    Background/Objectives: Chronic craniofacial inflammation is recognized as a factor in anxiety-like behaviors, yet effective therapeutic options remain limited. Agmatine, a dietary bioactive compound found in fermented foods such as sake lees, exhibits modulatory effects on neural functions, alleviating psychological distress like anxiety associated with local inflammation. Methods: We investigated both the therapeutic and preventive effects of agmatine on anxiety-like behaviors and the related neural basis in a mouse model of persistent craniofacial inflammation induced by complete Freund’s adjuvant (CFA). Results: Comprehensive behavioral assessments, including the elevated plus maze, open field, dark–light box, social interaction, and novel object recognition tests, revealed that therapeutic agmatine administration (1.0 and 30 mg/kg) significantly reduced CFA-induced anxiety-like behaviors, with the higher dose showing more robust and sustained effects across multiple time points. These behavioral improvements were paralleled by reductions in acetylated histone H3, FosB, and c-Fos expression in key anxiety-related brain regions, suggesting a reversal of craniofacial inflammation-associated neural changes. In contrast, preventive agmatine treatment exerted modest and time-dependent behavioral benefits with minimal molecular normalization. Notably, preventive agmatine did not affect general locomotor activity (indicated by total movement distance), indicating that its anxiolytic effects were not confounded by altered locomotor activity. Metabolomic analysis confirmed the presence of agmatine in sake lees (~0.37 mM), supporting the hypothesis that fermented food products might offer dietary routes to emotional resilience. Conclusions: These findings underscore agmatine’s promise as a context-specific epigenetic modulator capable of mitigating anxiety-like behaviors by normalizing inflammation-driven molecular dysregulation in the brain.

    The swallowing reflex can be induced by peripheral stimulation of the larynx. Although previous studies have suggested that potassium ions exert facilitatory effects on the initiation of swallowing, little information is available on the mechanism underlying the potassium ion-evoked swallowing reflex. In this study, we evaluated the effects of potassium ions on peripheral afferent responses and the initiation of swallowing in conscious and anesthetized rats. Furthermore, the possible receptors involved were explored. The topical application of potassium chloride (KCl) significantly facilitated the swallowing reflex; these facilitatory effects were more prominent than those of distilled water (DW) or sodium chloride (NaCl). This phenomenon depended not on the concentrations of anions but on those of potassium ions. The potassium ion-induced response in the superior laryngeal nerve was most prominent after treatment with KCl, especially at the early stage. In chronic rats, without differences in licking behavior between DW, NaCl, and KCl, the intervals between swallows were the smallest during KCl-associated licking. Inward rectifier potassium channel (Kir)3.1- and Kir6.2-positive cells were detected in the nodose ganglion and vocal folds. The rate of expression of these molecules in immunoreactive cells was relatively high at 74.1% for Kir3.1 and 75.3% for Kir6.2. Kir3.1- and Kir6.2- blockers significantly decreased the number of KCl-induced swallows. Possible mechanisms underlying potassium ion-induced swallowing are discussed. Our findings suggest that Kir3.1 and Kir6.2 are involved in K ion-induced swallowing in rats.

    The aim of this study was to clarify whether the posterior belly of the digastric (post-Dig) muscle is activated during the swallowing reflex and whether the post-Dig muscle is directly controlled by the swallowing central pattern generator (CPG) in anesthetized rats, using physiological and immunohistochemical approaches. In physiological study, electromyograms (EMGs) of the post-Dig, sternohyoid and thyrohyoid muscles, and the diaphragm were recorded during respiration and swallowing with and without airway stenosis. In the immunohistochemical study, c-Fos immunoreactivity for expression of cells during swallowing was analyzed. Motoneurons were identified using immunohistochemistry with Fluoro-gold (FG). EMG bursts were observed in the hyoid muscles during the inspiratory phase and swallowing. With airway stenosis, the swallowing EMG activity was facilitated in terms of duration and area only in the post-Dig muscle. The coordination of these EMG activities during swallowing was maintained with airway stenosis. In contrast, the offset of the post-Dig EMG burst was delayed with airway stenosis. c-Fos-positive cells were observed in the accessory facial nucleus (Acs7), but only in the rostral portion. FG-labeled cells were observed in Acs7. Several c-Fos/FG double-labeled cells were observed only in the rostral Acs7. These results suggested that the post-Dig muscle is activated during swallowing, the activation of which is controlled by the swallowing CPG, and that the distribution of Acs7 neurons, which innervate the post-Dig muscle, was uneven in the nucleus. In addition, the modulation of post-Dig muscle activity during inspiration might be due to changes in peripheral conditions via respiratory CPG.

    This study examined the effects of treadmill running (TR) regimens on craniofacial pain- and anxiety-like behaviors, as well as their effects on neural changes in specific brain regions of male mice subjected to repeated social defeat stress (SDS) for 10 days. Behavioral and immunohistochemical experiments were conducted to evaluate the impact of TR regimens on SDS-related those behaviors, as well as epigenetic and neural activity markers in the anterior cingulate cortex (ACC), insular cortex (IC), rostral ventromedial medulla (RVM), and cervical spinal dorsal horn (C2). Behavioral responses were quantified using multiple tests, while immunohistochemistry measured histone H3 acetylation, histone deacetylases (HDAC1, HDAC2), and neural activity markers (FosB and phosphorylated cAMP response element-binding protein (pCREB). The effects of both short-term TR (2 days, TR2) and long-term TR (10 days, TR10) regimens were conducted. TR10 significantly reduced anxiety- and formalin-evoked craniofacial pain-like behaviors in SDS mice. It normalized SDS-induced increases in histone H3 acetylation in both the anterior and posterior portions of the ACC, as well as the anterior portion of the IC. These inhibitory effects were also observed in SDS-related increases in HDAC1, FosB, and pCREB expression. Additionally, TR10 normalized increased histone H3 acetylation in the RVM and C2 regions, with specific effects on FosB and pCREB expression observed in the C2 region. In contrast, TR2 showed limited effects on craniofacial pain-like behaviors but reduced anxiety-like behaviors in SDS mice. Under sham conditions, TR2 had minimal impact on histone H3 acetylation. Paradoxically, TR2 increased formalin-evoked craniofacial pain-like behaviors during the early phase despite not altering acetylated histone H3 expression. In conclusion, the TR10 regimen is effective in attenuating SDS-induced craniofacial pain- and anxiety-like behaviors, likely by normalizing epigenetic modifications and neural activity in key brain regions.

  • Nutritional Strategies for Chronic Craniofacial Pain and Temporomandibular Disorders: Current Clinical and Preclinical Insights Reviewed

    This narrative review provides an overview of current knowledge on the impact of nutritional strategies on chronic craniofacial pain associated with temporomandibular disorders (TMDs). Individuals experiencing painful TMDs alter their dietary habits, avoiding certain foods, possibly due to chewing difficulties, which might lead to nutrient deficiencies. Our literature investigation revealed that the causal links between nutritional changes and craniofacial pain remain unclear. However, clinical and preclinical studies suggest that nutraceuticals, including vitamins, minerals, polyphenols, omega-3 fatty acids, isoprenoids, carotenoids, lectins, polysaccharides, glucosamines, and palmitoylethanolamides, could have beneficial effects on managing TMDs. This is described in 12 clinical and 38 preclinical articles since 2000. Clinical articles discussed the roles of vitamins, minerals, glucosamine, and palmitoylethanolamides. The other nutraceuticals were assessed solely in preclinical studies, using TMD models, mostly craniofacial inflammatory rodents, with 36 of the 38 articles published since 2013. Our investigation indicates that current evidence is insufficient to assess the efficacy of these nutraceuticals. However, the existing data suggest potential for therapeutic intervention in TMDs. Further support from longitudinal and randomized controlled studies and well-designed preclinical investigations is necessary to evaluate the efficacy of each nutraceutical intervention and understand their underlying mechanisms in TMDs.

    The conditions of stress contagion are induced in bystanders without direct experiences of stressful events. This study determined the effects of stress contagion on masseter muscle nociception in mice. Stress contagion was developed in the bystanders after cohabitating with a conspecific mouse subjected to social defeat stress for 10 days. On Day 11, stress contagion increased anxiety- and orofacial inflammatory pain-like behaviors. The c-Fos and FosB immunoreactivities evoked by masseter muscle stimulation were increased in the upper cervical spinal cord, while c-Fos expressions were increased in the rostral ventromedial medulla, including the lateral paragigantocellular reticular nucleus and nucleus raphe magnus in stress contagion mice. The level of serotonin in the rostral ventromedial medulla was increased under stress contagion, while the number of serotonin positive cells was increased in the lateral paragigantocellular reticular nucleus. Stress contagion increased c-Fos and FosB expressions in the anterior cingulate cortex and insular cortex, both of which were positively correlated with orofacial inflammatory pain-like behaviors. The level of brain-derived neurotrophic factor was increased in the insular cortex under stress contagion. These results indicate that stress contagion can cause neural changes in the brain, resulting in increased masseter muscle nociception, as seen in social defeat stress mice.

    We investigated the effects of social defeat stress (SDS) and treadmill running on masseter muscle nociception, which was quantified by the orofacial formalin test and c-Fos and FosB immunoreactivities in the upper cervical spinal cord (C1/C2) region in male mice. After daily SDS or non-SDS conditioning for 10 days, SDS-conditioned mice were categorized into SDS-susceptible versus resilient mice. Several mice, including non-SDS-conditioned, SDS-susceptible, and resilient mice, were selected to assess masseter muscle nociception on Day 11. SDS conditioning for 10 days increased masseter muscle-evoked nocifensive behaviors and c-Fos and FosB expression in SDS-susceptible compared to non-SDS and resilient mice. The remaining SDS-susceptible and non-SDS mice were subjected to an additional 10 days of SDS plus treadmill running or sedentary sessions before assessing masseter muscle nociception on Day 21. Daily treadmill running sessions reduced enhanced masseter muscle nociception in SDS-susceptible mice but not in non-SDS mice. The preventive effects of daily treadmill running immediately after each SDS conditioning for 10 days on orofacial nocifensive behaviors were assessed on Day 11. Treadmill running conducted immediately after daily SDS inhibited enhanced orofacial nocifensive behaviors. These findings indicate that repeated SDS increases masseter muscle nociception, which could be prevented by daily treadmill running exercise.

    Chronic pain in temporomandibular disorder (TMD) is a common health problem. Cumulating evidence indicates that the etiology of TMD pain is complex with multifactorial experience that could hamper the developments of treatments. Preclinical research is a resource to understand the mechanism for TMD pain, whereas limitations are present as a disease-specific model. It is difficult to incorporate multiple risk factors associated with the etiology that could increase pain responses into a single animal. This article introduces several rodent models which are often employed in the preclinical studies and discusses their validities for TMD pain after the elucidations of the neural mechanisms based on the clinical reports. First, rodent models were classified into two groups with or without inflammation in the deep craniofacial tissues. Next, the characteristics of each model and the procedures to identify deep craniofacial pain were discussed. Emphasis was directed on the findings of the effects of chronic psychological stress, a major risk factor for chronic pain, on the deep craniofacial nociception. Preclinical models have provided clinically relevant information, which could contribute to better understand the basis for TMD pain, while efforts are still required to bridge the gap between animal and human studies.

    This study was designed to investigate the effects of Sake Lees extracts (SLE, Sake Kasu) on the functional activity of odontoblastic cells and tooth pulp of the rats. For in vitro studies, a rat clonal odontoblast-like cell line, KN-3 cells were cultured. SLE significantly decreased KN-3 cell proliferation, but showed no significant cytotoxicity. SLE effects on several protein productions of KN-3 cells were compared with PBS. SLE and PBS increased alkaline phosphatase (ALP), dentin sialoprotein (DSP), and osterix in a day-course dependent manner, while SLE increased the induction of ALP on day 9-21 and DSP on day 15-21. SLE also increased Runx2 expression on day 3 and 9 compared to PBS. Alizarin Red stainings revealed that SLE showed a subtle increase in mineralization of KN-3 cells on day 15 and 21. A histological investigation was conducted to assess if SLE induced reparative dentin formation after direct capping at the exposed tooth pulp in rats, suggesting that SLE could increase the reparative dentin formation more than PBS. These findings suggest that Sake Lees could have functional roles in the alterations of odontoblastic activity, which might influence the physiology of the tooth pulp.

    Psychophysical stress can cause neural changes that increase nociception in the orofacial region, particularly the masseter muscle (MM). The nucleus raphe magnus (NRM), which is located in the brain stem, serves the crucial role of regulating nociception through descending modulatory pain control. However, it remains unclear if neural activities in the NRM are affected under psychophysical stress conditions. This study conducted experiments to assess (1) whether neural activity, indicated by Fos expression in an NRM that has experienced MM injury, is affected by the stress of repeated forced swim tests (FST); and (2) whether the selective serotonin reuptake inhibitor fluoxetine administered daily after an FST could affect the number of Fos-positive neurons in the NRM. Results revealed that the stress from repeated FSTs significantly increased the number of Fos-positive neurons in an NRM that had been affected by MM injury. Fluoxetine inhibited increases in the number of Fos-positive neurons in the NRM that occurred as a result of FSTs, but this was not observed in sham rats. These findings indicate that the stress from FSTs could increase nociceptive neural activity in an NRM that has experienced MM injury. This could be due, in part, to changes in serotonergic mechanisms.

    We tested whether Sake Lees (SL) had inhibitory effects on hyperalgesia in the hindpaw under psychophysical stress conditions. Male rats were subjected to repeated forced swim stress treatments (FST) from Day -3 to Day -1. Intraperiotoneal administration of SL which contained low concentration of ethanol (SLX) was conducted after each FST. On Day 0, formalin-evoked licking behaviors and Fos responses in the lumbar spinal cord (DH) and several areas within the rostral ventromedial medulla (RVM) were quantified as nociceptive responses. FST-induced hyperalgesia in the hindpaw was prevented by repeated SL and SLX treatments. Fos expression was significantly increased in DH and some areas within the RVM under FST, which was prevented by repeated SL or SLX. These findings indicated that daily administration of SL had the potential to alleviate stress-induced hyperalgesia.

    Three patients with diffuse sclerosing osteomyelitis of the mandible who had been treated by intra-arterial infusion of urokinase and antibacterial agents including fosfomycin were evaluated. All patients underwent decortication at the same time. They were all free from symptoms an average of 3 years 7 months after treatment.<BR>Urokinase and fosfomycin may cause the dissolution of biofilm, which prevents contact between antibacterial agents and causative organism. Thus, the intra-arterial infusion of urokinase, fosfomycin, and other antibacterial agents can efficiently eradicate bacteria in sclerotic bone in osteomyelitis.

    The therapeutic effects of neo-adjuvant chemotherapy administered by two route infusion of cisplatin and its antidote, sodium thiosulfate, and intravenous infusion of vinblastine and peplomycin were studied on 35 patients with previously untreated squamous cell carcinoma of the oral cavity, maxillary sinus, or oropharynx. The overall response rate to chemotherapy was 91.4%, and complete response rate was 57.1%. When the patients who died of a disease other than tumor were excluded, the cumulative 5-year survival rate was 100% in CR and 87.5% in PR plus NC. This chemotherapy could be carried out in a shorter time period because of the appropriate supporting therapy. Twelve out of 20 CR patients underwent conservative surgery, and they are all free from tumor and have good oral function. These data showed that this chemotherapy may be more effective for treatment of head and neck cancer. The results are preliminary, and additional studies using more patients are needed.

    不随意な筋の協調障害により運動動作が障害されるジストニアの一症状である口顎部(オーラル)ジストニアは、詳細な病態が明らかになっておらず疾患に対する包括的な理解が進んでいない。本研究計画では、全身性ジストニア様の運動障害を呈するDst 遺伝子 (細胞骨格リンカータンパク質をコードし、変異によりニューロパチーを引き起こす) 変異マウスを用いて、①病態モデル確立のための表現型解析 ②顎口腔系で発症する不随意運動の原因部位の探索について研究を行うことを計画している。具体的には、先行研究で原因部位の候補とした脳幹・感覚神経系それぞれに部位選択的にDst発現を抑制したcKOマウス、部位選択的にDst発現を回復したcRescueマウスを作成し、電気生理学・組織学実験による詳細な表現系解析を行い、発症する症状の比較により原因部位を明らかとする。研究初年度に当たる本年度は、上述した遺伝子組み換え動物の開発元である新潟大学医学部からの凍結胚の譲渡並びに繁殖を実施するための法的手続きや実験・飼育環境の整備を中心に行った。さらに、電気生理学実験を実施するに当たり、慢性実験系を計画していることから、慢性実験記録系を確立するため、C57B6/Jのマウスを用いて記録筋電図電極や農繁電極の装着方法を探索し、個体で安定した記録環境の構築を行ってきた。実験個体に対しては、本研究の目的である自由行動下での咀嚼運動を行わさせ、物性の異なる食品咀嚼時の咀嚼運動を記録し解析を行った。

    申請者は電気生理学的、行動学的、免疫組織化学的手法を用い、中枢および末梢神経系における5HT2A,5HT3受容体の顎顔面部侵害受容における役割について検討した。
    1)顎関節炎症モデルにおいて咬筋の5HT2A,5HT3受容体をそれぞれ拮抗薬でブロックしておく。続いて同部へのホルマリン刺激を行うと三叉神経脊髄路核尾側亜核に発現するFos陽性細胞数は有意に減少した。つまりこれら末梢における受容体は顎顔面部侵害受容反応に深く関係し、痙痛/痛覚過敏を促進させる役割のあることを示唆するものである。(Okamoto et al. Neuroscience 143.597-606.2006)。また顎関節の持続性炎症モデルラットでは後肢においても痛覚過敏を起こす事を確認した(Okamoto et al. Experimental Brain Research 170.358-367.2006)。
    2)さらに顎関節非炎症モデルラットを用い、三叉神経脊髄路核尾側亜核の5HT2A受容体を活性化させておく。そして同部の侵害受容ニューロンの興奮性の変化を細胞外記録、およびFos免疫組織化学的手法を用い検索した。結果、咬筋へのホルマリン刺激、顔面皮膚への侵害機械刺激によって引き起こされる神経興奮はvehicle群と比較して有意に低下した。さらに大槽内への5HT2A受容体作動薬の投与しておくと、ホルマリンの咬筋刺激によって引き起こされる顎顔面部疼痛関連行動は有意に短縮した。つまり中枢神経系における5HT2A受容体活性は顎顔面部侵害受容反応を抑制する可能性を示唆するものである。

    以上の実験はmaleラットで行ったが、今後はfemaleラットを用い三叉神経脊髄路核尾側亜核の5HT2A受容体の役割がMaleと異なるか否かについて検索を行う。
