    Abstract

    Salivary alpha‐amylase (sAA) is an enzyme found in saliva and is considered a noninvasive biomarker for sympathetic nervous system activity. While a wide range of sAA activity in response to stress has been reported in nonhuman primates, the effects of stress on sAA activity in common marmosets are still unknown. We tested the hypothesis that advanced age and cognitive function may have an impact on stress‐related sAA reactivity in marmosets. Thirteen marmosets (nine males and five females) had saliva samples collected during a stressful condition (manual restraint stress) at two different time points, with a 60‐min interval. On the next day, the animals underwent the object recognition test (ORT, a type of cognitive test), and then oral examinations. The animals were categorized into two age groups: old (10–13 years), and very old (15–22 years). Irrespective of age, sAA levels showed a significant difference between T1 (mean 2.07 ± 0.86 U/mL) and T2 samples (mean 1.03 ± 0.67 U/mL), with higher values observed at T1 (p < 0.001). The intra‐assay coefficients of variation (CV) for low and high sAA concentrations were 10.79% and 8.17%, respectively, while the interassay CVs for low and high sAA concentrations were 6.39% and 4.38%, respectively. Oral health issues were common but did not significantly impact sAA levels. The ORT indicated that the animals could recognize an object placed in the cage 6 h after familiarization. In conclusion, all marmosets showed a higher sAA concentration in the first saliva sample as compared to the second saliva sample collected 1 h later, indicating adaptation to stress. No significant differences in sAA levels were observed between sexes, ORT performance, or oral health. Our results indicate that autonomic responsivity and cognitive (memory) functions were preserved even in very old marmosets.

    Abstract

    Objectives

    This study aims to evaluate the effects of socioeconomic, behavioral, and psychological factors on oral health status and oral health‐related quality of life in Myanmar adults.

    Methods

    Data were from a convenience sample of Myanmar adults who are from a township health center in Yangon city. Face‐to‐face interviews using a paper‐based questionnaire in the Myanmar language and clinical oral examinations were performed at the health center. Mann–Whitney U test and linear regressions were used to assess the association of socio‐demographic, behavioral, and psychological variables with oral health outcomes.

    Results

    In socio‐demographic variables, significant associations were observed in individual income with periodontal pocket and OHIP‐14, and history of COVID‐19 infection with OHIP‐14. However, there were no associations between behavioral factors and oral health outcomes. After adjustment for sex, age, educational level, and individual income, the depression subscale was statistically significant with the number of present teeth, decayed teeth, decayed, missing, and filled teeth, and OHIP‐14. The anxiety subscale remained significant with periodontal pocket, but no stress subscale was associated with oral health outcomes.

    Conclusion

    The experiences of oral health status were high, and the oral health‐related quality of life was unfavorable in Myanmar adults. A large proportion of them suffers from psychological distress. Additionally, individuals with lower income and who experienced a history of COVID‐19 infection were susceptible to poor oral health‐related quality of life. Psychological distress indicates a higher risk for oral health problems in Myanmar adults.

    A cross-sectional study was conducted among 3- to 5-year-old preschool children in Yangon City in 2016-2017 to investigate the association between their caries experience and maternal factors. Children’s oral examination and questionnaire surveys to their mothers were conducted. Children’s caries prevalence was 82.9% with mean dmft of 6.4 ± 4.9. Mean oral health knowledge score of their mothers was 8.5 ± 2.8. Mother’s educational level and oral health knowledge were significant predictors of childhood caries. Third- or later-born children were twice more likely to have dental caries than first-born children ( P = .022). Mother’s regular dental check-up resulted in significantly lower odds ratios for caries development in her child. Children’s caries experience was high and mother’s oral health knowledge regarding the importance of primary teeth was unsatisfactory. It is, therefore, necessary to improve awareness and attentiveness for prevention of childhood caries among parents and guardians in Myanmar.

  • Oral Health Promotion under the 8020 Campaign in Japan—A Systematic Review Reviewed

    (1) Background: The aim of this study is to review the benefits of the 8020 Campaign since its inception. (2) Methods: We followed the PRISMA guideline and collected information regarding the 8020 Campaign through online database searches. (3) Results: Twenty-five studies met the inclusion criteria and were eligible for analysis. The main outcomes of the 25 included studies were reviewed. The quality evaluation demonstrated a range of studies showing a credible relationship between masticatory function, number of teeth, salivary secretion, frequent dental check-ups, and general health concerns. Due to the risk of bias, publication bias, and indirectness, 22 studies were considered that only had “fair” quality. (4) Conclusions: The 8020 Foundation funded several of the studies, and other research papers noted the 8020 Campaign in their papers, however there were no clear explanations for any direct relationship between their findings and the 8020 Campaign. As a result, evidence for the direct effectiveness and benefits assessment of the 8020 Campaign positive outcomes were compromised by confounding social and economic variables over the 30-year period. To fully elucidate how improvement in Japan’s oral health was directly related to the 8020 Campaign, a more informed and systematic explanation of the campaign’s structure and activities is required.

    <title>Abstract</title><sec>
    <title>Background</title>
    Unintended weight loss and the reduction in appetite are common phenomenon among older people. Reduced appetite has been linked to medication related reductions in saliva production, reduced taste ability and poor oral health. Poor appetite can result in reduced nutrient intake ensuing weight loss. It is possible that poor appetite is a mediating step on the causal pathway between oral health and weight loss. This study investigates whether poor oral health and loss of appetite are related to weight loss.


    </sec><sec>
    <title>Methods</title>
    This is an observational study where data were obtained from the Concord Health and Ageing in Men Project (CHAMP). Information on socio-demographics, appetite and health related behavior was collected by self-completed questionnaire. Intraoral assessment was conducted by calibrated oral health therapists. Height and weight were measured by trained staff. Regression analysis investigated associations between oral health and appetite as risk factors for weight loss.


    </sec><sec>
    <title>Results</title>
    Participants included 542 community dwelling older males. 99 older men (18.3%) experienced 5% or more weight loss over 3 years. Men who lost weight from baseline had lower BMI and lower body weight, had higher prevalence of frailty and depression, reported poorer appetite, and had fewer teeth (13.8 ± 9.5) than those who did not lose weight (16.3 ± 9.3). Before adjustment, the prevalence ratio (PR) for weight loss was 1.76 (95% Confidence Interval (CI), 1.19–2.59) for participants with 0–19 natural teeth present compared to those with 20 or more teeth. When adding appetite and other variables to the model, the PR for number of teeth and weight loss was unchanged: 1.78 (95% CI, 1.06–3.00). The mediation analysis showed that the indirect effect of appetite on the association between number of natural teeth on weight loss was not found to be significant.


    </sec><sec>
    <title>Conclusion</title>
    This study found that number of natural teeth present and appetite are independently related to weight change among elderly men in Australia. Tooth loss can increase the risk of swallowing difficulty leading to change in food preference, avoidance of foods and a decrease in energy intake. Our study showed the importance of oral health interventions to encourage maintenance of 20 or more natural teeth in older people.

    In recent years, equipment that generates intermediate-frequency electromagnetic fields (IF-EMFs) has become increasingly prevalent, and the influence of IF-EMFs on human health is thus attracting increasing attention. The present study was conducted with the aim of analyzing whether there is a relationship between the penetration of induction heating cookers and birth outcomes using an ecological study design at the prefectural level. We created data sets for all 47 prefectures in Japan using previously published statistics. Spontaneous fetal death rate, fetal death rate after 22 weeks of pregnancy, perinatal mortality rate, and proportion of newborns weighing less than 2500 g were used as birth outcomes in correlation analysis. A weak positive association was observed between the penetration of induction heating cookers and the fetal death rate after the 22nd week of pregnancy (r = 0.27, p = 0.07), but it was not statistically significant. In addition, a weak negative association was observed between the penetration of induction heating cookers and the spontaneous fetal death rate (r = -0.27, p = 0.07), but it was not statistically significant. In the present ecological study, no statistically significant association were shown between the penetration of induction heating cookers and birth outcomes. To demonstrate further the safety of induction heating cooker use, observations in epidemiological studies of other designs should be considered.

    Background<br />
    The Concord Health and Ageing in Men Project (CHAMP) is a cohort study of the health of a representative sample of older Australian men. The aim of this paper is to describe the oral health behaviours and dental service use of CHAMP participants and explore associations between oral health behaviours with and general health status.<br />
    Method<br />
    Information collected related to socio‐demographics, general health, oral health service‐use and oral health behaviours. Key general health conditions were ascertained from the health questionnaire and included physical capacity and cognitive status.<br />
    Results<br />
    Fifty‐seven percent of the men reported visiting a dental provider at least once or more a year and 56.7% did so for a “dental check‐up”. Of those with some natural teeth, 59.3% claimed to brush their teeth at least twice or more a day. Most men (96%) used a standard fluoride toothpaste. Few participants used dental floss, tooth picks or mouth‐rinses to supplement oral hygiene. Cognitive status and self‐rated general health were associated with dental visiting patterns and toothbrushing behaviour.<br />
    Conclusions<br />
    Most older men in CHAMP perform favourable oral health behaviours. Smoking behaviour is associated with less favourable dental visiting patterns, and cognitive status with toothbrushing behaviour.

    With rapid aging of the world’s population, the demand for research, for a better understanding of aging and aging-related disorders, is increasing. Ideally, such research should be conducted on human subjects. However, due to ethical considerations, animals such as rodents and monkeys are used as alternatives. Among these alternative models, non-human primates are preferred because of their similarities with humans. The small South American common marmoset (Callithrix jacchus) may offer several advantages over other non-human primates in terms of its smaller size, shorter life-span, and dental anatomy identical to humans. The purpose of this study was to determine the viability of using the marmoset as a human oral disease model. We collected saliva samples from eight marmosets and eight human subjects. Prokaryotic DNA was extracted from the saliva samples, and 16S bacterial rRNA gene sequencing was performed on each of the samples. Our results indicated that the types of oral microbiomes detected among human and marmoset samples were nearly indistinguishable. In contrast, the oral microbiomes of our human and marmoset subjects were distinctly different from those reported for rats and dogs, which are currently popular research animals. The oral microbiomes of marmosets showed greater diversity than those of humans. However, the oral microbiota of marmosets exhibited less variation than those of humans, which may be attributed to the fact that the marmoset subjects were kept in a controlled environment with identical lifestyles. The characteristics of its oral microbiota, combined with other technical advantages, suggest that the marmoset may provide the best animal model thus far for the study of oral health. This study characterized the oral microbes of the marmoset, thereby providing information to support future application of the marmoset as a model for age-related oral disease.

    Background: Little data are available regarding halitosis in Japanese children. The aim of the current study was to investigate the prevalence and risk factors associated with halitosis in Japanese elementary and junior high school children. Methods: The subjects consisted of 520 elementary (1st–6th grade: boys, n = 284
    girls, n = 236) and 248 junior high (7th–9th grade: boys, n = 136
    girls, n = 112) school children aged 6–15 years in Saitama Prefecture, Japan. A self-administered questionnaire survey
    halitosis measurement using an organoleptic assessment method
    and clinical oral examination were conducted. Results: Overall, 44.9% of subjects had halitosis. The proportion of boys with halitosis was 43.6% and that of girls was 46.6%. On logistic regression analysis, grade and tongue coating were significant predictors of halitosis. The 7th–9th graders were significantly more likely to have halitosis than 1st–3rd graders (OR, 1.83

    Objective: This survey was conducted to evaluate changes of students' English competency as they progress through the academic years, and disclose the effect of outbound exchange programs (OEP) that students can visit oversea universities as an elective course. Materials and Methods: All undergraduate dental students from 1st to 6th year (n = 332), at School of Dentistry, Tokyo Medical and Dental University were invited to participate in the study, and asked to fill out a self-administered questionnaire. As an objective scale of student's English competency, TOEFL-ITP scores were used. In total, 236 (71.1%) of students participated. Students were classified into two groups by the participation or absence of OEP. Results: Experience from OEP was associated with improved English competency. Students without the participation showed decline in English competency over time. Self-confidence in English communication coincided statistically with the objective measurement of TOEFL-ITP scores. Discussion and Conclusions: Exposure to English communication through experience in organized OEP appears to be important for improving their confidence in English communication. Greater exposure to English language for the dental students and exposure to different culture may help development of international perspectives, motivation of students for further study, and influencing their career choice.

    The aim of the present study was to assess the influence of various workplace parameters and oral health behaviors on tooth decay, periodontal disease, and the number of teeth present in industrial workers. The study participants were 1,078 workers (808 males, 270 females, mean age 42.8 ± 11.4 yr) employed at 11 different workplaces. Oral examinations and a self-administered questionnaire were conducted for participants. A logistic regression analysis was conducted to identify factors related to their oral health status. Factors significantly associated with decayed teeth were smoking (OR=2.02), not having received tooth brushing instruction (OR=1.73), not having annual dental examinations (OR = 1.64) and not brushing before sleeping (OR = 0.55). The factors significantly associated with severe periodontal disease were employment with a company with fewer than 50 employees (OR = 15.56) and not brushing teeth before bedtime (OR = 2.41). The factors significantly associated with having 23 teeth or fewer were subjects in the education and learning support industry compared with manufacturing industry (OR = 5.83) and transport industry (OR = 12.01). The results of the present study showed that various occupational parameters and health behaviors are associated with oral health status including tooth decay, periodontal disease, and tooth loss.

    Objective: This study was conducted to acquire a deeper understanding of Japanese students' motivation to study abroad and to provide baseline information for nurturing globally-minding dental students. Materials and Methods: Students from the first to the sixth year (n = 332), at the Faculty of Dentistry, Tokyo Medical and Dental University, were invited to participate in the study and were asked to complete a self-administered questionnaire, written in Japanese. Self-reported English competency was assessed in four areas: reading; writing; speaking; and listening. Students were asked to score their competencies in these areas using a 10-point visual analogue scale (VAS). Of the total eligible students, 312 (94%) participated. Results: The results of the study suggest that competency and confidence in the English language could be one of the biggest obstacles for students to study abroad. Students with an interest in studying abroad were highly motivated and studied English independently. Discussion and Conclusions: Japanese educational authorities and dental schools may need to consider a range of incentives and motivational strategies to nurture clinicians, educators and researchers who can function confidently in an increasingly borderless world. These could include increasing exposure to English language skills through mandatory lectures and seminars in English, increasing the range of inbound international students through student exchange programmes and encouraging students to develop and communicate with international colleagues. The Faculty of Dentistry at Tokyo Medical and Dental University has begun to increase exposure of students to English through the regular curriculum. Outcomes will be followed and evaluated.

    Objectives. The objective of this study was to assess whether saliva viscosity, measured by a viscometer, was a predictor of oral malodor. Materials and methods. The subjects were 617 patients who visited an oral malodor clinic. The organoleptic test (OT) was used for diagnosis of oral malodor. An oral examination assessed the numbers of teeth present and decayed teeth as well as the presence or absence of dentures. Further, periodontal pocket depths (PD), gingival bleeding, dental plaque and tongue coating were investigated. Unstimulated saliva were collected for 5 min. Saliva viscosity was measured with a viscometer. Logistic regression analysis with oral malodor status by OT as a dependent variable was performed. Possible confounders including age, gender, number of teeth present, number of decayed teeth, number of teeth with PD &gt;= 4 mm, number of teeth with bleeding on probing, presence or absence of dentures, plaque index, area of tongue coating, saliva flow rate, saliva pH and saliva viscosity were used as independent variables. Results. Saliva viscosity (p = 0.047) along with the number of teeth with PD &gt;= 4 mm(p = 0.001), plaque index (p = 0.037) and area of tongue coating (p &lt; 0.001) were significant variables for oral malodor. Subjects with a higher number of teeth with PD &gt;= 4mm(OR = 1.32), plaque index (OR = 2.13), area of tongue coating (OR = 3.17) and saliva viscosity (OR = 1.10) were more likely to have oral malodor compared to those with lower values. Conclusions. The results suggested that high saliva viscosity could be a potential risk factor for oral malodor.

    Background: Two types of mucins, MUC7 and MUC5B constitute the major salivary glycoproteins, however their metabolic turnover has not been elucidated in detail to date. This study was conducted to examine turnover of MUC7 and MUC5B in saliva, by focusing on the relationship between their deglycosylation and proteolysis.
    Methodology/Principal Findings: Whole saliva samples were collected from healthy individuals and incubated at 37 degrees C in the presence of various protease inhibitors, sialidase, or a sialidase inhibitor. General degradation patterns of salivary proteins and glycoproteins were examined by SDS-polyacrylamide-gel-electrophoresis. Furthermore, changes of molecular sizes of MUC7 and MUC5B were examined by Western blot analysis. A protein band was identified as MUC7 by Western blot analysis using an antibody recognizing an N-terminal epitope. The MUC7 signal disappeared rapidly after 20-minutes of incubation. In contrast, the band of MUC7 stained for its carbohydrate components remained visible near its original position for a longer time indicating that the rapid loss of Western blot signal was due to the specific removal of the N-termimal epitope. Pretreatment of saliva with sialidase facilitated MUC7 protein degradation when compared with samples without treatment. Furthermore, addition of sialidase inhibitor to saliva prevented proteolysis of N-terminus of MUC7, suggesting that the desialylation is a prerequisite for the degradation of the N-terminal region of MUC7. The protein band corresponding to MUC5B detected in both Western blotting and glycoprotein staining showed little sign of significant degradation upon incubation in saliva up to 9 hours.
    Conclusions/Significance: MUC7 was highly susceptible to specific proteolysis in saliva, though major part of MUC5B was more resistant to degradation. The N-terminal region of MUC7, particularly sensitive to proteolytic degradation, has also been proposed to have distinct biological function such as antibacterial activities. Quick removal of this region may have biologically important implication.

    Background and Objective: Only a few clinical research studies have assessed different therapeutic approaches to oral malodor in subjects affected by periodontal diseases. The aim of this study was to evaluate the effects of periodontal treatment and tongue cleaning on oral malodor parameters in periodontitis and gingivitis patients.
    Material and Methods: The subjects were 102 periodontitis and 116 gingivitis patients with oral malodor. Oral malodor was measured by organoleptic test and Oral Chroma (TM). Oral health status, including tooth conditions, periodontal health, tongue coating and proteolytic activity of the BANA test in tongue coating were assessed. Subjects in each periodontal disease group were randomly assigned into two subgroups depending on the sequence of treatment: periodontal treatment and tongue cleaning. Oral malodor and oral health parameters were compared by groups and sequence of treatment.
    Results: For subjects in the periodontitis group, there were statistically significant reductions in oral malodor after periodontitis treatment or tongue cleaning; however, major reductions were found after periodontitis treatment. For those in the gingivitis group, there were also statistically significant reductions in oral malodor after gingivitis treatment or tongue cleaning, but the most marked reductions were observed after tongue cleaning. At the completion of treatment, all oral malodor parameters fell below the threshold levels in all subgroups.
    Conclusion: The present study indicated that periodontal treatment played an important role and tongue cleaning contributed to a lesser extent to reduction in oral malodor in periodontitis patients. In contrast, tongue cleaning alone can be the primary approach to reduce oral malodor in gingivitis patients.

    Volatile sulfur compounds (VSCs), which are major sources of oral malodor, are produced by putrefactive activities of bacteria. Saliva provides easily degradable protein substrates, and most proteins are glycosylated. We hypothesized that oral malodor would be associated with enhanced proteolysis or deglycosylation in saliva. The purpose of this study was to evaluate properties of glycoproteins in saliva and assess their association with VSC levels. Subjects were 88 patients who visited "the Fresh Breath Clinic", Dental Hospital, Tokyo Medical and Dental University. They were classified into malodor (n = 67) and non-malodor (n = 21) groups. After collecting saliva, the amounts of the total proteins and carbohydrate were determined. Molecular size distributions of salivary proteins/glycoproteins were analyzed by SDS-polyacrylamide gel electrophoresis (SDS-PAGE). The amount of the total salivary proteins was significantly higher in the malodor group. Major proteins/glycoproteins observed in SDS-PAGE analyses showed similar distributions between the two groups. In the malodor group, the salivary protein concentrations were positively correlated with the CH3SH levels (p &lt
    0.05), and the carbohydrate contents were negatively correlated with the H2S levels (p &lt

    Background: Previous research has shown the oxidizing properties and microbiological efficacies of chlorine dioxide (ClO(2)). Its clinical efficacies on oral malodor have been evaluated and reported only in short duration trials, moreover, no clinical studies have investigated its microbiological efficacies on periodontal and malodorous bacteria. Thus, the aim of this study was to assess the inhibitory effects of a mouthwash containing ClO(2) used for 7 days on morning oral malodor and on salivary periodontal and malodorous bacteria. Methods/Design: A randomized, double blind, crossover, placebo-controlled trial was conducted among 15 healthy male volunteers, who were divided into 2 groups. Subjects were instructed to rinse with the experimental mouthwash containing ClO(2) or the placebo mouthwash, without ClO(2), twice per day for 7 days. After a one week washout period, each group then used the opposite mouthwash for 7 days. At baseline and after 7 days, oral malodor was evaluated with Organoleptic measurement (OM), and analyzed the concentrations of hydrogen sulfide (H(2)S), methyl mercaptan (CH(3)SH) and dimethyl sulfide ((CH(3))(2)S), the main VSCs of human oral malodor, were assessed by gas chromatography (GC). Clinical outcome variables included plaque and gingival indices, and tongue coating index. The samples of saliva were microbiologically investigated. Quantitative and qualitative analyses were performed using the polymerase chain reaction-Invader method. Results and Discussion: The baseline oral condition in healthy subjects in the 2 groups did not differ significantly. After rinsing with the mouthwash containing ClO(2) for 7 days, morning bad breath decreased as measured by the OM and reduced the concentrations of H(2)S, CH(3)SH and (CH(3))(2)S measured by GC, were found. Moreover ClO(2) mouthwash used over a 7-day period appeared effective in reducing plaque, tongue coating accumulation and the counts of Fusobacterium nucleatum in saliva. Future research is needed to examine long-term effects, as well as effects on periodontal diseases and plaque accumulation in a well-defined sample of halitosis patients and broader population samples.

    The purpose of this study was to identify factors influencing the motivation for regular dental check-ups and anxiety about future dental problems in the elderly.
    Regular dental check-ups are important to maintain oral health. Anxiety affects the quality of life of the elderly. However, few studies have investigated the factors influencing the motivation for regular dental check-ups and anxiety in the elderly.
    Among 452 elderly persons, 215 participants provided useful answers to our questionnaire. The questionnaire consisted of items concerning oral and living conditions of the subjects. Logistic regression analysis was conducted.
    The mean age of the study sample was 75.7 +/- 5.8 years. The percentage of edentulous subjects was 12.1%. The motivation for regular dental check-ups was significantly positively correlated with the number of teeth present (p &lt; 0.01), using inter-dental brushes (p &lt; 0.01), being on regular prescribed medication (p &lt; 0.05), and having experience of providing oral care for the bedridden (p &lt; 0.05). Anxiety about future oral problems was significantly associated with self-rated health status (p &lt; 0.01) and the presence of dentists who could be counted on to provide domiciliary visits (p &lt; 0.05).
    The participants of this study had a high motivation for regular dental check-ups, but were concerned about the future, particularly if they became bedridden.

    Oral malodor was evaluated before rinsing, right after rinsing and every 30 minutes up to 4 hours with OM, and concentrations of hydrogen sulfide (H(2)S), methyl mercaptan (CH(3)SH) and dimethyl sulfide ((CH(3))(2)S), the main VSCs of human oral malodor, were evaluated with GC.
    Results: The baseline oral condition in the subjects in the 2 groups did not differ significantly. The mouthwash containing ClO(2) improved morning bad breath according to OM and reduced concentrations of H(2)S, CH(3)SH and (CH(3))(2)S according to GC up to 4 hours after rinsing. OM scores with ClO(2) were significantly lower than those without ClO(2) at all examination times. Significant reductions in the concentrations of the three kinds of VSCs measured by GC were also evident at all examination times. The concentrations of the three gases with ClO(2) were significantly lower than those without ClO(2) at most examination times.
    Conclusion: In this explorative study, ClO(2) mouthwash was effective at reducing morning malodor for 4 hours when used by healthy subjects.

  • Clinical oral malodor measurement with a portable sulfide monitor Reviewed

    OBJECTIVE: The aim of this study was to evaluate the clinical ability of Breathtron (R) by comparing it with other malodor measurement procedures: the organoleptic test (OT) and gas chromatography (GC).
    SUBJECTS AND METHODS: Patients were 475 patients who visited a fresh breath clinic. Oral malodor was measured with the OT, GC, and Breathtron (R). Correlation analysis and two linear regression analyses were conducted to examine the relationship of the Breathtron (R) values with OT scores and volatile sulfide compound (VSC) concentrations by GC: i.e. the regression of Breathtron (R) on OT and the regression of Breathtron (R) on total VSCs by GC. Receiver operating characteristics (ROC) analysis was conducted to investigate the sensitivity and specificity of Breathtron (R).
    RESULTS: The Breathtron (R) values were significantly correlated with OT and VSCs by GC. In the regression analysis, predicted Breathtron (R) values were 199.3 and 520.1 ppb for OT scores 1 and 2, and reasonably close to total VSCs by GC between 550 and 750 ppb. The ROC analysis demonstrated that Breathtron (R) is a useful and valuable adjunct measurement tool.
    CONCLUSIONS: Breathtron (R) is a simple, rapid and reliable appliance for screening oral malodor if an appropriate malodor threshold level is chosen.

    Previous studies have examined the relationship between cognition and oral health among older populations. To further understand these relationships, this study examined the associations between cognitive function, chewing capacity and the number of natural teeth present in community dwelling older males in Australia.
    Data were obtained from cross-sectional analysis of the 4th wave of The Concord Health and Ageing in Men Project (CHAMP). 369 community dwelling males aged 78 years or over participated in the study. Cognitive function was measured by utilizing Mini-Mental State Examination (MMSE). Chewing capacity was determined by a nutritionist administered survey of participants capacity to chew 11 food items, and oral health data were collected by calibrated dental professionals. 17 participants (4.6%) had cognitive impairment (MMSE score < 24). Ordinal regression analysis adjusting for confounders showed participants with fewer than 20 natural teeth (OR = 1.62; 95% CI 1.07- 2.43) and with limited chewing capacity (OR = 1.61; 95% CI 1.03- 2.49) were more likely to be cognitively impaired. Further research is required to elaborate the causative pathways of these associations.
    The study suggests that older men with fewer natural teeth and those with limited chewing capacity are at a higher risk of cognitive impairment.

    Assessing perceived dental treatment need is important for understanding and predicting health behaviors. The objective of this study was to examine the association of participants in the Concord Health and Ageing in Men Project perceived treatment needs with their normative oral health and service use. Data were obtained from a cross-sectional analysis of 274 dentate community dwelling men aged 78 or over. Most older men viewed their oral health positively, but more than half of the men with dental caries or periodontal diseases did not see themselves as required dental treatment. Underestimating their perceived dental treatment need could be a barrier to behavioral change of taking recommended preventive actions.

    北欧諸国では「口腔保健医療の質評価指標」を使用して口腔保健医療に関する国際比較を2010年より実施している。本報告では日本と北欧4か国の比較、指標の妥当性や有用性の検討を行った。その結果、国によって調査年、対象年齢、調査方法が異なっているなど、共通指標を用いて国際比較することは難しいことが判明した。

    東京医科歯科大学では、海外研修プログラムによって、学生の時から海外研修に参加する機会を設けている。学生のニーズにあった研修を提供するために、学生を対象に国際交流に関する意識を行った。その結果、研修プログラムに参加したいと答えたものの、参加にあたり「不安である」と回答した学生が60%以上で、最も多かったのが語学に対する不安であった。

    本研究の目的は海外研修プログラムに参加した学生の意識変化について調査することである。東南アジアから受入れた海外学生24名に対して、研修実施前後に質問票調査を実施し、意識変化について検討を行った。その結果、研修後の英語能力に対する自信、および日本人学生及び本学に対する評価が高くなっていた。実際に日本の大学に来て学習することで、日本の歯学教育、歯科医療及び研究レベルが高い水準にあることを認識したことが示唆された。

    北欧諸国では「口腔保健医療に関する評価指標」を作成するためのプロジェクトを立ち上げ、検討を行っている。本研究では北欧の歯科専門家らが提案した口腔保健医療の評価指標を紹介し、日本の状況と国際比較を行った。分析の結果、北欧4カ国においても、共通指標を用いて口腔保健状況や歯科医療状況を国際比較することは簡単ではないことが判明した。医療の国際標準化および国際協調が進展していく中、口腔保健医療に関する世界共通の評価指標を開発していくことが重要と思われた。

    本研究は口臭患者の主観的な口臭評価およびQOLと口臭の実測値の関連を明らかにし、今後の口臭治療に役立てることを目的とする。対象者は、口臭外来を受診した825名である。分析の結果、主観的な口臭評価は口臭実測値と関連していなかった。口臭患者の中には、口臭値が低いのにもかかわらず、口臭があると考え、不安になる者も多い。口臭治療では、実際の口臭値を下げるだけでなく、患者の不安を軽減することが大切であると考えられた。

    厚生労働省は1957年より6年ごとに歯科疾患実態調査を実施している。本研究では、2011年の歯科疾患実態調査の結果をもとに、乳歯のう蝕有病状況について検討を行った。過去の調査と比較して乳歯のう蝕有病状況の改善が認められた。その背景には、母子歯科保健事業による歯科健康診査や保健指導、フッ化物応用の普及、歯科医療従事者の増加、保護者の口腔の健康への関心度の向上などとの関連が考えられた。

    本研究では歯科疾患実態調査結果をもとに、歯周疾患の有病状況について検討した。2011年の歯周疾患の有病状況を2005年と比較すると、歯周ポケット4mm以上の者は高齢者では増加し、それ以外の年代では減少傾向がみられた。高齢者の歯周疾患の有病状況が悪化しているのは、対象歯が多く残るようになったためと考えられた。今後は、高齢者において歯周疾患の予防に積極的に取り組んで、残存歯を良い状態で保持していくことが重要であると考えられた。

    本研究では、歯科疾患実態調査の結果から永久歯についてのう蝕の有病状況について報告する。我が国における永久歯のう蝕有病者率は全体で90%を越えており、依然として国民的疾患であるといえるが、DMFTは1981年の調査をピークにすべての年齢層において改善傾向が認められる。DF歯保有者率は45歳以降の階級では増加傾向が認められたが、現在歯数の増加によるためと考えられ、高齢者においてう蝕治療ニーズが増加することを示唆していると考える。

    本研究では、2011年の歯科疾患実態調査の結果をもとに、15歳以上の永久歯の保有状況および補綴状況について調査を行った。本調査の結果から、我が国の成人の歯の保有状況は改善していることが確認できた。今回の調査で追加した「かみあわせの状況」では、補綴物は除外して現在歯のみで判定するが、高齢になると両側とも咬合接触のない者の割合が大きく増加していることが判明した。今後は、歯の数を増加することだけを目標とするのではなく、上下顎の歯の咬合状態にも注目して歯を保持していくことが必要であると考えられた。

    韓国歯科大学教育協議会が2012年にまとめた韓国歯科医学教育白書を翻訳し、大韓民国(韓国)の歯科大学の現況について調査を行った。歯科大学は11校あり、教育期間が6年制の大学が4校、学士入学の4年制の大学が7校ある。4年制の大学は、歯学部ではなく専門大学院として教育を行い、卒業時には修士の資格が得られる。我が国の隣国である韓国と歯学教育に関する情報交換を行うことは、日韓両国の歯学教育の改善や教育の国際標準化に有益であると思われた。

    英国では、歯科医師になることを希望する者を対象としたキャリアガイドブックが発行されている。歯科大学受験生の数が減少している我が国においても、今後、優秀な学生を確保していくために、英国で実施している取り組みは参考になると考えられる。そこで、本研究ではこの本の内容について検討を行った。このキャリアガイドブックを参照することは、日本にとっても有益と考えられた。

    本研究では、デンマークの学校歯科保健制度について調査を行った。日本の学校歯科保健制度では、歯列・咬合の状態を0・1・2で評価しているが、学校歯科医によってその評価基準は大きく異なっている。そのため、矯正治療の適応者を選別するデンマークのシステムは参考になると思われた。また、デンマークでは成人の定期歯科健診受診率は60%と高いが、これには高校生になると学校歯科室から開業歯科医院への橋渡しがあることも影響していると示唆された。

    本研究では、英国の公的保険制度、ナショナルヘルスサービス(NHS)のもとで提供されている矯正歯科治療について、日本と比較した。英国ではNHSと契約した歯科診療所や病院では、条件を満たせば、矯正歯科治療も提供されている。18歳以下で矯正治療の必要性があると診断された場合、本人負担はない。2008年度のウェールズでの調査では約38,000人の対象年齢のうち約11,000名(29%)が矯正治療を受けていた。

    本研究では米国と日本の歯科治療費を比較検討した。米国と比較すると日本の歯科治療費は全般的に安いことが判明した。米国の大学病院では患者が治療前に料金表を見て術者を選択することができる。歯科学生の行う処置が最も安い料金に設定されており、臨床研修医は学生の約1.5倍、教員は学生の約2倍であった。このシステムは患者、術者双方に有益と考えられ、日本の歯科大学での臨床教育の際に参考になると思われた。

    The common marmoset may offer several advantages over other non-human primates in terms of its smaller size, shorter life-span, and dental anatomy identical to humans. Our results indicated that the types of oral microbiomes detected among human and marmoset were nearly indistinguishable. In contrast, the oral microbiomes of our human and marmoset subjects were distinctly different from those reported for rats and dogs, which are popular research animals. The oral microbiomes of marmosets showed greater diversity than those of humans. However, the oral microbiota of marmosets exhibited less variation than those of humans. The characteristics of its oral microbiota, combined with other technical advantages, suggest that the marmoset may provide the best animal model thus far for the study of oral health. This study characterized the oral microbes of the marmoset, thereby providing information to support future application of the marmoset as a model for age-related oral disease.

    In the common marmoset, the problems associated with aging such as jaw and cranium, periodontal tissue, jaw movement, chewing / swallowing, respiratory function, etc. were clarified. Then, the utility of common marmoset as a human elderly model was evaluated. Three-dimensional X-ray motion pictures (cineradiograph) were taken with the elderly common marmosets in unconstrained state, and comparative analyzes of functions such as jaw movement, chewing / swallowing, respiration were conducted with reference to juvenile controls. In results, anatomical and functional changes was confirmed, and metagenome analysis of oral bacterial flora, bacteria common to humans and marmosets and the species-specific bacteria were clarified. Changes in the oral flora with aging were also observed. Common marmoset was confirmed to be highly useful as an oral disease model of human elderly.

    General degradation patterns of salivary proteins and glycoproteins were examined by SDS-polyacrylamide-gel-electrophoresis. A protein band was identified as MUC7 by Western blot analysis using an antibody recognizing an N-terminal epitope. The MUC7 signal disappeared rapidly after 20-minutes of incubation. In contrast, the band of MUC7 stained for its carbohydrate components remained visible near its original position for a longer time indicating that the rapid loss of Western blot signal was due to the specific removal of the N-termimal epitope. MUC7 was highly susceptible to specific proteolysis in saliva, though major part of MUC5B was more resistant to degradation. The N-terminal region of MUC7, particularly sensitive to proteolytic degradation, has also been proposed to have distinct biological function such as antibacterial activities. Quick removal of this region may have biologically important implication.

    We developed a Japanese oral health literacy index and investigated how oral health literacy was related to oral health behaviors and clinical oral health status.
    A self-administered questionnaire and dental examination collected information from 1,025 adult residents of Akita prefecture, including oral health literacy items (20 dental terms and 8 dental knowledge sentences). The total score for the oral health literacy was a simple sum of responses, ranging from 0 to 28. For statistical analysis, the oral health literacy score was categorized into tertile levels of the total score: low (0-12), medium (13-18), and high (19-28).
    The higher a subject’s oral health literacy level, the more often they had regular dental check-ups and the better their oral hygiene status. Furthermore, subjects with higher oral health literacy level had higher numbers of natural teeth and FTUs. Oral health literacy was associated with differences in oral health behaviors and clinical oral health status.
