    This study investigated the effects of an online interprofessional education (IPE) program on nursing students at a university in Japan. It conducted a comparative analysis between traditional face-to-face and online interventions. All students who enrolled in the “Team Medical Practice” course in both 2019 and 2020 were invited to participate. After team-based learning with different healthcare professions, we quantitatively analyzed pre- and post-intervention outcomes using two measures: the Readiness for Interprofessional Learning Scale (RIPLS) and the IPE Questionnaire TSUKUBA model (IPET). We compared the results of 153 nursing students. The RIPLS results demonstrate significant pre- and post-intervention score differences in the teamwork and collaboration subscale for the online group only. The post-IPE score analysis revealed that scores were significantly higher in the online group in all subscales: Teamwork and collaboration, Opportunities for IPE, and Uniqueness of profession. Based on the IPET results, there were no significant differences in pre- and post-intervention scores. However, the online group showed a significant increase in post-intervention scores in participation in group work, thoughts about the team in health and welfare, and thoughts about interprofessional collaboration. These findings indicate that the online IPE program improved nursing students’ readiness for interprofessional learning, demonstrating practical efficacy.

    BACKGROUND: Zinc (Zn) deficiency, malnutrition, sarcopenia, and frailty are prevalent among older adults and are prominent factors contributing to disability and mortality. OBJECTIVE: This scoping review was conducted to aid understanding of the extent and types of research addressing the role of Zn in nutritional status, sarcopenia, and frailty, among older individuals. METHOD: A systematic search was performed in August 2022 of 3 electronic databases (PubMed, Web of Science, and ProQuest) using predefined search terms. The review was conducted referring to the Arksey and O'Malley framework and PRISMA-ScR. RESULTS: The search retrieved 16 018 records, and a total of 49 studies were included in this review after the screening. Of those, 30 were based on dietary Zn intake, 18 on tissue Zn levels, and 1 on both. Most studies were based on cross-sectional data from community-dwelling older adults. Studies addressing the associations between Zn status and individual anthropometric and sarcopenia-related variables reported inconsistent results. However, most studies reported inverse associations between malnutrition, frailty, and Zn status. CONCLUSION: There was more consistent evidence of the relationship of Zn status with malnutrition, sarcopenia, and frailty rather than with individual nutritional parameters. Validated screening and assessment tools and criteria and prospective studies are required to elucidate the relationship of Zn with sarcopenia and frailty in the older population.

  • Mental Health of Mothers of Preschoolers Amid the COVID-19 Pandemic in Japan: A Cross-sectional Study Reviewed

    Background:

    The COVID-19 pandemic and the government’s emergency declarations in Japan may have influenced people’s mental health. In particular, among women, there are concerns about the occurrence of neuroses, such as depression and anxiety.

    Objective:

    This study aimed to identify the factors related to mental distress among women in Japan who were raising children amid the COVID-19 pandemic.

    Methods:

    An online survey was conducted in 2020 among 730 Japanese women raising preschoolers. The survey included questions about child-rearing, anxiety, and the 12-item General Health Questionnaire (GHQ-12). The average age of the respondents was 34.4 years (21–52 years), and 31.5% of the respondents were living in “Prefectures under Specific Cautions” areas.

    Results:

    The prevalence of clinically significant levels of mental distress according to GHQ-12 scores was 41.5%. The highest predictor of mental distress was “Maternal anxiety about child-rearing” (Odds Ratio = 3.27, 95% Confidence Interval: 2.41–4.45). Other factors that affected their mental condition included concerns about children’s health and avoiding visiting hospitals for treatment.

    Conclusion:

    Mothers raising preschoolers experienced increased mental distress because of pandemic-related closures and heightened maternal anxiety about child-rearing. Thus, women who are raising preschoolers and confined to their homes need to be provided mental healthcare.

    本調査は、現行カリキュラムで教育を受けた3年間の卒業年次学生の看護学専攻のカリキュラム評価の推移と本学における物的・人的学習環境の評価を明らかにすることを目的とした。2015、2016、2017年度に卒業した4年生計226人を対象に自記式質問紙調査を実施し、回答が得られた197人(87.2%)を分析対象とした。調査内容は、学習目標の達成状況やカリキュラム、学習支援状況に関する学生の評価である。カリキュラム全体の充実度および学習目標の達成度は、「充実していた」、「できた」と回答した割合が、3ヵ年ともに8割を超え、学習支援については、7割以上が「充実していた」と回答した。自由記述からは、科目の開講時期調整の要望や、学習設備面の改善を期待する回答が多かった。本調査からは、現行カリキュラムの教育内容、教育体制については、概ね高い評価が示された。今後は調査結果に基づき、次期カリキュラム改正に向けて、より充実した看護基礎教育と学習支援体制が必要であることが示唆された。(著者抄録)

    目的:本研究は、病棟保育士の医療に関する知識および病児対応の学習ニーズを明らかにすることを目的とした。方法:無記名式の質問紙を用いた横断調査を実施した。対象:全国の小児専門病院および小児科を標榜する定床300床以上の病院に勤務する経験年数3年以上の病棟保育士に調査を行った。結果:有効回答者78人(91.8%)、病棟保育士経験8.71(SD:6.8)年であった。全員が「医療知識は必要」と認識しており、ほぼ全員が学びたいことは「病院内での個別遊び」と回答した。医療知識の習得への取り組み経験がある病棟保育士43人は、経験がない病棟保育士27人よりも、勤務形態が「常勤」(p=.003)、保育士以外の資格「あり」(p=.036)、院外研修参加経験「あり」(p=.003)、「看護師との意見の相違がある」(p=.019)、「病児の保育に不安がある」(p=.019)の5項目が有意に高率であった。今後、病棟保育士への現任教育として病児に適した遊びのガイドライン、および各病院での教育プログラムが必要であると示唆された。(著者抄録)

    看護師は、病気について子どもや保護者に説明することをどの程度重視しているのだろうか。また、子どもが病気についてどの程度の理解力をもつと考えているのだろうか。本研究では、これらの考えが看護経験によって異なるかどうか、質問紙調査により検討した。調査対象者は、看護経験が5年以上あり主に小児看護に従事する看護師110人(小児群)と、主に成人看護に従事する看護師85人(成人群)である。子どもへの説明を重視する度合いについては群差が認められず、どちらの群でも子どもより保護者への説明を重視していた。病気の原因・経過・種類等に関する子どもの理解については、小児群の方が幼児期後半および児童期前半の子どもの能力をより高く評価していた。以上より、小児看護を専門とする看護師は、実践の中で病気理解に関する子どもの有能性を認めるようになることが示唆された。(著者抄録)

    The principal aim of the present study was to clarify the environment of the pediatric inpatients and outpatients in Japan in comparison with previous studies regarding whether consideration and adjustments are being carried out for children.<br />
    A national cross-sectional survey was administered by mail to hospitals with more than 300 beds that advertise a pediatric department and pediatric hospitals (1,069 hospitals), and responses were obtained from 256 hospitals (response rate: 23.9%).<br />
    The results revealed that physical adjustments were performed in approximately 50% of hospital pediatric outpatient departments. In hospital wards also, the rate of placement of a playroom or educational facilities and the rate of placement of nursery teachers were higher than in previous studies. Moreover, the results of the present study show that new pediatric medical professionals are being deployed in hospitals. These findings suggest great improvements in the current pediatric medical environment in Japan in comparison with the past.     本研究の目的は、重度障がい児を育てる母親の、子どもの反応に関する内面的支え体験を明らかにすることである。母親12名を対象に半構造的面接方法によりデータを収集し質的帰納に分析を行った。結果、【子どもの「元気な状態」が維持できた】【わが子らしさや子どもの好きなことや楽しみなどがわかった】【期待・希望をもって育児に向かう】【子どもなりの成長をとらえ子どもに合わせてかかわることができた】【子どもが家族や周囲の人から大切にされた】【子どもとのやり取りを通して母親として思いを深めた】の6つの体験が明らかになった。育児の負担軽減と同時に、母親の育児の内面的支えを理解することが、母親自身のレジリエンスを支援するうえで重要であることが示唆された。子どもの身体状態の安定、成長を支える機会の提供と環境調整、さらなるインテグレーションの促進が重要と考えられた。(著者抄録)

    本研究の目的は、就学前の子どもをもつ保護者の、メディア・リテラシーの基盤となる基礎的情報能力の実態とその関連要因を明らかにすることである。研究対象の保護者168人の基礎的情報能力の平均得点と標準偏差は、112.05±17.26点であり、大学生とほぼ同程度の情報活用の能力を有していた。特に基礎的情報能力総得点と情報活用の自己評価が高い保護者は、子育ての自信度も高いことが明らかとなった。子どもへの情報機器の使用、早期からの情報教育の賛否については、基礎的情報能力得点の下位尺度の「判断力」が高い保護者は、子どもに情報機器を扱わせることに賛成であり、情報の「収集力」が高い保護者は、より早期からの情報教育に賛成していた。(著者抄録)

    看護師が重症心身障害児の感情を捉える視点を明らかにすることを目的に、先ず、研究者が、X県内の重症児病棟に入院する特定の児1名(10代後半。脳性麻痺)と看護師の日常のかかわりやケアを5日間参加観察するとともに、同病棟に勤務する看護師5名へ適宜、ケアの意図などについてインタビューを行った。また、同病棟の看護師で、重症児の感情を捉えた経験があり、日常的に児のケアを行っている看護師12名(上記の5名を含む)を対象に、自由記述による質問紙調査を行った。これらの内容分析の結果、看護師が児の感情を捉える視点として、【表情、声、動き】【呼吸循環、排便、姿勢、睡眠など日常生活の維持】【環境】【苦痛や不快の有無】【快の有無】【発達段階を踏まえたその人らしい生活】の6カテゴリーが抽出された。

    本研究の目的は、小児臨床写真を観察している時の視線運動と、その観察意図から、卒業年次の看護学生の危険予知の実態を明らかにすることである。大学4年生の看護学生11名を対象に、小児臨床写真を1分間観察させた後、その視線運動の解析ならびに面接調査から分析を行った。写真は危険を予知できる8領域を区分し、その領域間の注視回数、注視時間を比較した。その結果、以下の点が明らかとなった。対象者は、画面から危険を探ろうとする視線の動きと探索的な観察法を認めた。危険予知の内容では、転倒・転落は多くの対象者が指摘したが、輸液の間違いや薬剤無投与の危険について発言した者はいなかった。(著者抄録)

    A病院血管撮影室では、局所麻酔下で脳血管内治療を受ける患者に対して2008年からカテーテル前訪問(以下;カテ前訪問)を実施している。今回、脳血管内治療を受けた患者の心理プロセスを明らかにする目的で、当該患者6名に半構成的面接を行い、「カテ前訪問が心の準備にどの程度役立ったか」「治療前・中・後にどのようなことを考えていたか」などについて聴取し、修正版グラウンデッド・セオリー・アプローチ法でカテゴリー化した。結果、患者の心理プロセスとして【治療を決心するまでの葛藤】【個別性のあるカテ前訪問】【治療に伴う合併症の恐怖との戦い】【恐怖からの解放と未来への転換】というカテゴリーが抽出され、また、プロセスの全段階を通して存在する『生と死の狭間にいる危機的な状況』というコアカテゴリーが抽出された。

    一般病院または総合病院に勤務する診療放射線技師の子どもへのプレパレーションに対する理解の有無と子どもへの放射線検査や治療に対して、どのような認識で医療にあたっているのか明らかにした。総合病院に勤務し、5年以上臨床経験のある診療放射線技師5名を対象とした。インタビューガイドを用いた半構造化面接を実施した。データ分析はKJ法に基づいて質的に分析した。小児に対するプレパレーションの用語と意味内容については5名全員が「知らなかった」と述べていた。診療放射線技師は、子どもを意思疎通が難しく、動いてしまうとの認識により、「撮影や治療が困難な存在」と捉えていることがわかった。診療放射線技師は、検査・治療時の説明や同意、固定については、子どもがもつ権利の遵守よりも、専門職としての役割を実施していたと考えられた。また、検査・治療の事前説明の役割は看護に対して期待をしていることが明らかになった。

    The aim of the present study was to assess the relationship in children between dental fear and the noise of the dental drill. The subjects consisted of 104 patients (52 males and 52 females) at the Pediatric Dental Clinic of Niigata University Medical and Dental Hospital. They ranged from 4 to 15years of age (The mean age being 8.5). The patients were allocated into two groups based on their age: The low-age group (from 4 to 8 years of age) and the high-age group (from 9 to 15 years of age). The patients themselves evaluated their impressions of 3 sounds: The control sound (familiar music), thunderstorms and the noise of the dental drill, by using the Face Pain Rating Scale (FS). The CFSS-DS questionnaires were then used to assess their dental fear.<BR>The results were as follows:<BR>1. In both the low-age and the high-age groups the mean scores of FS with respect to the sound of thunderstorms and the noise of the dental drill were higher than those of the control sound. It was suggested that the noise of the dental drill was squeakier than control music from the child point of view.<BR>2. The results from factor analysis with varimax rotation indicated that the noise of the dental drill in the low-age group was related to the sound of thunderstorms. On the other hand, in the high-age group, the noise showed a positive relationship to the CFSS-DS score as the amount of dental fear.<BR>3. In the low-age group, dental experience tended to raise the score of the impression of pain when hearing the noise of the dental drill. The CFSS-DS score, however, showed a negative relation to the age of the patients. These findings suggest that in the low-age group, older patients have a lower fear toward the dentist and dental treatment, although they have feelings of pain with respect to the noise of the dental drill.<BR>4. The present investigation shows that female patients in childhood have higher sensitivity to the noise of the dental drill.

    本研究の目的は、新潟大学医学部保健学科看護学専攻において平成24年度から実施している改訂カリキュラムの教育評価である。4年次生80人を対象に自記式質問紙調査を実施し、回答が得られた59人(73.8%)を分析対象とした。調査内容は、学習目標の達成状況やカリキュラム、保健師および助産師教育課程の科目に関する学生の評価である。学習目標の到達度はほとんどの項目で70%以上が「できた」と回答し、カリキュラム全体の充実度も高い評価であった。また、保健師および助産師の実践内容に関する学習状況も全項目で80%以上が「学習できた」と回答した。一方で、看護実践できる能力の育成や、教養科目と専門科目のバランス、セメスター毎の学習量の配分は他の項目に比べ評価が低かった。よりよいカリキュラムの運用と看護学教育の一層の充実のため、限られた時間数の中で、効果的な教育方法とカリキュラムの進度について検討していく必要がある。(著者抄録)

    本研究の目的は、心理的な傷つき体験を経て成人へと成長した小児がん経験者のPosttraumatic growth（以下、PTG）の構造に着眼し、小児がん経験者に感受性の高いPTG尺度を開発することである。2021年度は、前年度に続き、若年成人期世代にある小児がん経験者の語りを質的に分析し、PTGの構成概念抽出に取り組んだ。また、既存の日本語版外傷後の成長尺度（Taku et al.,2007）による小児がん経験者のPTG得点、人口統計学的指標、サバイバー因子を用いて、高いPTGへの影響要因を分析した。そして、研究参加者の質的データから抽出した構成概念・PTGへの影響要因と、国内外の研究成果を照合し、考察を進めた。
    先行研究では、ピアサポートをはじめとするソーシャルサポートがPTGを促進すると複数報告されている。このほか、自己開示のプロセスでトラウマによる負の影響が軽減されるとの見方もある。これらの知見は、本研究の分析結果と類似していた。
    本尺度の開発では、小児がん経験者による過去の傷つき体験への認識や、小児がんに関わる危機を乗り越えた自信にあたる質問項目が重要となる。これまでの研究経過をふまえ、小児がん経験者の自己開示の経験に着眼し、ソーシャルサポートをはじめとする他者との関係性を詳細に分析した。今後は、これを生かして、小児がん経験者のPTGを表す尺度項目を抽出していく計画である。

    2021年度は、てんかんを患い抗てんかん薬の服薬継続が必要な子どもの保護者への研究を行う準備期間とした。
    本研究の目的は、生涯にわたる適切な服薬継続を支援するための教育支援ツールの開発である。そのためには、服薬を継続するために保護者がどのように服薬行動の意味を捉えているのかを明らかにする必要がある。今までは、一般的な子育て世代を対象とし、データを収集・分析してきたが、一時的な服薬であっても、服薬が継続できるような生活スタイルに合わせた服薬方法の提案や説明が重要であることが示唆されている。得られた研究結果は、論文投稿し採択（査読あり）された。
    COVID-19の感染拡大により、当初計画をしていた半構成面接法を中止し、量的横断研究へと研究方法を柔軟に変更していくが、研究当初の目的からは外れてはいない。次年度の向けた準備期間として、有意義であった。

    We considered proposing a nursing support model for patients with primary amenorrhea associated with disorders of sex development (DSD). we qualitatively and inductively clarified the process and structure of psychological fluctuations, including the patient's gender identity, toward adaptation. Among women, the process of becoming free from obsession over their condition after receiving a diagnosis of primary amenorrhea associated with DSD, is characterized by “regaining confidence in being a woman.” Early education and support for families (particularly parents) are also required. Therefore appropriate and professional nursing support is needed for the reconstruction of gender identity.

    The purpose of this study was to develop and validate a scale that can quantify and evaluate information on appetite in children with cancer undergoing chemotherapy. After examining the items and accuracy of the scale, we introduced levels from hunger to satiety, and also designed a scale that includes three-dimensional effects and entertainment elements. The validity of the developed wooden Visual Analog Scale (VAS) was verified with children with cancer and their parents. Nine groups (18 subjects) of children with cancer aged 4-9 years and their parents or guardians undergoing treatment were tested before and after lunch, and their appetite and food intake, interviews, VAS values, and card-type psychometric scales were used for correlation and comparison tests. The results suggested that the developed scale was valid.

    We determined effect of preoperative psychological preparation of the children who underwent epilepsy surgery and were intended to evaluate the effect. We planned a preoperative anxiety evaluation using mYPAS standard, but there were the cancellation of the surgery and visit restrictions to a hospital under the influence of Covid-19, and the accomplishment of the study was difficult. Thus, we changed study contents and focused about thought for the surgery of the children of mother. This is because an opinion and the manner of the parent greatly influence it for decision making in the treatment participation in children.We analyzed interview data about the preoperative explanation to children for mother. As a result, three themes were found. "Heavy responsibility of the will of a proxy decision of the surgery" "Pains to explanation of the surgery to children" "Earnest desire to normal growth and normal life"

    In order to train nurses how to provide explanations to pediatric epileptic patients as psychological preparation for surgery, it is essential to develop a preparatory training text that is highly versatile and able to be modified to suit individual patients. In the present study, we developed a training text by visualizing the conscious and subconscious information that nurses include in their explanations to patients as psychological preparation for surgery. We categorized the information that nurses wanted to communicate to pediatric patients into six categories. Our findings indicated that nurses studied the information beforehand in an attempt to curb the patient’s anxiety. Based on our findings, we developed a lift-the-flap textbook.
    Our study findings also revealed that the mothers of these patients felt the same way as the nurses about curbing their anxiety regarding surgery. In future research, we will continue to investigate and assess this surgery preparation tool.

    A purpose of this study is to get basics document to examine nursing help to the feelings coaching of the family for a child needing long-term medical management. For mother whom the study method went with a hospitalization child and supported continuously, We used a Semi-structural interview.The results are as follows.Mother explained a fact to child about disease and treatment for the medical act with the pain. And the core of the feelings coaching of mother was to do `making of manner' not to evade for an affected child.
