    OBJECTIVE: Malignant lymphoma (ML), a lymphocyte-derived malignancy, is classified into Hodgkin lymphoma and non-Hodgkin lymphoma (NHL). In the oral cavity, NHL is the predominant type. However, diagnosing oral ML is challenging owing to its clinical similarity to other lesions. Diffusion-weighted magnetic resonance imaging and apparent diffusion coefficient (ADC) values may aid in differential diagnosis. Therefore, in the present study, we aimed to evaluate the usefulness of ADC in distinguishing ML from squamous cell carcinoma (SCC) and salivary gland carcinoma (SGC). METHODS: This retrospective study included 11 patients with ML, 35 with SCC, and 19 with SGC treated at our institution between 2012 and 2022. Clinical data and ADC values were analyzed, and magnetic resonance imaging was performed using a 3.0 T scanner. RESULTS: Clinical assessment alone frequently resulted in misclassification of ML cases, with most cases initially suspected of having benign or other malignant tumors. ML demonstrated markedly lower ADC values (mean 0.67 ± 0.13 × 10⁻3 mm2/s; range 0.45-0.87) than SCC (1.21 ± 0.14) and SGC (1.30 ± 0.27) (both P < 0.0001), with minimal overlap between the groups. An ADC cutoff of 0.87 × 10⁻3 mm2/s identified ML with 100% sensitivity and 98.2% specificity (area under the curve: 0.99), indicating a strong diagnostic differentiation from carcinoma cases. CONCLUSION: ADC assessment may facilitate early suspicion of oral ML and help clinicians in prioritizing it within the differential diagnosis. Integrating ADC values into the diagnostic workflow could facilitate early recognition and more appropriate biopsy strategies, thus supporting timely management.

    Objective Patients with oral squamous cell carcinoma (OSCC) may choose best supportive care (BSC) as initial treatment over active treatment for several reasons. However, no previous reports have compared the clinical characteristics of OSCC patients opting for BSC as initial treatment with those receiving standard treatment. This study retrospectively analyzed these differences to identify the factors influencing this decision. Materials and methods This retrospective study was conducted at Niigata University Medical and Dental Hospital, using medical records within January 2018-December 2022 (follow-up until June 2023). Among 135 patients with primary OSCC, received BSC (BSC group) and underwent standard treatment according to the disease stage (non-BSC group). Data included demographic and tumor characteristics, Eastern Cooperative Oncology Group Performance Status (ECOG-PS), underlying diseases, neutrophil-lymphocyte ratio (NLR), albumin levels, living situation (with family, in a care facility, or alone), rationale for BSC selection (untreatable due to poor general condition or no desire for treatment), overall survival (OS) rate, and disease-specific survival (DSS) rate. Age, NLR, and albumin levels were assessed using Student's t-test. Stage classification, ECOG-PS, and living situation were analyzed using the chi-square test. OS and DDS were calculated using the Kaplan-Meier method and compared using the log-rank test. Multiple logistic regression analysis identified factors associated with BSC. Results  The BSC group included 11 males and 13 females (mean age: 84 years; range: 47-92). The non-BSC group included 67 males and 44 females (mean age: 67.4 years; range: 31-89). Female predominance was higher in the BSC group (male-female ratio: 1:1.2) than in the non-BSC group (1:0.7). Primary sites were the lower gingiva in the BSC and the tongue in the non-BSC group. The BSC group exhibited higher NLR (p=0.50) and a significantly lower albumin level (p<0.001). Stage classification (I/II vs. III/IV) and ECOG-PS (0-1 vs. 2-4) differed significantly between groups (p<0.001). Cardiovascular disease was the most frequent comorbidity in both groups. Living situation exhibited no significant differences (p=0.99). The cumulative one-year OS rate in the BSC group was 29.8%, compared to 97.2% and 81.5% in the non-BSC group at one and five years, respectively. The cumulative one-year DSS rate in the BSC group was 31.9%, and 97.2% and 82.8% at one and five years in the non-BSC group were p<0.001 for OS and DSS. Age (OR: 1.20, 95% CI: 1.10-1.36; p=0.0008), stage classification (I/II vs. III/IV) (OR: 13.79, 95% CI: 2.34-137.75; p=0.0026), psychiatric disorder (OR: 8.73, 95% CI: 2.34-137.75; p=0.021), albumin level (OR: 0.03, 95% CI: 0.002-0.24; p=0.003) were factors in BSC. Among patients aged ≥75 years, stage classification (I/II vs. III/IV) (OR: 19.08, 95% CI: 2.51-321.07; p=0.0027), albumin level (OR: 0.003, 95% CI: 9.0×10-6-0.098; p=0.01) remained significant predictors of BSC. Conclusion Patients opting for BSC tended to be older. Stage classification and albumin level were key factors in BSC across all patients, including those aged ≥75 years. Early diagnosis and timely intervention are vital to improve treatment opportunities. Collaborative discussions among patients, families, and healthcare providers are crucial to develop individualized care plans, ensuring that patients receiving BSC can approach end-of-life care with dignity.

    R2TP is a well-conserved molecular chaperone complex, composed of Pontin, Reptin, RPAP3, and PIH1D, in eukaryotes. Recent studies have suggested an involvement of R2TP in cancer development. However, it remains unclear if it is related to the development of oral squamous cell carcinoma (OSCC), which is the most common type of oral cancer. Here, we identify and investigate the function of R2TP in OSCC development. Immunohistochemical analysis reveals that all of the R2TP components are strongly expressed in normal oral epithelia and OSCC tissues, where actively proliferating cells are abundant. Co-immunoprecipitation assay identifies that R2TP components form a protein complex in OSCC-derived HSC4-cells. Knockdown experiments show that all R2TP components, except for RPAP3, are required for the cell proliferation and migration of HSC-4 cells. Furthermore, we reveal that Pontin contributes to a gain-of-function (GOF) activity of mutp53-R248Q in HSC-4 cells by regulating phosphorylation levels of mutp53 at Ser15 and Ser46. To our knowledge, this study is the first to report the functional involvement of R2TP and its components in the malignant characteristics of OSCC cells.

    R2TP/PAQosome (particle for arrangement of quaternary structure) is a novel multisubunit chaperone specialized in the assembly/maturation of protein complexes that are involved in essential cellular processes such as PIKKs (phosphatidylinositol 3-kinase-like kinases) signaling, snoRNP (small nucleolar ribonucleoprotein) biogenesis, and RNAP II (RNA polymerase II) complex formation. In this review article, we describe the current understanding of R2TP/PAQosome functions and characteristics as well as how the chaperone complex is involved in oncogenesis, highlighting DNA damage response, mTOR (mammalian target of rapamycin) pathway as well as snoRNP biogenesis. Also, we discuss its possible involvement in HNSCC (head and neck squamous cell carcinoma) including OSCC (oral squamous cell carcinoma). Finally, we provide an overview of current anti-cancer drug development efforts targeting R2TP/PAQosome.

    2018年9月～2019年10月に全身麻酔下での人工関節置換術施行に際し、周術期口腔機能管理計画を策定した患者71人(男性20人、女性51人、平均年齢72.0歳)を対象に、周術期口腔機能管理の現状を調査した。現在歯数は平均20.1本で、歯周炎と診断されたのは55人(77.5%)、要抜歯の歯を有するのは22人(31.0%)であり、管理計画策定から手術までの期間は平均29.1日であった。かかりつけ歯科に定期的に受診していたのは30人(42.3%)であり、実際に行った術前処置として、口腔衛生指導や歯石除去は無歯顎者を除く全例で行われ、抜歯は19人に行われた。術後創部感染、術後肺炎を発症した者は2021年1月までの調査において認めなかった。

    A diverticulum is a pouch of the digestive tract mucosa that penetrates the muscular layer because of a partial defect in the muscle. It has often been reported in the digestive tract mucosa, but is extremely rare in the oral cavity. We report a case of a diverticulum-like lesion of the buccal mucosa. A 60-year-old man visited his regular dentist for dental treatment. An atheroma-like lesion was noted near the papilla of the parotid duct in the right buccal mucosa. The right maxillary molar was extracted because of a suspected odontogenic infection, but his symptoms did not improve, and he therefore consulted our hospital. Two pouches, one measuring 10 mm in depth and the other 5 mm in depth, were noted, and both contained food debris. These lesions were diagnosed as diverticulum-like lesions of the buccal mucosa and were resected with the patient under general anesthesia. A partial defect of the buccinator muscle was recognized under the pouch. Histopathologically, the buccal mucosa penetrated the buccinator muscle and reached the fatty layer, and buccal glands were located at the bottom of the pouches. These findings suggested the etiology of this lesion. No recurrence has occurred as of 1 year 6 months after surgery.

  • 2014年における下顎智歯抜歯599歯の臨床的検討 : 下顎智歯抜歯治療体系の現況と考察

    下顎智歯抜歯は口腔外科において頻度の高い処置である。個々の症例で難易度や合併症リスクが異なるため，体制の整った施設での加療が望ましい症例も多い。今回，紹介医との情報共有を目的に当科における現況を調査した。対象は2014年に施術した下顎智歯抜歯461例599歯とし，1.患者基本情報，2.画像情報，3.手術情報，4.術後合併症に関して調査した。1.性別では男性198例246歯，女性263例353歯，平均年齢は30.5±12.6歳であった。紹介率は92.9%であり，開業歯科医院からの紹介は全体の7割を超えた。2.Winter分類は，水平305歯，近心165歯，垂直118歯，Pell-Gregory分類は，IA:242歯，ⅡA:120歯，IB:107歯，apex positionはAp1:258歯，Ap0:185歯，Ap2-s:94歯の順で多かった。CT撮影は185歯(30.8%)で追加的に実施され，Ap2-s以上でその割合が急増していた。3.施術場所は，514歯が外来手術室，85歯が中央手術室であり，麻酔別では，全身麻酔82歯，静脈内鎮静法27歯，局所麻酔単独490歯であった。全身麻酔の選択理由は外科的侵襲の大きさや両側同時抜歯が最も多く，静脈内鎮静法の選択理由は手術に対する不安(歯科恐怖症を含む)が最も多かった。4.知覚異常は6歯(1.0%)で認められた。本調査結果を紹介医と共有し，患者説明や紹介の判断基準として有効に活用されれば，今後の地域における病診連携の拡大にもつながることが期待される。
