    Background: Cognitive impairment, including dementia, is a growing global health challenge, particularly as populations age. Previous studies have identified periodontitis and diabetes mellitus as modifiable risk factors for dementia, potentially linked through systemic inflammation. We hypothesize that systemic inflammation induced by periodontitis may contribute to an increased risk of cognitive impairment. Therefore, this study aims to explore the correlation between periodontal inflammation and the risk of Mild Cognitive Impairment (MCI) in type II diabetes mellitus. Materials and Methods: Baseline data analysis was performed as an analytical cross-sectional study among diabetic patients aged 40 and older who met the inclusion criteria from a randomized controlled trial (RCT) from November 2020 to April 2023. Periodontal inflammation was measured using the Periodontal Inflamed Surface Area (PISA) score. The MCI risk score was calculated using blood samples to assess eight protein markers correlated to MCI (ApoA1, TTR, C3, Albumin, ApoC1, A1BG, A2AP, and HPX). Fisher's exact test and Spearman's correlation analysis were performed. Results: 29 T2DM patients were included in the study. There was a significant difference in MCI risk score between the low and high PISA levels group (p < 0.05). Patients with low PISA scores tend to have a lower risk of MCI (p < 0.00). Variables correlated with MCI risk are PISA (ρ = 0.37; p < 0.05) and TTR levels (ρ = -0.51; p < 0.01). ApoA1 has a correlation with CRP (ρ = 0.42; p < 0.05) and IL-6 (ρ = 0.43; p < 0.05), and C3 (ρ = 0.42; p < 0.05) was correlated with CRP. Conclusions: This study found that periodontal inflammation status has a potential correlation to the risk of MCI.

    OBJECTIVES:  This article investigates the changes in blood-based biomarkers associated with mild cognitive impairment (MCI) risk in type 2 diabetic patients following local antibiotic periodontal treatment. MATERIALS AND METHODS:  A secondary analysis of data from a 24-week randomized controlled trial was conducted, involving 27 patients with type 2 diabetes mellitus and periodontitis. Participants received periodontal treatment biweekly from baseline until the 6th week of the study. Fourteen patients in intervention group received scaling with local antibiotic adjunct (Periofol 2%). The outcomes were periodontal inflammation score, which was measured using periodontal inflamed surface area, the inflammation markers levels (tumor necrosis factor-α, C-reactive protein, and interleukin [IL]-6), and MCI risk score, which was assessed using protein plasma analysis through blood test. The evaluations were performed at baseline and week 24th in both groups. The changes in periodontal inflammation scores, inflammation parameters, and MCI risk in baseline and week 24th were analyzed. STATISTICAL ANALYSIS:  The Wilcoxon signed-rank test was used for within-group analysis and the Mann-Whitney U test was utilized for between-group analysis. RESULTS:  Periodontal parameters were improved in both groups (p < 0.05). IL-6, complement C3, and alpha-2-antiplasmin levels were significantly decreased in the intervention group (p < 0.05). In between-group comparisons, there was a significant difference between the control and intervention groups in apolipoprotein A1, apolipoprotein C1, and alpha-1-B glycoprotein levels in week 24th (p < 0.05). CONCLUSION:  Even though the periodontal status showed significant improvement after being given local antibiotic periodontal treatment, the changes in MCI risk proteins plasma remained unclear.

    Abstract

    Background and Aims:

    The COVID-19 pandemic has significantly impacted various aspects of daily life, including oral health. However, limited research has explored the effects of the pandemic on oral health perceptions, hygiene behaviors, and their subsequent influence on oral health-related quality of life (OHRQoL) in underserved settings. This study aims to assess changes in oral health perceptions and behaviors during the pandemic and their impact on OHRQoL in Myanmar.

    Materials and Methods:

    A cross-sectional study was conducted in 2023 at a township health center in Yangon, Myanmar. A total of 158 participants were included in the study. Clinical oral assessments were performed alongside face-to-face interviews, which collected data on sociodemographic characteristics, oral health perceptions and behaviors, psychological factors, and OHRQoL. Descriptive statistics, chi-square tests, Mann–Whitney U tests, Kruskal–Wallis tests, and regression analyses were used to explore associations and predictors of OHRQoL.

    Results:

    Among the 158 participants, 30.4% reported a decline in oral health perception, and 27.8% reported negative changes in oral hygiene behaviors during the COVID-19 pandemic. These changes were significantly associated with a history of COVID-19 infection, depressive symptoms, and bleeding on probing during oral examinations. Regression analysis revealed that worsened oral health perception (B = −2.59; 95% confidence interval [CI] = −5.00, −0.18) and negative changes in hygiene behaviors (B = −2.27; 95% CI = −4.57, −0.02) were associated with poorer OHRQoL. However, these associations became nonsignificant after adjusting for confounding factors. Notably, a history of COVID-19 infection requiring hospitalization remained a statistically significant predictor of poorer OHRQoL in all models.

    Conclusions:

    A significant proportion of participants reported worsened oral health perception and negative changes in oral hygiene behaviors during the COVID-19 pandemic, which contributed to a decline in OHRQoL. However, the influence of other confounding factors, such as psychological and socioeconomic variables, needs to be carefully considered. Clinically, these findings highlight the importance of integrating psychological support and targeted oral health interventions in the post-pandemic recovery phase, particularly in underserved settings. Public health initiatives should address the long-term impacts of the pandemic on oral health behaviors and perceptions to improve OHRQoL in affected populations.

    ABSTRACT

    Objectives

    This study aimed to assess the intricate relationship between salivary IgA antibody levels to PAc (361–386) (PPA), mutans streptococci colonization, and root caries development in older adults.

    Materials and Methods

    This study included 307 participants aged 76 years residing in Niigata city, Japan. Clinical oral examinations were performed at baseline in 2004 and 1 year later, during which the total number of untreated and treated root caries was assessed using the root decayed, filled tooth (DFT) index. The stimulated saliva samples were collected using the spitting method during the baseline survey. Salivary IgA antibody levels to amino acid residues 361–386 of Streptococcus mutans PAc were quantified using an enzyme‐linked immunosorbent assay. Statistical analyses, including the χ<sup>2</sup> test, Mann–Whitney U test, and logistic regressions, were performed to examine the association of increased root DFT with the independent variables.

    Results

    Among the 307 participants (53.1% men), the mean root DFT at baseline was 3.77 ± 3.66, and 36.5% of the study sample exhibited increased root DFT after 1 year with a mean increment of 0.36 ± 0.48. Participants with increase in root DFT after 1 year had significantly higher rates of low PPA levels (≤ 25th percentile) than those without increased root DFT (p = 0.020). Low PPA levels (≤ 25th percentile) were significantly more likely to have an increased risk of root caries development compared with PPA levels &gt; 25th percentile (adjusted OR: 1.88, 95% CI: 1.09–3.25).

    Conclusion

    Low PPA levels and root caries incidence correlated significantly, suggesting that low levels of salivary IgA antibody to PAc (361–386) may serve as a risk factor for increased root caries in older adults.

    Abstract

    Background

    While research has explored the risk of periodontal disease in various eye conditions, the link between dry eye and periodontal disease remains underexplored, especially in Japanese adults. This study aims to investigate the association between dry eye and periodontal disease in community-dwelling Japanese adults.

    Methods

    This study is a subset of the Uonuma cohort study, which includes Japanese adults aged 40 years and older residing in the Uonuma area of Niigata Prefecture, Japan. Participants completed a self-administered, paper-based questionnaire. Statistical analyses, including the chi-square test, independent t test, ANOVA test, and logistic regressions, were employed to assess the association of periodontal disease with independent variables.

    Results

    Among 36,488 participants (average age 63.3 years, 47.4% men), 39.3% had a history of periodontal disease, and gender differences were statistically significant (p &lt; 0.001). Significant associations were found between periodontal disease and dry eye diagnosis or symptoms. Univariable logistic regression revealed links between periodontal disease and age, gender, living status, alcohol consumption, remaining teeth, bite molar availability, and history of dry eye disease or symptoms. Multiple-adjusted regression found that doctor-diagnosed dry eye was associated with a higher likelihood of periodontal disease (odds ratio, 1.12; 95% confidence interval, 1.03–1.22). Participants who never experienced dryness or foreign body sensation had lower ORs of periodontal disease than those who always experienced such symptoms across all models.

    Conclusion

    A significant correlation was found between dry eye and periodontal disease in Japanese adults. Regular check-ups, early detection, and effective management of both conditions are strongly recommended.

    PURPOSE: To investigate the association between plasma antibody levels against Porphyromonas gingivalis (PG) and atrial fibrillation (AF) history in community-dwelling older individuals in Japan. MATERIALS AND METHODS: This study was a subset of the Uonuma cohort study, including 3091 participants aged 60-79 years. Data were collected, including AF history as a dependent variable, plasma immunoglobulin G antibody levels against PG as an independent variable, and previously reported AF risk factors and demographic information as covariates. RESULTS: The median age of the participants was 69 years. Of the 3091 participants, 1411 (45.6%) were men, and 56 (1.8%) had an AF history. AF prevalence was significantly higher in participants with higher antibody levels against PG than in those with lower antibody levels (3.0% vs 1.4%; p = 0.005). Multivariable logistic regression analysis showed that participants with higher antibody levels against PG had twofold higher odds of having AF (odds ratio = 2.13; 95% confidence interval = 1.23-3.69). Restricted cubic spline analysis indicated a nonlinear relationship between antibody levels against PG and AF history. CONCLUSION: Plasma antibody levels against PG were associated with AF history in community-dwelling older individuals in Japan.

    OBJECTIVE: Side-effects of medications cause xerostomia. There have been cases where a medication has been discontinued owing to its severe side-effects. Therefore, the xerostomia must be treated to ensure that the primary disease is managed effectively. This study analyzed the actual status of patients with medication-induced xerostomia and investigates factors associated with its improvement. METHODS: This study assessed 490 patients diagnosed with medication-induced xerostomia who had an unstimulated salivary flow of ≤0.1 mL/min and received treatment for xerostomia at a xerostomia clinic. Patient age, sex, medical history, medications used, disease duration of xerostomia, and psychological disorders were recorded. The anticholinergic burden was assessed using the Anticholinergic Cognitive Burden scale. The unstimulated salivary flow was measured by the spitting method. According to their symptoms and diagnoses, the patients were introduced to oral lubricants, instructed on how to perform massage, and prescribed Japanese herbal medicines, and sialogogues. Factors associated with the subjective improvement of xerostomia and objective changes in the salivary flow rate were recorded at six months. RESULTS: Xerostomia improved in 338 patients (75.3%). The improvement rate was significantly lower in patients with psychiatric disorders (63.6%) (P = 0.009). The improvement rate decreased as more anticholinergics were used (P = 0.018). However, xerostomia improved in approximately 60% of patients receiving three or more anticholinergics. The unstimulated salivary flow increased significantly more in patients who reported an improvement of xerostomia (0.033±0.053 mL/min) than in those who reported no improvement (0.013±0.02 mL/min) (P = 0.025). CONCLUSION: Xerostomia treatment improved oral dryness in 75.3% of patients receiving xerogenic medications in this study. If xerostomia due to side-effects of medications can be improved by treatment, it will greatly contribute to the quality of life of patients with xerogenic medications and may reduce the number of patients who discontinue medications.

    The study aimed to investigate the prevalence and associated factors of dental caries on primary teeth and to evaluate the cariogenic bacteria in dental plaque and saliva among Myanmar schoolchildren. A cross-sectional study was undertaken among 264 schoolchildren from three primary schools in the urban area of Yangon city in 2019. Clinical oral examinations, caries risk tests, and questionnaire surveys were conducted. Overall caries prevalence was 84.1% with a mean decayed, missing, filled primary teeth (dmft) count of 5.84 ± 4.57. Children’s caries experience and debris scores were significantly associated with their bacteria score level. Father’s occupation was significantly associated with caries prevalence ( P = .024) and dmft ( P = .021). Father’s occupation, snack types, habit of eating sweet food combinations, drink types, and debris score were five significant predictors of dental caries in our study. Dental caries is currently a public health problem among Myanmar children. Caries experience and oral hygiene status were strongly influenced by cariogenic bacteria.

    <title>Abstract</title>The clinical features of xerostomia induced by immune checkpoint inhibitors (ICI) are similar to those of Sjögren’s syndrome (SS), whereas the immunohistological and serological features are known to differ from those of SS. However, the radiologic imaging features of salivary glands are not yet well known. We report a case of a 56-year-old male patient diagnosed with ICI-induced xerostomia. The patient underwent various imaging examinations to investigate the condition of the salivary glands, which indicated the following: (1) less specific findings on contrast-enhanced computed tomography, (2) mixed with intermediate and low signal intensity on both <italic>T</italic><sub><italic>1</italic></sub>-weighted and fat-suppressed <italic>T</italic><sub><italic>2</italic></sub>-weighted magnetic resonance imaging and poor “salt and pepper” appearance on magnetic resonance sialography, and (3) multiple ovoid hypoechoic areas with hyperechoic bands without acute sialadenitis on ultrasound. These radiologic imaging findings suggested remarkable lymphocyte infiltration, which could be a characteristic of ICI-induced xerostomia.

    BACKGROUND AND OBJECTIVES: Few studies have assessed the possible interaction between and impact of IL-6 variants and serum α-tocopherol levels on periodontal condition in older individuals. Here, we assessed the relationship between IL-6 variants and serum α-tocopherol levels on periodontal condition by considering effect modification. MATERIAL AND METHODS: Among the study participants, 359 who were 71 years of age underwent a dental examination, biochemical analysis, and interview. After dividing the participants into tertiles based on serum α-tocopherol levels, we conducted Poisson regression analysis to compare the prevalence rate ratio (PRR) for periodontal disease markers with the IL-6 genotype (rs1800796) based on each tertile adjusted by the number of teeth present (offset). RESULTS: The PRRs of the IL-6 genotype for periodontal condition (probing pocket depth [PPD], clinical attachment level [CAL], and bleeding on probing [BOP]) which were adjusted by the number of teeth present (offset) were 1.17 (P < .001), 1.37 (P < .001), and 1.08 (P = .048), respectively. In addition, a significant association was found between the reciprocal number of PRRs of the IL-6 genotype and three serum α-tocopherol levels. The adjusted PRRs (± standard error) of the IL-6 genotypes for PPD were 0.48 (0.12) for the first group (P < .001), 1.54 (0.04) for the second group (P < .001), and 2.11 (0.03) for the third group (P < .001); similar tendencies were seen for CAL and BOP. CONCLUSION: The results of this study suggest a potential association between the IL-6 genotype and periodontal condition in relation to serum antioxidant concentrations.

    <b>Aim</b>: In dental treatment, to remove an instrument, such as dental mirror or to remove a dental cotton roll inserted in the gingivobuccal fold, often brings the patient discomfort or pain. To prevent these uncomfortable conditions, dental instruments or dental cotton rolls are moistened based on an empirical basis. However, there has been no research done evaluating whether discomfort is reduced by these techniques. The purpose of this study was to evaluate the effects of moistened techniques on the patientsʼ discomfort and the operatorsʼ ease. <b>Materials and Methods</b>: Thirty eight subjects without hyposalivation were studied. Two procedures were implemented. One was that a dry or moistened dental mirror was removed after exclusion of the buccal mucosa. The other was removal of a dental cotton roll from the gingivobuccal fold after either moistening it or leaving it dry. The subjectsʼcomfort and the operatorsʼ treatment ease were evaluated on five point scales. <b>Results</b>: During these procedures, the subjectsʼ comfort was significantly increased by moistening of both the dental mirror and the cotton roll. The ease of performing both procedures was also significantly increased for operators in the moistened condition. In addition, there was a significant positive correlation between the subjectsʼ comfort and the operatorsʼ ease. <b>Conclusion</b>: When using dental instruments and removing cotton rolls, moistening increases both the subjectsʼ comfort and operatorsʼ treatment ease.

    口腔診査結果から算出された健診受診者の現在歯数の平均値は,母集団の推定値に比べ,70歳では2.6〜3.9本,80歳では0.8〜1.8本多いことが確認された.健康度については,質問紙の回収時期による差が認められたが,全般的に現在歯数の差の程度と比べると小さかった.以上より,高齢者を対象とした歯科健診を希望制により実施する場合には,全身的な健康状態よりも現在歯数によるバイアスが生じる可能性が大きいことが示された

    2歳時健康診査受診者の内,3歳児健康診査も受診した244人の母親を対象とした.テスト群(177人)には,歯科衛生士が対象者1人あたり約10分の個別指導を実施した.母親の口腔健康状態には1年間で変化は認められなかった.歯間ブラシ又はフロスを補助清掃用具として使用している割合は,コントロール群では15.0%(ベースライン)から17.3%(1年後)と同様の使用率であったのに対し,テスト群では17.1%(ベースライン)から28.2%(1年後)と有意に増加した.又,1年後の補助用具の使用の有無に対し,有意な要因として選択されたものは,ベースライン時の補助用具の使用,歯科医より歯周疾患の情報入手及び歯科衛生士による個別指導の3変数であった

    口臭のおもな原因物質である揮発性硫黄化合物（VSC, Volatile Sulfur Compounds）は口腔内の歯周病原因菌が産生する。VSCは不快臭を持つとともに生体組織に対する毒性を持つ。したがって，口腔内のVSC濃度は歯周炎進行のリスクファクターになることを先行研究で明らかにした。それを受けて，本研究では口腔内VSC濃度による歯周炎進行予測検査としての可能性を想起した。口腔内VSC濃度は専用の機器を用いて容易に測定することが可能であり，その検査機器も近年開業歯科医院においても普及しはじめている。したがって，口腔内VSC濃度検査は簡便で非侵襲的に歯周炎進行を予測するツールとして有用である。
    しかしながら，先行研究では，古いタイプの検査機器を用いたため，個々のVSC濃度を測定できず，健常／歯周炎の重要な指標であるメチルメルカプタン濃度割合が不明であったため，基準値を明確に定義することができなかった。本研究では，単純なVSC濃度のみならず，個々のVSCの構成割合などを加味して，歯周炎進行を予測しうる基準値を明確に定義し，口腔内VSCを有用な予測因子とするためのエビデンスの確立を目指している。
    昨年度にサンプル採取の場として当初予定していた大規模疫学調査が新型コロナウイルスの流行により実施が不可能となったため，それに代わる新たな方法を模索した。本年度は，人間ドックの受診者を対象者にするため関連施設の協力が得られることとなったため，新たな研究体制と関係者間との調整に終始した。

    高齢者の増加に伴いドライマウス患者が増加している。唾液の減少は口腔内細菌叢の組成バランスを崩し、この菌叢の変化はドライマウスの病態に影響を及ぼしていると考えられる。
    新潟大学医歯学総合病院 予防歯科の外来患者を対象として、口腔細菌叢とドライマウスに伴う口腔症状との関連性について検討を行った。
    通常、口腔内細菌の検体採取としては、唾液採取や擦過サンプルを使用することが多い。ただ、今回はドライマウス患者から検体を採取するため、唾液を献体として使用することが難しい。また、ドライマウス患者は粘膜が過敏になっている方も多く、粘膜を擦過することは負担を強いることになりうる。さらに擦過サンプルは部位によって菌叢が大きく変化することが予想されることから、今回は一定時間洗口した後に吐き出してもらう洗口サンプルを使用した。
    新型コロナウイルス流行の影響により、研究の進行が遅れ気味で、現在も対象者の募集を継続中であるが、現在まで集まった洗口サンプルのうち、特に口腔乾燥と舌痛の訴えのある患者1名で、他の対象者とは異なる特徴的な菌叢が認められた。すなわち、通常、Streptococcusの割合は30～40%の範囲に収まる患者が多い中で、この患者はStreptococcusの割合が57%を占めており、そのうち、anginosus groupの菌数が特に高い値を示していた。anginosus groupは、日和見的に口腔内や深部臓器などで化膿性疾患を引き起こす細菌である。引き続き、例数を増やして口腔乾燥やそれに伴う口腔内の疼痛を訴える方の細菌叢の特徴について検討していく予定である。

    本研究は高齢者における10年後の唾液量の減少に関わる要因を検討することを目的とした。対象者が80歳および90歳時に刺激時唾液と安静時唾液（口を動かさない時の唾液）の測定と血液検査、ストレス、生活習慣の調査を行った。その結果、10年間に刺激時唾液は平均20%、安静時唾液は平均10%の減少があった。また、唾液減少のリスク因子として性差、腎機能低下およびβ3アドレナリン受容体遺伝子型が示唆された。

    認知機能検査Mini-Mental State Examination（MMSE）23 点以下（認知機能障害46名）と24点以上（健常78名）を本研究の対象者とした（平均76歳の男女）。ロジスティック回帰分析にて脳血管疾患・心筋梗塞・高血圧・糖尿病・脂質異常症の有病状況、APOE ε4 allele保有、各種血清抗体価、高感度CRP、血清アルブミンの影響を調整した後、MMSE23点以下に対する歯の1本減少のオッズ比は1.06（95%CI;1.01-1.11, P= 0.030）で有意な関連を示した。

    口臭の主な原因は舌苔(舌の上にある苔状の組織)から生ずる気体である.そのため舌苔を除去することで口臭が抑制される.本研究では植物由来のタンパク分解酵素アクチニジンによりタンパク質を多く含む舌苔が分解・除去され、口臭が減少することを検討した.結果として、アクチニジンは口の中の細菌に影響を与えることなく舌苔の量を減少させ、それに伴って口臭を減少させることが確認された.
