    PURPOSE: This study aimed to elucidate the relationship between diurnal masseter muscle activity and awareness of diurnal awake bruxism (d-AB) by conducting a comparative analysis of electromyographic (EMG) data from individuals with and without awareness of diurnal awake bruxism (d-AB), utilizing EMG data gathered from multiple subjects. METHODS: Unilateral masseter electromyography (EMG) recordings were performed during the daytime using an ultraminiature wearable EMG device. A total of 119 participants (59 with awareness of diurnal tooth clenching [d-TC] and 60 without awareness of d-TC) were included. Waveforms longer than 0.25 s with the two amplitude conditions, exceeding twice the baseline and >5% of maximum voluntary clenching, were extracted. In addition, the number of bursts and episodes (groups of bursts), burst duration, and burst peak amplitude were calculated for each participant. RESULTS: There were no significant differences in the EMG parameters between the groups with and without awareness of d-TC. Additionally, the frequency distribution of the number of EMG waveforms exhibited wide ranges and substantial overlap between the two groups. CONCLUSIONS: The variability in the number of bursts and episodes, burst peak amplitude, and burst duration among subjects suggests the need for an objective classification of d-AB severity based on EMG values. The absence of significant differences and large overlap in frequency distributions between the groups with and without awareness of d-TC indicate difficulty in predicting muscle activity solely based on awareness of d-AB.

    INTRODUCTION: Occlusal overload is considered to be one of the causes of late implant failure. However, it is unclear whether the magnitude of the patient's occlusal force is a risk factor for late implant failure. PURPOSE: This case-control study aimed to clarify the association between the cross-sectional area (CSA) of the masseter muscle and late implant failure. METHODS: This case-control study was limited to implant-supported fixed prostheses. We compared cases with at least one late implant failure (n = 25 patients) to controls (n = 82 patients) without implant failure. Patients were matched by age, sex, year of surgery, jaw and tooth type, and bone graft. Log-rank and Cox proportional hazard regression analyses were used to identify possible risk factors for late implant failure. RESULTS: The incidence of late implant failure was significantly associated with masseter muscle CSA ≥504.5 mm2 (hazard ratio: 4.43; 95% CI: 1.82-10.79; p < 0.01). CONCLUSION: Higher masseter muscle CSA increases the risk of late implant failure.

    OBJECTIVES: Failed implant removal using a high-frequency electrosurgical device (HFED) has been reported to be less invasive than other surgical techniques. We sought to clarify the mechanism of removal torque reduction in an implant by heating with HFED. MATERIALS AND METHODS: Sixty-eight Wistar rats received titanium implants on the maxillary bone 4 weeks after extraction of the first and second molars. The control group was sacrificed 6 weeks after implant installation. In the experimental group, the implant was heated by HFED for 10 s using three different power outputs, and samples were collected at 3, 7, and 14 days after heating. Removal torque measurement and histological analysis were performed in the control and experimental groups. Implant surfaces were observed using an electron-probe microanalyzer (EPMA). Data were analyzed using Mann-Whitney U test at a significance level of 5%. RESULTS: The removal torque could not be measured in the control group due to fracture of the implant. After heating, the removal torque was measurable without fracture and decreased significantly at 14 days as compared with that at 3 days (p < .05). Heating with "min" power output resulted in a significantly smaller blank lacunae area and fewer osteoclasts at 14 days after heating (p < .05). EPMA revealed bone matrix adherence to outer surface of heated implant. CONCLUSIONS: After heating, an enlarged area of blank lacunae around the implant and an increased number of osteoclasts into the bone marrow cavity were observed, which may have contributed to the reduction in removal torque.

    PURPOSE: The purpose of this study was to determine whether removing the superstructure of the implant bridge in cases of full-arch implant restorations for edentulous atrophic arches at the abutment level during professional mechanical plaque removal (PMPR) affects bacterial counts. METHODS: This crossover clinical trial included 20 patients who received screw-retained prostheses at the abutment level. Patients were randomly assigned to two groups and received PMPR with or without removal of the superstructure. After a three-month washout period, the type of treatment was reversed between the groups. Bacterial counts around the cylinder and abutment were measured and compared before and after PMPR. RESULTS: Bacterial numbers around the cylinder and abutment were significantly reduced after PMPR as compared with before PMPR regardless of whether the superstructure was removed (p <0.05). However the ratio of subjects with bacteria at 1.0 × 105 colony forming unit/ml (cfu/ml) or more after PMPR was significantly higher when the superstructure was not removed (p < 0.05). Among patients with bacterial counts of less than 10 × 105 cfu/ml, bacterial loads were reduced to less than 1.0 × 105 cfu/ml even when superstructures were not removed. Among patients with bacterial load of >10 × 105 cfu/ml, bacterial numbers were not reduced to <1.0 × 105 cfu/ml when PMPR was performed without removing the superstructure. CONCLUSIONS: Removal of the superstructure in cases of full-arch implant restorations for edentulous atrophic arches during PMPR reduces bacterial numbers around the implant bridge at the abutment level.

    <p><b>Purpose:</b> Previous clinical studies have indicated that the loss of retention between post and tooth is a major cause of failure in the restoration of endodontically treated teeth. Since bubble-like defects were found in the cement layer of the debonded post, we hypothesized that this cement defect decreases the adhesive strength. The purpose of this study was to evaluate if the technique used to insert resin cement influences the presence of bubbles and the push-out bond strength.</p><p><b>Methods:</b> The root canal of a transparent resin block and bovine tooth were parallel-wall drilled with a Peeso Reamer to achieve a post length of 10 mm. Prefabricated metal posts were cemented using self-adhesive resin cement, for nine combinations of three tools and three cement application sites. The resin block was scanned with a digital camera, then image analysis software was used to calculate the percentage of bubbles in the cement layer. Bovine teeth were sectioned into five 1-mm thick slices for the push-out bond strength test.</p><p><b>Results:</b> The bubbles were concentrated at the apical side for all methods. When we used the plastic needle to apply cement to the post and root canal, the bubble mixture rate was significantly lower than that of the other methods. Using the brush resulted in a significant decrease in the push-out bond strength at the apical side, whereas there was no significant difference in the push-out bond strength between the sites with other tools.</p><p><b>Conclusions:</b> It is considered that there is less adherence at the site of air bubbles, and therefore a decrease in the adhesive strength. We conclude that it is important to use a tool that reaches the deepest part of the prepared root canal to reduce the air bubble mixture rate and to obtain the highest adhesive strength.</p>

    Background: The free vascularized fibula flap presents many advantages such as sufficient length of the bony segment, good vascularization, better quality of the bone, and a long vascular pedicle, but it is also associated with some disadvantages with regard to prosthetic rehabilitation because of its limited height. Improvement in bone height is necessary for ideal dental implant treatment of reconstructed mandibles. Case presentation: For two squamous cell carcinoma patients, mandibular bone reconstruction was performed secondarily with the peroneal flap after tumor resection. Since the bone height was insufficient at the time of implant treatment, occlusion reconstruction by dental implant was performed after vertical distraction osteogenesis. Conclusions: Vertical distraction osteogenesis is a suitable treatment option for alveolar ridge deficiency resulting from fibula transplantation for mandibular reconstruction following tumor surgery.

    PURPOSE: This study analyzed risk factors for post-loading implant loss in cases of implant-supported prostheses applied to edentulous jaws of Japanese patients. METHODS: In total, 245 dental implant fixtures placed in 54 edentulous jaws of 46 patients performed at Niigata University Hospital were retrospectively analyzed. Kaplan-Meier curves were used to estimate the cumulative survival rate (SR) of implants, and multiple Cox regression analysis was used to identify predictive factors of implant loss. The following risk factors for implant failure were examined: age, sex, survival time, implant length, implant location, smoking habit, bone density, bone augmentation, opposing dentition, loading period, and type of final restoration. The Cochran-Mantel-Haenszel test was used to examine difference in survival curves of the extracted predictors. RESULTS: Sixteen implants failed during the observation period (SR=92.8 %). Multiple Cox regression analysis revealed that male sex [hazard ratio (HR)=16.1; p=0.007] and use of maxillary removable restorations (HR=12.7; p<0.000) were risk factors for implant failure. Other factors had no significant effect on implant failure. The SR of implants for males (SR=86.9%) was significantly lower than that for females (SR=99.1%). The SR of implants for maxillary removable restorations (SR=76.4%) was significantly lower than for maxillary fixed restorations (SR=99.1%) and mandibular fixed restorations (SR=97.8%). CONCLUSIONS: Maxillary implants with removable restorations and male sex were risk factors for implant failure among Japanese edentulous patients.

    Streptococcus pneumoniae is a leading cause of bacterial pneumonia. Our previous study suggested that S. pneumoniae autolysis-dependently releases intracellular pneumolysin, which subsequently leads to lung injury. In this study, we hypothesized that pneumococcal autolysis induces the leakage of additional intracellular molecules that could increase the pathogenicity of S. pneumoniae. Liquid chromatography tandem-mass spectrometry analysis identified that chaperone protein DnaK, elongation factor Tu (EF-Tu), and glyceraldehyde-3-phosphate dehydrogenase (GAPDH) were released with pneumococcal DNA by autolysis. We demonstrated that recombinant (r) DnaK, rEF-Tu, and rGAPDH induced significantly higher levels of interleukin-6 and tumor necrosis factor production in peritoneal macrophages and THP-1-derived macrophage-like cells via toll-like receptor 4. Furthermore, the DNA-binding activity of these proteins was confirmed by surface plasmon resonance assay. We demonstrated that pneumococcal DnaK, EF-Tu, and GAPDH induced the production of proinflammatory cytokines in macrophages, and might cause host tissue damage and affect the development of pneumococcal diseases.

    We developed an ethylene vinyl acetate (EVA) containing surface pre-reacted glass-ionomer (S-PRG) filler, as a new mouthguard material for preventing intraoral bacterial infection. We examined its physical properties, antimicrobial activity against a major cariogenic bacterium Streptococcus mutans and a periodontopathogen Porphyromonas gingivalis, and its cytotoxicity toward human gingival epithelial cells. S-PRG filler was added to EVA copolymer at 5, 10, 20, or 40 wt% and was processed into disc-shaped test specimens. Only minor differences between the Shore hardness and rebound resilience properties of EVA materials with and without the S-PRG filler were observed. The specimens with S-PRG filler showed bacteriostatic activity toward S. mutans and P. gingivalis and inhibited S. mutans biofilm formation. No cytotoxicity against the gingival epithelial cells was observed. Our findings show that EVA containing S-PRG filler has antimicrobial activity toward pathogenic oral bacteria and may be an effective material for maintaining the oral health of athletes.

    <B>Patient: </B> The patient was 40 years old woman with chief complaint by intermittent jaw lock with severe pain. CT and MRI examination showed disc displacement without reduction on both sides and a typical osteoarthrosis of head of mandible, but the mobility of condyles was good. Neurology examination did not show the disorder, but in electromyogram, left side masseter and temporal muscle showed involuntary muscle activity more than 3 times of MVC. It was diagnosed as jaw lock by the hemimasticatory spasm. A prosthetic therapy using the overlay removable partial denture succeeded and was able to control it for masticatory spasm well for 10 years.<br><B>Discussion:</B> It contributed to dramatic decrease of the masticatory spasm onset that occlusal vertical dimension of the overlay removable partial denture decided EMG examination in reference.<br><B>Conclusion:</B> The overlay removable partial denture contributed to control of masticatory spasm and recovery of a chewing function.

    It is known that small head movements accompany the movements of the jaw during mastication; however, it is unknown whether these movements occur rhythmically and synchronously. The objective of this study was to determine whether there exists a functional coupling between the head and mandibular movements. Four healthy male adults (mean age 25.5) with normal occlusion and without TMD history were selected as subjects. Using the Trimet system, we measured tridimensionally both the movement of the head and the mandible by tracking upper and lower incisal points, respectively, during tapping movements with different opening range and frequency, then analysed the vertical component of these movements. The upper incisal point moved in opposite direction to the mandible in all tapping strokes in all subjects, during opening the head moved in a cranial direction and during closing in a caudal direction; the incidence rate for this concomitant movement was 98%, implying that the head moves periodically and rhythmically, as the mandible does. The cycle time of these coincident movements showed a correlation coefficient of 0.94. Moreover, the vertical range of head movement was within 10% of the jaw's movement. From these results we concluded that, at least during teeth tapping, the head moves in rhythmical coordination with mandibular movement.

    It is well known that the range of motion during mouth opening changes with the food size. We have previously reported the relation of head movement to the mandibular movement ; the head moves in the extended direction during mouth opening. In this study, we analyzed the head movement simultaneously with the mouth opening during food intake. One male and two female adults (age range : 24-26yrs. ) without any craniomandibular dysfunction were selected as subjects. The test food consisted of an apple cut into the different square size (6 to 35mm). Then, we recorded the head and mandibular movements at two head positions (horizontal to the Frankfort and Camper plane) with the 6-degree-of-freedom measuring device without any restrictions of the subjects' head. The results were as follows : 1 ) The food space was observed during food intake. The size of this space at the same food size was kept constant even when the head position was changed. 2 ) The head movement was larger when the head position was horizontal to the Frankfort plane than to the Camper plane. This suggests that the head movement exists in order to acquire support for the mandible during mouth opening.

    It has been reported that duringmandibular tapping movements the head moves in functional coordination with the mandible in subjects with a normal occlusion and free of TMD symptoms. However, whether this finding can be applied to TMD patients remains unclear. The purpose of this study was to confirm the coordination of head and mandibular movements during tapping in TMD patients. Using the Trimet system, we measured tridimensionally both the movement of the head and the mandible by tracking upper and lower incisal points respectively during tapping movements, then analyzed chronometrically the vertical component of these movements. We found that the incidence of coordinated head and mandibular movements in TMD patients was lower than in normal subjects. Our findings confirmed the existence of coordination between the head and mandibular movements during tapping, and conclude that it is different for normal subjects and TMD patients. Our results raise the question on whether there is a relation between the presence and intensity of the TMD symptoms and the decrease in the incidence of the coordinated head and the mandible movements during tapping in TMD patients.

    当該年度は前年度から継続して、研究対象となる臼歯部にインプラント治療を予定し、インプラント埋入計画の段階での医科用CTまたは歯科用コーンビームCTの撮影済みであり、かつ、データ利用の同意を得られた対象者からCT画像データを収集した。このデータは「初期モデル」の骨梁三次元有限要素モデルのベースとなる。また、モデル荷重条件設定に必要となる、インプラント治療の進展に伴い、記録された上部構造装着後の咬合圧データを入手した。下顎臼歯部の単独インプラント症例において、得られた歯科用コーンビームCT画像データおよび咬合圧データをもとに、歯列模型を光学３Dスキャニングしたデータおよび別途撮影したインプラント体のマイクロCT画像と重ね合わせたインプラント咬合負荷解析用の骨梁三次元有限要素モデルの作成手法を設立した。
    また、当該年度も新型コロナウイルス感染症の影響に伴い歯科受診患者数が総合的に減るなかで、本研究の新たな対象者が当初想定していたより少なかったが、以前より研究対象者となっていた者の定期的に採得・記録された咬合検査データの経時的変化を分析し続けた。その臨床経過の追跡中の対象者全員において、インプラント周囲組織の状態は良好で保たれ、特に異常な辺縁骨吸収は生じず、インプラントの良好な維持・管理ができていたことから、それぞれの症例において測定された咬合圧によるインプラント周囲骨が受けた力学的な負荷は生理的範囲内であったと推定される。
    一方、全症例において異常な辺縁骨吸収は生じなかったことにより当該年度も「骨吸収モデル」の対象となるデータを得る状況は発生しなかった。

    We were able to construct a clinical flow from scanning the abutment construction cavity using an intraoral scanner, performing virtual wax-up on CAD, to fabricating a one-piece dowel core by cutting the block with a CAM cutting machine, and bonding it to the abutment tooth.
    The intraoral scanner was capable of scanning post cavities up to 10 mm deep. Both the fiber-reinforced resin block and the zirconia dowel core showed good adhesion to the root canal with resin cement, and the CAD control of the thickness of the cement layer was also useful.

    With the aim of establishing diagnostic criteria for diurnal awake bruxism (d-AB), we performed electromyography with an ultraminiature wearable electromyographic device in collaboration with multiple facilities. As a result, we were able to construct a database of daily daytime masseter muscle activity without restraint and various clinical findings in more than 150 subjects. The distribution of the number and magnitude of muscle activity was wide in both the d-AB conscious group and the d-AB non-conscious group, and the overlap between the two groups was also very large. It was shown that it was difficult to explain the amount and magnitude of daytime masticatory muscle activity only by classification of d-AB based on self-awareness.Therefore, based on the numerical distribution of the newly established database, a method of grading masticatory muscle activity during the day in several stages and using the grading as an index for d-AB evaluation was suggested.

    With patients’ informed consent, cone-beam CT data of unilateral free-end edentulous ridges, occlusal forces and occlusal contact registrations, obtained for implant treatment planning purposes, were combined with micro-CT data of implants and patient superstructures to create 3D finite element (FEM) models in which trabecular structure, implants, superstructures and occlusal contacts were simulated with high accuracy ensuring detailed stress analyses. To predict in the treatment planning phase the implant forces required for the FEM analyses, maximum occlusal forces measured with pressure-sensitive sheets on the remaining teeth before implant treatment and on implants after superstructure set were analyzed by linear regression analysis. A positive correlation was found between implant occlusal forces and occlusal forces on the remaining teeth, as well as on the posterior teeth of the same side with the edentulous ridge to be restored with implants.

    A purpose of this study was to determine the effect of treatment of temporomandibular joint mobilization performed for the decrease in temporomandibular joint excursion patients and the improvement mechanism of the symptom. The change after mobilization in the decrease in temporomandibular joint excursion patients was following three. 1) The change was not found in an electromyogram. 2) The range of motion of mandibular condyle increased. 3) The hardness of masseter muscle decreased. As for the increase of the range of motion of the mandibular condyle, it was thought with a thing by the mobile improvement in the joint capsule and effect by the easy extension of the embarrassment muscle, but was not able to confirm the change of the soft tissue in the joint capsule.
    Hardness of the embarrassment muscle pays attention to an improved point and it will be clear in future and wants to do procedure establishment and effect of treatment of temporomandibular joint mobilization.

    Development of diagnosis support system for mastication-swallowing function was studied by this Grant-in-Aid for Scientific Research (C) (19592231).
    For developing new method of the evaluation in mastication function, we investigated or the effects of prosthetic treatment by counting the number of chewing strokes until initial swallow of rice crackers. TMD signs and symptoms of 80-year old subjects were not so common. Therefore, there is no relation between mastication function and TMD in elderly subjects.
    Since a Piezoelectric Pulse Transducer (PPT) detects peripheral pulse-waves, the PPT has already been used to simply record the laryngeal upward-downward movements. Based on this fact, we developed the prototype to evaluate the swallowing function. The telemedicine in the dentistry was also tried.

    顎関節症への罹患しやすさ及び病態の進行度に関する因子の解明の手がかりとして,顎関節のあそびを定量的に評価する事が,顎関節症の発症原因を究明する上で急務である.
    本実験は,健常者群および顎関節症発症患者群の顎関節のあそびを定量的に評価し,顎関節のあそびと顎関節症との因果関係について検討することを目的とした.
    実験内容を十分に説明し,同意の得られた顎関節症患者および年齢と性別をマッチングさせた健常者について実験を行った.
    被験者にブラキシズムを想定した側方位における臼歯部クレンチングを行わせた.その際の咬合力が各被験者間で一定となるように咬合力計により規定した.この側方位クレンチングにより,作業側下顎頭へ負荷が加えられる.その際の下顎頭の動きを6自由度顎運動測定装置にて測定した.被験者個々の下顎頭の運動量に関して定量的に評価し,以下の結果を得た.
    側方位クレンチング時の下顎頭の運動量は,健常者より顎関節症患者の方が大きい平均値を示した.すなわち顎関節症患者の顎関節はあそびが大きい傾向が認められたが,今回の被験者数では両者に有意差は認めなかった.そこで顎関節症の病態別に比較したところ,関節円板転位患者では有意差は認められなかったが,病態の進行度が高いとされる変形性顎関節症患者では,顎関節のあそびが有意に大きかった.
    本実験の結果のみでは,顎関節のあそびが,顎関節症の発症原因を示すものなのか,病態の進行に伴って増大するものなのかは未だ不明である.顎関節内障患者の顎関節のあそびは大きいという本実験の仮説は,変形性顎関節症患者に限定されるが,証明されたと考える.

    本実験は,健常者群および顎関節症発症患者群の顎関節のゆるみを定量的に評価し,顎関節のゆるみと顎関節症との因果関係について検討することを目的とした.
    実験内容を十分に説明し,同意の得られた健常者および顎関節症患者の被験者について測定を行った.
    被験者にブラキシズムを想定した側方位における臼歯部クレンチングを行わせた.その際の咬合力が各被験者間で一定となるように咬合力計により規定した.この側方位クレンチングにより,作業側下顎頭へ負荷が加えられる.その際の下顎頭運動を6自由度顎運動測定装置にて測定した.被験者個々の下顎頭の運動量に関して定量的に評価し,以下の結果を得た.
    側方位クレンチング時の下顎頭の運動量は,健常者より顎関節症患者の方が大きい値を示した.すなわち顎関節症患者の顎関節はゆるい傾向が認められた.顎関節症の病態別では,関節円板転位患者は正常者よりあそびが大きい傾向はあったが,有意差は認められなかった.病態の進行度が高いとされる変形性顎関節症患者では,顎関節のあそびが有意に大きかった.この結果のみでは,顎関節のあそびが,顎関節症の発症原因を示すものなのか,結果を示すものなのかは未だ不明であるものの,顎関節のあそびを定量的に評価することの意義が明らかになったと考えられる.

    We have previously demonstrated the existence of a functional-coupling between the head and the mandible. The objective of the present study was to reveal mechanisms of this functional coupling and effects of occlusion on this coupling.
    We measured tridimensionally both the movement of the head and mandible and the electromyographic activities in the masticatory and head-neck muscles in healthy subjects and patients of temporomandibular disorders.
    In healthy subjects, heads moved cyclically in the same cycle time as that of both masticatory and tooth-tapping jaw movements. The head underwent a rotation-like sagittal movement whose rotation center located above the cranial base point. The masticatory muscles and the neck muscle (the stemocleidmastoid muscle) showed rhythmical activities synchronized with jaw movements. The neck muscle activity showed a linear correlation with masticatory force. Additionally, it was seem that the neck muscle began the activity before the occlusal phase of jaw movements. It is demonstrated that the neck muscle contribute to the synchronized head movement in the anticipatory manner. However we could not confirm that this coordinated head movement would influenced on whole body flicking.
    In patients of temporomandibular disorders, the degree of synchronization between head and jaw movement was approximately three-quarters of that with healthy subjects during mastication and tooth-tapping. This variable was influenced by existence of TMD symptoms, especially limited jaw opening, which suggests mechanical feature of the coupling between head and jaw movement. Contribution of occlusal features to head movement has not been confirmed in our study.

    顎関節症は多因子性の疾患であると考えられている.顎関節症の病因因子の中で,素因と呼ばれる個体ごとに異なる顎関節の抵抗力あるいは耐久性に関しては,未だ明らかではない.そこで顎関節症への罹患しやすさに関する因子の解明の手がかりとして,顎関節のゆるみを定量的に評価する事が,顎関節内障の発症原因を究明する上で急務であると考えた.
    本実験は,健常者群およぴ顎関節内障発症患者群の顎関節のゆるみを定量的に評価し,顎関節のゆるみと顎関節内障との因果関係について検討することを目的とした.
    本年度は,健常者群およぴ顎関節内障患者群の被験者について測定を行った.
    実験は,被験者にブラキシズムを想定した側方位クレンチングを行わせた.その際の咬合力が各被験者間で一定となるように圧力センサーにより規定した.この側方位クレンチングにより,作業側顆頭へ持続的強い力の負荷が加えられる.その際の顆頭運動を6自由度顎運動測定装置トライメットにて測定した.被験者個々の顆頭の運動方向と運動量に関して定量的に評価し,以下の結果を得た.
    同一咬合力による側方位クレンチング時の顆頭の運動量は,健常者群より顎関節症患者群の方が大きい値を示した.すなわち顎関節内障患者群の顎関節はゆるい傾向が認められた.この結果は顎関節内障の原因を示すものなのか結果をしめすものなのかは不明であるものの,現状における個体ごとに異なる顎関節の素因を定量的に評価する上で重要なものであると考えられる.
