    Because a relationship has been reported between masticatory behavior, obesity, and postprandial blood glucose, it is recommended to chew well and take a longer time to eat. The purpose of this study was to examine the possibility of changing masticatory behavior using a small ear-hung wearable chewing counter, which can monitor masticatory behavior without disturbing daily meals. In total, 235 healthy volunteers participated in a 4-wk randomized controlled trial and were divided into 3 groups. All participants were instructed about the importance of mastication at the first visit. During the intervention, group B used the chewing counter without an algorithm during each meal (notification of the number of chews after meal), and group C used the chewing counter with a masticatory behavior change algorithm (setting a target value and displaying the number of chews in real time). Group A was set as the control group. The number of chews and the meal time when consuming 1 rice ball (100 g) were measured before and after the intervention using the chewing counter, and the rate of change in these values was evaluated. Participants also provided a subjective evaluation of their changes in masticatory behavior. The number of chews and the meal time of 1 rice ball increased significantly in groups B and C compared with before the intervention, and the rate of change was significantly higher in group C than in group A and group B. In addition, the subjective evaluation of the change in the number of chews was highest in group C. Self-monitoring of masticatory behavior by providing a target value and the degree of achievement for the number of chews using a wearable chewing counter with a behavioral change algorithm could promote effective change in masticatory behavior and lead to an increased number of chews.     Abstract

    Objective

    This study aimed to verify the compatibility of masticatory performance tests using different two types of gummy jellies.

    Background data discussing the present status of the field

    Compatibility between the glucose concentration measuring method (GC) and 10‐steps visual scoring method (VS), which are masticatory performance tests for diagnosing decreased masticatory function (DMF) in oral hypofunction and the use of different types of gummy jelly, has yet to be confirmed.

    Materials and methods

    The participants consisted of 134 Japanese older people (51 men and 83 women). GC, VS and the increased surface area measuring method (ISA), as an alternative analysis method for VS, were conducted to evaluate masticatory performance. Cohen's Kappa coefficient was used to verify the consistency for detecting DMF between GC and VS. Pearson's correlation coefficient was used to analyse the relationship between GC and ISA. Each masticatory performance was compared among three different groups for the number of residual teeth. Cut‐off values of VS and ISA for DMF that were consistent with GC were identified.

    Results

    Glucose concentration measuring method and VS showed an excellent consistency (Kappa coefficient = 0.86). There was a high positive correlation between GC and ISA (r = .70). However, the change in masticatory performance according to the number of teeth was different between GC and VS/ISA. The cut‐off values of VS and ISA were a Score of 2 and 1687 mm<sup>2</sup>, respectively.

    Conclusion

    Glucose concentration measuring method and VS/ISA remained fairly consistent for detecting DMF, which might be useful information to interconnect the various studies on masticatory performance.

    <B>Purpose:</B> This study aimed to define the relationship between satisfaction of wearing denture and three factors ("easy to chew", "retention in chewing", "absence of pain"), as well as the missing teeth pattern with Miyachi classification, the chewing ability.<br><B>Methods:</B> A total of 256 subjects (born in 1927) living in Niigata city, Japan who had answered a questionnaire survey in 2007 on the relationship between oral health and health status were selected for this study. Satisfaction with dentures and three factors ("easy to chew", "retention in chewing", and "absence of pain") were evaluated with the VAS score. This study also investigated grouping the pattern of missing teeth based on the Miyachi classification. In addition, a questionnaire was conducted to investigate whether the elderly people could chew around 15 different food items or not.<br><B>Result:</B> It was clearly shown that satisfaction with dentures was correlated with the three points of chewing easily, stability, and pain-free among elderly people. However, none of the three points showed a far stronger correlation than the others. The results also showed that there was no relation between the missing teeth pattern classified with the Miyachi classification and satisfaction with dentures. The results of the questionnaire revealed that the number of food items taken by the elderly people was significantly lower in the group not satisfied with dentures than the other groups (Excellent, good, poorly satisfaction group). <br><B>Conclusion:</B> It was confirmed that chewing easily, stability and pain-free enhance satisfaction with dentures. However, satisfaction with dentures was not ascribed to the pattern of missing teeth; accordingly, we consider that we can keep elderly people satisfied by fabricating good dentures and performing appropriate adjustments regardless of difficult patterns of missing teeth.

    【方法】健常成人男性5名を被介助者とし、介助者は同一の1名が行った。被介助者は背板のないイスに着座し、介助者は被介助者の右側に座る。介助者は、直径15mm、高さ15mmの円柱形のリンゴ片を刺したフォークを手に持って、被介助者にリンゴを食べさせた。その際に食物を近づける方向は、被介助者の口裂に対し下方20度とし、フォークを向けるスピードは概ね高、中、低速とランダムに変化させた。また、被介助者の前方に設置したテーブル上にフォークに刺さったリンゴを置き、被介助者自身でフォークを把持して食べるタスクを行った。この食物取り込み動作において、被験者の頭部、体幹、フォークに標点を取り付け、三次元動作解析装置VICON(Vicon Motion Systems Ltd. UK)を用いてその動きを記録した。得られたデータより、オトガイ等の動きを解析し、フォークの速度との関連を分析した。また、注視点計測装置 Dekablis (ERGONEERS, 独・米)を用いて、被介助者の食物取り込み時の視線の位置を検索した。
    【結果と考察】自力摂取の場合、すべての被験者は口裂から200mm前後に食物があるときに開口を開始していたが、介助を受ける場合にはより遠くに食物があるときに開口を開始し、その距離にはばらつきがあった。フォークの速度と開口開始時のフォークの位置には一定の傾向はみられなかった。開口開始後のフォークのスピードは、遠くで開口した場合には速く、近くで開口する場合には遅くなる傾向があり、食事介助においては被介助者と介助者の挙動が相互に影響しあう可能性も推測された。

    The purpose of this study was to clarify the effects of meal assistance methods on the assisted person's posture and oral movements during food intake and mastication, and to propose desirable assistance methods for better eating.
    Recordings and analysis of the caregivers' head-torso and oral movements during meal assistance suggested that the direction in which food is directed toward the caregiver affects the caregiver's oral intake movements, and that the least burden is placed on the caregiver when the food is directed from the front downward.

    We analyzed the feeding movements of oral cavity using a motion capture, when subjects chewed food with various kinds of posture and when a meal was provided from various kinds of directions during assisted feeding. It was suggested that the posture of the body affects the chewing motion, and that the head posture and then the food-intake movement of mouth come under an influence of the direction of feeding assistance. We note that attention on food-intake and mastication is required during management of posture and an assistance method during feeding.

    Most home care patients and dentists desire of a simple technique for compete denture construction that may reduce the number of home-visit. However, its efficiency has not been established.
    Biofunctional prosthetic system (BPS) fundamentally requires a four-appointment procedures in order to provide high quality dentures for patients. We have modified the system to accommodate the delivery for home care patients and run a pilot test through our Dental hospital outpatient department. Result show that there was no significant difference between masticatory efficiency of patients who received treatment through the conventional and modified technique. This may suggest that the modified technique may be comparable with the conventional procedure with regards to the patient’s functional recovery.

    Jaw opening movement during food intake was investigated in elderly people.
    It was characterized by performing a coarse opening movement. With the loss of occlusal supports and wearing removable dentures, function of adjusting the opening amount according to the size of the food was drying. As with the movement of whole body, it was considered that accommodation ability is lost with age even in the oral area. Also by tooth loss and wearing dentures, it is similar to happen further.

    The purpose of this study was to develop a new educational support system for clinical training in dentistry using the Ubiquitous computing which could gain necessary information on site. Before treatment, students could reconfirm clinical procedures and usage of instruments or materials on chair-side by getting close the items attached RFID tag to the handheld reader, and then they treated with this information as reference. After treatment students keyed in effectiveness of information, clinical questions faced during treatment, and necessary additional information. The instructor estimated input information from a student and added modified information to reflect on clinical training for other students. This educational support system using the Ubiquitous computing may be effective for clinical training and reduce instructor's burden.

    Development of diagnosis support system for mastication-swallowing function was studied by this Grant-in-Aid for Scientific Research (C) (19592231).
    For developing new method of the evaluation in mastication function, we investigated or the effects of prosthetic treatment by counting the number of chewing strokes until initial swallow of rice crackers. TMD signs and symptoms of 80-year old subjects were not so common. Therefore, there is no relation between mastication function and TMD in elderly subjects.
    Since a Piezoelectric Pulse Transducer (PPT) detects peripheral pulse-waves, the PPT has already been used to simply record the laryngeal upward-downward movements. Based on this fact, we developed the prototype to evaluate the swallowing function. The telemedicine in the dentistry was also tried.

    【研究目的】
    捕食時の開口運動における頭頸-下顎連関が、どのような要素によってコントロールされているのかを明らかにする。
    【研究実績】
    被験者は、下顎運動が回転要素主体と言われている骨格性下顎前突を呈する有歯顎ボランティア6名とし、あらかじめ実験の概要について十分に説明し、実験への協力に対する同意を得た。被験者を背もたれのない椅子に着座閉眼させ、竹串に刺した種々の大きさのリンゴを手で把持させた後、開眼して食べるように指示した。6自由度顎運動測定装置TRIMETを用いて、食物摂取時の下顎運動と頭部動態を測定した。被験食品として、直径9,12,15,18,21,24mm、厚さ5mm円盤形の食物(リンゴ)を用い、各サイズにおいて5回ずつの試行を行った。種々の大きさの開口における頭部および下顎の動態を分析し、開口運動と下顎運動要素、及び頭部運動の関連性を調べた。
    骨格性下顎前突症例においても、正常者と同様に開口量が増加すると頭部運動量が増加した。しかし、パターンは一様でなく、正常者と同様に小さな頭部運動を示す群(A群)と正常者に比べ頭部運動量が大きい値を示す群(B群)が存在した。A群では下顎頭移動量も正常者と同様大きい値を示した。一方B群では、下顎頭移動量が正常者に比べ小さい傾向を示した。下顎頭の回転量に大きな差はなかった。
    以上のことから、開口運動においては、下顎運動要素のひとつ下顎頭移動量は、下顎運動だけでなく、頭部運動にも大きな関連を持つことが明らかとなった。このことは、開口運動が下顎運動と頭頸部運動の連合した運動であることを裏付けている。

    We have previously demonstrated the existence of a functional-coupling between the head and the mandible. The objective of the present study was to reveal mechanisms of this functional coupling and effects of occlusion on this coupling.
    We measured tridimensionally both the movement of the head and mandible and the electromyographic activities in the masticatory and head-neck muscles in healthy subjects and patients of temporomandibular disorders.
    In healthy subjects, heads moved cyclically in the same cycle time as that of both masticatory and tooth-tapping jaw movements. The head underwent a rotation-like sagittal movement whose rotation center located above the cranial base point. The masticatory muscles and the neck muscle (the stemocleidmastoid muscle) showed rhythmical activities synchronized with jaw movements. The neck muscle activity showed a linear correlation with masticatory force. Additionally, it was seem that the neck muscle began the activity before the occlusal phase of jaw movements. It is demonstrated that the neck muscle contribute to the synchronized head movement in the anticipatory manner. However we could not confirm that this coordinated head movement would influenced on whole body flicking.
    In patients of temporomandibular disorders, the degree of synchronization between head and jaw movement was approximately three-quarters of that with healthy subjects during mastication and tooth-tapping. This variable was influenced by existence of TMD symptoms, especially limited jaw opening, which suggests mechanical feature of the coupling between head and jaw movement. Contribution of occlusal features to head movement has not been confirmed in our study.

    [研究目的]
    本研究は、頭頸部の運動と顎機能の相互関係を明らかにするために、顎機能の一つ、咀嚼時における頭頸部の運動に焦点を当て、顎と頭頸部の協調運動の存在とその制御機構について調べたものである。
    [研究実績]
    実験にはウサギを用いた。麻酔下頭部固定状態で、大脳皮質咀嚼野に連続電気刺激を与えて咀嚼様運動を誘発し、下顎運動と頸筋および咀嚼筋筋活動の記録を行った。
    後屈筋である頭半棘筋は咀嚼様運動に伴って持続的な活動を示し、下顎運動に同期する活動は見られなかった。木片を咬合した場合、持続性の活動は増加したが、下顎運動に同期するリズム性活動は見られなかった。このことから、平成12年度に本研究で行った慢性動物実験において、後屈筋の活動が開口時に(周期的に)減少したという現象は、頭位の変化によって二次的に姿勢反射として誘発されたものであることが推測された。
    前屈筋である胸骨乳突筋は、空口咀嚼の場合は周期性の活動はほとんど見られなかったが、木片を咬合した場合には、閉口相にほぼ一致する活動を示した。これは平成12年度の慢性実験でも同様に観察された現象である。すなわち、咬合に起因する感覚情報が、周期性頸筋活動、ひいては周期性頭部運動の発生要因であることを示している。さらに、この前屈筋周期性活動のonsetが咬合相にやや先行する、というケースが半数で見られたことから、胸骨乳突筋の予測的制御機構の存在も推測された。
