infertility. Many indicated that their coping was supported by being able to engage in nursing activities and behavior.

Continuous sleep-logs kept by mothers were used to investigate mothers' and infants' sleep patterns from the fifth through twelfth weeks in the postpartum period. Subjects were twenty-two pairs of primiparae and their infants. Data described 1,104 days for the mothers and 1,147 days for their infants. We calculated sleep parameters: total sleep time (TST), wakefulness (AW), sleep efficiency, number of awakenings and total naptime. Means for each week were calculated from daily data. Parameters were analyzed by one-way repeated measures of analysis of variance. After the 5th and 6th weeks, mothers' TST increased from 354.4 minutes in week 5 to 418.1 minutes in week 11 and AW at night decreased from 81.2 minutes in week 5 to 36.8 minutes in week 12. The % AW at night decreased gradually; the increase in TST was seen especially after the 9th week. A gradual transition from interrupted sleep to non-interrupted sleep in the mothers was observed between 9 and 12 weeks postpartum. These results from continuous sleeplogs are similar to our previous findings based on intermittent polysomnographic measurements. There is no significant relationship between %AW at night and total naptime the following day. Mothers showed great variation in their individual napping patterns. For infants there was gradual and significant decrease in %AW at night each week. %AW decreased from about 15% in the 5th and the 6th weeks to under 10% after the 7th week. By the 7th week, sleep time during the day was decreased, and sleep time in the night was increased. A transition from ultradian rhythm dominantly to circadian rhythm dominantly was observed after the 6th week. In summary, during the 5th to 12th weeks postpartum, infants are obtaining their circadian rhythm and mothers' sleep fluctuates between interrupted sleep and noninterrupted sleep.

The purpose of this study was to compare the knowledge and understanding of reproductive health of women and adolescents in Malawi and Japan with the goal of promoting safe motherhood by preventing unwanted pregnancy and induced abortion. Documents and interviews provided data for this study. Significant issues warranting interventions were observed in the two countries. These are lack of adequate reproductive health knowledge, social problems and gender imbalance. We found out that these three are the main contributors to unwanted pregnancy, hence induced abortion. The results suggest that midwives in the two countries have a significant responsibility in promoting reproductive health education with adequate follow-up for the promotion of safe motherhood. High rates of unprotected sex are a threat to the promotion of safe motherhood. During the study we also realized the importance of increasing the awareness of the different cultures and exchanging ideas from the two countries.

To improve the integrated curriculum implemented in April 1995 at St. Luke's College of Nursing, the curriculum evaluation was performed. The present investigation is performed to know how the faculties evaluate the integrated curriculum by the group specifically aimed at "evaluation of the curriculum by faculty members". The survey questionnaire was sent to all the 42 full-time faculties as of the 1999 academic year and 38 of them answered. The survey consisted of the following three sets of questionnaires, (1) evaluation of the subjects (11 items), (2)evaluation of practice of the curriculum (16 items), and (3) evaluation of the satisfaction level (7 items). When making questionnaires, we took into consideration the results of content analysis of group discussion made by faculties in March 1999. The results are summarized as follows. The faculty members highly evaluated the idea of the curriculum. The subjects are properly grouped, sequenced and taught at the right school year and the term of the year. Whether each subject is compulsory or elective is generally proper. On the other hand, the problems are raised concerning the following points: Insufficient coordination subjects, and the lack of them, improper allotment of the time on them, study environments, number of teachers, and proper team work of teachers especially in the team teaching. The faculty's level of satisfaction was varied depend on their professional position. Those involved in the initial phase of establishing the integrated curriculum are more satisfied with it. It seems to be an important matter for more satisfaction to deepen their understanding of the meaning and idea of the integrated curriculum. To continue curriculum evaluation by the faculty, we need to device the questionnaire available to compare answers by faculty and students, and to develop a new evaluation sheet where faculties of each subject can write them problems and tasks.

This report traces international cooperation for a project between Brazil and the Department of Maternal Infant Nursing and Midwifery at St. Luke's College of nursing over the past five years. As one of our goals as members of the WHO Collaborating Center for Nursing Development in Primary Health Care, we are searching for effective strategies to ensure safe motherhood. The Maternal and Child Health Improvement Project in Northeast Brazil was launched in April 1996, and will be carried out though march 2001 in the State of Ceara. This project is collaboration between the Government of the Federal Republic of Brazil and the Government of Japan with the purpose of improving health conditions in Northeast Brazil. One of the objectives of the project is to improve maternal and child health services and to promote health education activities at the community level by including the training of community-based health personnel. The initial survey revealed that cesarean section rates were high and there was much artificial intervention. In addition, laboring women did not receive humanistic care. It was felt that delivery related care, including care at the primary level, would be improved by utilizing the spirit of "Safe Motherhood" and by introducing human caring related to the "midwifery concept". This report describe the delegation of personnel resources, self-directed learning in terms of education and acceptance of counterparts, evaluation, cooperation with the International Conference on the Humanization of Childbirth, the extent of research activities and prospects for the future.

The purpose of this study is to describe the prenatal and postnatal process of women's decision-making regarding their childbirth experiences. The subjects of the study were 419 women outpatients at hospitals, maternity hospitals or birth centers. Questionnaires were used to collect data before and after childbirth.<BR>The factors which influenced the process of decision-making are classified into three groups: medical care which needs professional medical judgement; care which needs professional advice; activities which can be conducted only with appropriate knowledge. Decisions made before and after childbirth are different because of these factors. Variables such as quality of care facilities, interest in childbirth, knowledge, and age are also highly related with the capability of decision-making. Combinations of decisions made before and after childbirth are categorized into four: made a decision by themselves before as well as after childbirth; given chances to make a decision; made a decision with reluctance; no decision by themselves. These combinations were significantly correlated with the three factors of childbirth experiences.

目的:本研究の目的は,患者の意思決定の葛藤をスクリーニングするSURE(Sure of myself;Understand information;Risk-benefit ratio;Encouragement)test日本語版を,言語的妥当性を踏まえ開発することである.方法:SURE test日本語版は,第1段階:2名による順翻訳,第2段階:順翻訳統合,第3段階:2名による逆翻訳,第4段階:研究者協議(暫定版作成),第5段階:一般市民,医療者への調査,第6段階:再検討,を経て開発した.結果:暫定版作成後,第5段階の一般市民と医療者32名の調査において,「わかりやすい」と回答した割合は各項目で47%から78%であった.第6段階にて言語的妥当性を再検討し,日本語版を確定した.結論:一連の過程を経て,言語的妥当性を踏まえたSURE test日本語版を開発した.(著者抄録)

It had not been done validation of midwifery program in Faculty of Health Sciences, Niigata University after established in 2000. The aim of this study is to clarify an employment trend among midwives after graduating and to evaluate the midwifery education program by graduates. The subjects were graduates of our midwifery program (N=207). We conducted two combined surveys: one online survey using a self-administered questionnaire, and the one by interview with voluntary participants (N=4) to clarify the opinion on the program evaluation in detail. This study was approved by the Ethics Committee of the Niigata University School of Medicine in Niigata, Japan (No.2390). The survey results revealed that almost 80% of graduates had begun working as midwives after graduation, majorities of whom were employed in advanced medical centers, approximately half were employed outside the prefecture. Participants reported that they had learned the basic midwifery skills and attitudes necessary to work as professional midwives in clinical practice. Also, learning about lactation and breastfeeding care in midwifery education was an important and expected issue for graduates.

目的:本研究の目的は、People-Centered Care(以下、PCC)の実践として、ダウン症候群児のよりよい成育環境を検討する会(以下、C会)の構成メンバーが、協働し運営や企画に取り組んできたことに対して感じ考えたことや、自身が変化したことや得たものから、おのおのの体験を明らかにすることを目的とした。方法:研究方法は、質的記述デザインとし、研究対象者は、(1)ダウン症候群児の母親、(2)看護学生、(3)講師として成育プログラムを担当した専門職(以下、3者)とした。半構造化面接で、3年目の会の体験の語りの内容をデータとし、KJ法を用いて意味内容のカテゴリーの構造化を行った。結果:(1)児の母親3人、(2)看護学生2人、(3)「音楽」「体操」を担当した専門職2人にインタビューを行った。児の家族の体験は、【協働を通して成果を実感】し、【子どもの将来を見越した能力の獲得を目的としたプログラムの発案】に発展していた。看護学生は、【ダウン症候群児に対する概念が変化】し、【児と主体的にかかわる意欲をもつ契機になった】ととらえ、【児の親と専門職との協働を意義があること】ととらえていた。専門職は、【ダウン症候群児の親子にとって専門職や親同士の情報交換の機会となった】こと、【看護学生の参加が児の積極性を高めたことを評価】し、【ダウン症候群を理解する医療者の育成につながることを期待】していた。また【自己を内省】し、【専門職自身の活動につながる学びを得た】ととらえていた。結論:C会で協働した3者は、相互作用のなかで評価し学び合い、会の活動を内省することにより、地域社会でダウン症候群児がよりよく成育し理解を得るための具体的な方法を考えるという体験を共有していた。ダウン症候群児の家族の体験から、協働の成果は、当事者の階層からC会、地域社会へ視点が拡大することが明らかになった。今後PCCを進めるためには、3者の視点からのさらなる力量形成および仕組拡充が必要であることが示唆された。(著者抄録)

＜文献概要＞本研究は,乳がんに罹患したことを契機に遺伝性乳がん卵巣がん(hereditary breast and ovarian cancer; HBOC)と診断され,リスク低減手術を選択した女性の体験を明らかにすることを目的としている.研究協力者5名に対して半構成的インタビュー調査を行い,そのうち4名の協力者のデータを質的記述的方法に基づき分析を行った.分析の結果,乳がんに罹患したことを契機にHBOCと診断され,リスク低減手術を選択した女性の体験は【がんという病気が当事者とその周囲の人に与える影響を実感する】【いつがんに罹患しても慌てないように心構えをしながら生活する】【自分のことよりもがんや遺伝による周囲への影響を心配する】【遺伝子検査結果に抱いていた不安と期待が落胆に変化する】【リスク低減手術に対する葛藤を経て最終的にがんのリスクを減らすことを優先する】などの10の体験が明らかになった.協力者らの体験から,リスク低減手術を選択した女性は,乳がんに罹患する前から,長期にわたるがんに対する恐怖心との共存があったこと,リスク低減手術の選択には葛藤が伴っていたことが示され,過去のがん体験に配慮した対象の理解を行うことや適切な情報提供を行うことが,看護として重要であることが示唆された.

日本医療機関評価機構の産科医療補償制度加入施設のうち、助産院を除く病院・診療所2884施設の中から独自の基準で選定した全国各地域350施設に対して2013年9月〜11月に質問票を送付し、「無痛分娩実施の有無」「2012年の無痛分娩件数」「無痛分娩の名称」「妊婦に対する無痛分娩の情報提供」などについて調査した。147施設から回答が得られ、無痛分娩を実施している施設の割合は、病院で38.5%、診療所59.6%であった。2012年1年間の無痛分娩件数は、病院・診療所とも20件以下の施設が多かった。無痛分娩の名称は、「無痛分娩」74.5%、「和痛分娩」19.6%、「麻酔分娩」5.9%であった。妊婦への情報提供については、無痛分娩を実施していない施設において無痛分娩の利点や副作用などの情報を提供している施設は皆無であった。無痛分娩を実施している施設での情報提供は、妊娠期保健指導時に行っている施設が65.4%、母親学級時に行っているのが38.0%で、妊娠期に少なくとも1回は行っている施設が82%であった。

目的:筆者らは2008年に年代別(若年編、家族編、前老期編)の骨粗鬆症予防目的のパンフレットを作成した。作成したパンフレットは都内A看護系大学における市民向け健康相談にて活用されている。本研究は市民健康相談の利用者と相談に対応している医療専門職の両者からパンフレットに関する評価を得て、その改善を図る目的で行われた。方法:骨粗鬆症予防のパンフレットを用いて相談を受けた利用者への質問紙調査と、パンフレットを選択し相談に応じた医療専門職へのインタビュー調査の二者を対象に評価した。結果:利用者からの評価については、調査対象者は105人で、女性90.5%、男性9.5%であった。平均年齢は59.5歳で、30〜80歳代に分布していた。配布されたパンフレットの種類は、若年編7人、家族編64人、向老期編36人であった。パンフレットは、利用者の年代よりもスティフネス値と生活背景の情報から選択されていた。パンフレットの評価としては、分りやすく、読みやすく、見やすいと8割が回答し、また全員が満足したと回答した。医療専門職からの評価では、5人が看護職で、1人が栄養士の計6人の女性で、平均年齢60.2歳の対象者であった。専門職によるパンフレットの有用性は、だれでも一定した根拠ある、目で見て分かる。個人の生活に合わせた情報提供ができることが挙げられた。また、パンフレットへの改善点については、カルシウム食品と運動に関する情報の見直しの必要性と、若年編パンフレットの必要性への疑問があった。結論:利用した市民、医療専門職の両者からパンフレットが有用であると評価された。しかし、年代別のパンフレットは、単に年代ではなく、利用者の生活背景と骨のスティフネスに合わせて活用できるパンフレットが必要なことが示唆された。また、カルシウムの含有量の高い食材リストと運動強度が示された運動リストの追加が示唆された。(著者抄録)

本稿では、2011年度改訂カリキュラムの作成過程と概要を報告する。1995年度改訂カリキュラムは総括評価において、いくつかの問題点が明らかとなったため、2007年度からカリキュラム改訂のためのワーキンググループを立ち上げて検討した。2011年度改訂カリキュラムは本学の理念とビジョンに基づくものである。また、4つの概念とその関係性を基盤とした1995年度改訂カリキュラムの枠組みに、本学が文部科学省COEプログラムとして取り組んだPeople-Centered Careの概念を取り入れたことに大きな特徴がある。2011年度改訂カリキュラムは教養科目と基礎科目の充実を図るとともに、専門科目については、臨地実習の充実を図り、保健師の国家試験受験資格にかかわる教育を選択制とした。科目名称については、科目内容のわかるものとし、専門科目は専門性が明確となるものとした。(著者抄録)

2011年4月から、聖路加看護大学大学院(修士課程)看護学専攻に、遺伝看護学を開設した。本稿においては、国内外の遺伝看護師の動向を踏まえて本学が遺伝看護学を開講するに至った経過とそのカリキュラム内容を概説する。専攻分野は、「看護学専攻:基礎系看護学II」とし、修士論文コース、上級実践コースの2コースを設置した。初年度は、1名の学生が遺伝看護学を主専攻として入学した。また、基盤分野に「臨床遺伝学」を新たに開設し、遺伝医学を系統的に、すべての院生が学べるように配置した。初年度は、14名が履修した。さらに、本学学部において、「遺伝看護ゼミナール 遺伝看護学(選択科目)」を履修した学生が将来、大学院に進学し遺伝看護を専攻する動機付けとなるように学部生と院生の交流の機会を意図的に設けた。将来的に、認定遺伝カウンセラーの受験を希望する修了生が、その資格を取得できるように関連機関との調整を図ることが今後の課題である。(著者抄録)

聖路加看護大学看護実践センターでは2011年3月11日の東日本大震災の後、NPO法人日本臨床研究支援ユニットが立ち上げた東北地方災害支援のための「きぼうときずなプロジェクト」に賛同し、福島県災害支援プロジェクトに4月から取り組み始めた。活動地域はいわき市、相馬市、郡山市で、各地域に聖路加看護大学と縁のある看護師や保健師を1〜2名ずつ派遣した。8月31日までの派遣者は363名である。いわき市では市の保健師と連携・調整のもと、避難所における看護、被災住宅の戸別訪問を通して住民ニーズを把握し、支援活動にあたった。相馬市では福島県立医科大学の心のケアチームを支援し、地域の精神医療のシステム化に向けたサポートを行った。郡山市では福島第一原発20km圏内で町ごと避難している富岡町に対して、仮設住宅の戸別訪問やその後のフォローを行った。今回は8月31日までのプロジェクトの活動報告を行うこととする。(著者抄録)

自分の健康に主体的である市民の育成を目指し、その基本となるからだに関する知識を子供に提供するためのプログラムを開発した。それは、聖路加看護大学21世紀COEプログラムのひとつであり、people-centered careであった。その教育プログラムの開発過程を、人々の集まり方の側面についてcoalitionという概念を用いてcommunity-based participatory research(CBPR)という視点で振り返り、研究手法としてのCBPRの有用性を検討した。核となる大学教員のはじめたプロジェクトが、人づてにネットワークとして広がり、そこに定期的会合、議事録作成、連絡方法が確立し、資金支援を受け、教育プログラムの作成が進んだ。また、組織としてのシステムが定まったプロジェクトは、地域などへ対外的に活動をすることができ、内部では役割分担も起こった。コミュニティとの関係は、はじめは、プロジェクト側の力が強いものであったが、コミュニティと関わりながらその教育プログラムを精選し作成していく中で、コミュニティとの間で、エンパワーメントとコンピテンスが移動し、プロジェクト側も、コミュニティ側もともにキャパシティビルディングが起こった。ここにおいてCBPRのスタートとしてのコミュニティとの対等な関係ができた。ここまではらせんモデルに示すcoalitionである。これはCBPRの最初の段階であるコミュニティを見つける段階に含まれる。われわれはこの段階を4期に分けてとらえ、これがコミュニティのニーズを知り、コミュニティと対等な関係で協働していくうえで、とても重要であることを見出した。(著者抄録)

【はじめに】聖路加看護大学21世紀COEプログラム「市民主導型の健康生成をめざす看護形成拠点」におけるPeople-Centered Care(PCC)の5年間の活動成果から、PCCの戦略的実践に有用な具体的方策と示唆を得ることを目的とする。本稿では、PCCのプロセスにおけるパートナーシップに着目し類型化を試みたので報告する。【方法論】研究方法には、質的記述的研究の手法を用いた。15プロジェクトの研究代表者、市民・当事者を対象に行ったグループインタビュー内容をデータとした。研究参加者は、研究代表者15人、活動グループメンバー75人であった。語られたデータからパートナーシップの意味内容のカテゴリーを生成した。分析は(1)パートナーシップ生成における保健医療者の関わりの特徴、(2)市民・当事者(コミュニティ)の活動への参加状況、(3)活動の進展度合いの視点から行った。【結果】市民と保健医療者のパートナーシップは、[共同推進型パートナーシップ][伴走支援型パートナーシップ][掘り起こし開拓型パートナーシップ]の3つの類型に分類された。[共同推進型パートナーシップ]では共通する健康課題に取り組む自助グループと手を組んで、ともに活動を展開し、社会へのメッセージを発信していく活動にまで至っている特徴があった。[伴走支援型パートナーシップ]では、健康問題を抱える個人にケアを提供しながら、ニーズを共有するグループとして組織され、医療や社会に発言できるよう保健医療者が橋渡しをするという活動の特徴があった。[掘り起こし開拓型パートナーシップ]は、保健医療者はそのコミュニティの潜在的なニーズをキャッチしていた。そして市民・当事者にとって必要なケアプログラムを先取りして、提案しながら活動のメンバーや場所、内容などを拡大していくという活動の特徴があった。このような類型化により、あらゆるコミュニティに普遍的に適応可能なパートナーシップに基づく活動としての新たなケア形態の方法論が得られた。(著者抄録)

本研究の目的は、幼稚園・保育園年長児を対象としたプログラム&quot;自分のからだを知ろう&quot;に対する評価指標を検討し、OUTCOMEモデルを提示することである。研究方法は、からだの知識7系統(消化器、呼吸器、泌尿器、循環器、筋骨格、神経、生殖器)で構成されている紙芝居、臓器Tシャツ、絵本を幼稚園・保育園の年長児に行い、保護者にプログラムの感想とからだの知識に関するアンケート調査を行い、その内容をカテゴリー化した。さらにカテゴリーでOUTCOMEモデルを作成し、仮説モデルと比較検討を行った。倫理的配慮は、研究倫理審査委員会で承認を得ると共に、プログラム実施時およびアンケート配布時に研究参加の自由意思、データの守秘等を考慮した。結果、幼稚園・保育園年長児の子ども側とその親側の両側面からカテゴリーが抽出でき、OUTCOMEモデルが作成できた。OUTCOMEモデルは仮説モデルの内容をおおよそ支持できる内容であったが、モデル内の変数間の関係性や、変数に準じた測定道具の選定など課題も残った。今後、追研究でそれらを補い、事象に近づくモデルに精錬していく必要がある。(著者抄録)

The objective of this article is to provide an overview of the St. Luke's College of Nursing 21st Century COE Program 7th International Relay Symposium "Let's Learn about Our Body with Children!" and to consider the components of people-centered care by analyzing and evaluating the process by which the symposium was conducted. The objective of the symposium was to provide methods through which five-year-old children can learn about the human body and to facilitate the meaningful exchange of opinions with the general public. The symposium was targeted for five-year-old children and their parents, as well as various specialists working with five-year-olds, such as childcare professionals, kindergarten teachers, and nurses and school health nurses. The symposium was planned and carried out in cooperation with the general public. Each event on the program was kept to within 20 minutes, and a picture story show and song and dance were included in the program to ensure that children did not lose their interest. Several things were achieved through the process of working cooperatively with the general public in order to conduct the symposium. The elements of people-centered care acquired through past COE activities were confirmed. The staff members experienced the process of "drawing out and exposing (ascertaining them from each other) hidden needs within the community, and shifting activities towards cooperative efforts with specialists and further developing those activities". In addition, a new important element in such activities, "ascertaining things from each other", was uncovered. The symposium also allowed us to shift the focus of cooperation with the general public, something that we had initially been searching for ways of achieving effectively, towards vibrant and energetic cooperation with the community. Moreover, it was confirmed that the process of cooperation with the general public has continued to develop, moving towards symposiums whereby the community and specialistswork together in search of further progress in their activities. Several future issues were re-identified: the status of further cooperation with the community, and concrete methods for evaluating methods by which five-year-olds can learn about the body.

This research is on the assumption that knowledge of the body is necessary for people to take initiative in their own healthcare. Prior research indicates that age 5 is the best age to start systematic education about the body. We have developed an educational program about the digestive system. This program was presented to 69 5-year-olds in two nursery schools and two classes of a kindergarten. Teaching aids include a panel theater ('kamishibai'), picture book and a T-shirt depicting the digestive system. Nurse teachers presented the story "How an apple becomes a BM" using the panel theater. Then, the nurse teachers reviewed what they had presented, while manipulating the special T-shirt. From the T-shirt teachers could remove the parts, open the organs and show the stuffed fabric representing apples at various phases in their digestion. Lastly, each preschooler was given a picture book to take home. Forty-five mothers and five nurse teachers responded to an evaluation questionnaire and indicated that this program was an incentive to learn about the body; that they expected a series of the programs; and that after the preschoolers returned home, they talked with their mothers and other family members about the digestive system using words they learnt. From our work we recognize that partnership with parents and nurse teachers is critical for continued program development. A cohort study is needed in order to evaluate the contribution of our program to understanding of the body.

This research aims to present important contents of People-centered care integrated through a series of symposia "COE International Relay Symposia" we held, and to analyze issues to be addressed to achieving People centered care based on partnership with people and that is accepted in society. Records of meetings, lectures, discussions, and questionnaires were kept throughout every step of planning, organizing and assessing the series consisting of three symposia. Content analyses were conducted on qualitative data while quantitative data were analyzed by descriptive statistics. The theme of each symposium was "How do you want to spend the end of life?", "Let's talk about health care! Patients are part of the team", and "Living the life of your choice", respectively. Examining the data from the entire process of planning, organizing, and assessing the symposia, important essence of People-centered care were found as follows : compassion, experiential knowledges from living, effective communication, creation of useful health information, understanding of different perspectives, and decision making. In the near future, the discovered essence will further be crystallized into obvious and useful concept, system and nursing service as outcomes of the research and the practice development through our endeavor of conducting each project of St. Luke's College of Nursing Center of Excellence Program.

This study attempts to document collaboration between healthcare practitioners and community members in the process of organizing a series of symposia as a means of realizing "People-Centered Care". Using the records of the organizing committee and materials distributed at the symposia as data, a content analysis was conducted on the process of planning and organizing the symposia including the collaboration between the practitioners and community members. The committee met 15 times (total of 28 hours) for a series of three symposia. The number of people who attended each symposium was 320, 160, and 200, respectively. Each symposium was evaluated by the participants as well as designated external evaluators based on : desirability of the theme, relevance of the outcome to policy making, and its utility to the wider international community members. In the early phase of this process of finding ways of collaboration between the healthcare practitioners and community members, the primary focus was on promotion of the symposia. A Logo and a catch phrase were made for the promotion. Similarly, a symbol-quilt for the symposia was crafted involving the participants and a number of volunteers throughout the series. The program of the series was refined, symposium by symposium, based on feedbacks and evaluations obtained from the prior symposium. Throughout the process of collaboration with community members, the healthcare practitioners, as the symposia organizers, consistently encountered the following issues : (1) ecessity of confirming the basic theme of the series, (2) continual search for strategies to ensure community participation, (3) clarification and simplification of the messages and concepts, (4) repeated examination of the goals of the series as well as "People-Centered Care", (5) dissemination of the outcome, and (6) role reversal of the healthcare practitioners to learn from community members.

St. Luke's College of Nursing Graduate School has celebrated its 20th anniversary. The objective of this study was describe characteristics of the graduates of the first 20 years and learn their ideas about graduate education in order to take them into consideration in our discussions of the future orientation and policies of the Graduate School. In July 2001 we mailed questionnaires to the 299 graduates (278 master's graduates and 21 doctoral graduates). Slightly more than one-third provided data for our study. The findings refer to both groups, with some detail presented for master and doctoral graduates separately. ・Of the graduates, 80% entered graduate school as private persons and 20% were taking official leave from their employers or receiving other support from them. ・Motivations for entering graduate school included: improvement of research ability, study in a field or theme of particular interest, improvement of ability in a special nursing field and motivation to learn more. More than 90% reported that these objectives were achieved. ・Their present occupations are: 70% in education-related positions; 20% in clinical settings. They wish to continue in their fields. ・Numbers of academic societies they were: master's, 4.9±2.3; doctoral, 6.9±3.9. All had written at least one research paper as first author in the last five years (master's, 3.7±3.1 (1〜15); doctoral, 4.5±3.3 (1〜12)). ・ Graduates indicated their preferences for the future direction of this college in response to a set of questions that listed system, program and policy options for graduate education. 90% wanted more lectures and courses from nurses in clinical practice. Among many other suggestions, in decreasing order of frequency, were: admission for working people, day and evening courses, programs of study organized on a course basis. ・Many patterns were seen as pathways from when they completed their basic nursing education to the future. Our conclusions: The graduates have high expectations for the Graduate School. They, themselves, wish to actively develop in their careers throughout their lives. They want the Graduate School to take into account the diversity in ways of life of professional nurses and allow students flexibility in their courses of study. With respect to graduate education in nursing, the committee sees that in future the Graduate School should focus more on the cyclic relationship among nursing research, education and practice. The Graduate School can become a focal point for maintaining and improving each and the linkage among them, there-by promoting its social and professional relevance.

The Second International Conference of the Global Network of WHO Collaborating Centres for Nursing and Midwifery was held in Korea this past year.St.Luke&#039;s College of Nursing joined in the poster session in the “Expo Nursing Edu“which focused on nursing education， specifically education for nursing specialists.The midwifery curriculum and master and doctoral courses were introduced in the poster session.Information exchange and discussion at this session gave us an opportunity to learn more about the current status of education in universities in various countries and territories.Throughout the conference， we found opportunities for collaboration within the global network， regional offices， countries and centers.We also gained a greater understanding of global and regional viewpoints， and we obtained information and recommendations as to our role as a member of the WHO Collaborating Centres.
