    特別支援教育では知的障害者の適応行動の困難さの背景要因である実行機能に着目し,様々な支援が行われている。本研究は,施設通所者の口腔保健行動を支援するために通所者の口腔保健状態,実行機能と実行機能からみた歯磨き行動を把握することおよび歯磨き行動質問紙の適用可能性を検討することを目的とした。通所型障害者福祉施設2施設の通所者24名[男性18名,女性6名,平均年齢(標準偏差):34.4(12.0)]を対象とし,日本歯科医師会の標準的な成人歯科健診質問紙票(一部改変)と独自に作成した実行機能に基づく歯磨き行動の質問紙(Executive function-based questionnaire of toothbrushing behavior:EF-TB),実行機能の質問紙(Dysexecutive Questionnaire:DEX,Executive Functions Questionnaire:EFQ)による調査を行った。家族からの回答結果では多くの対象者はかかりつけ歯科医をもち,定期歯科健診を受け,就寝前に歯を磨き,フッ化物入り歯磨剤を使い,歯科専門職による歯磨き指導の経験があった一方で,甘い飲食物の間食は95.8%,フロス・歯間ブラシの使用は16.7%,歯磨き時の出血・歯肉の腫れは37.5%であり,その結果として対象者の75.0%は実技指導が必要であると判定された。DEX,EFQ,EF-TBの総合点の平均点(標準偏差)はそれぞれ,36.13(14.33),64.0(8.29),71.13(21.56)であった。EF-TBはモニタリング,シフティング,プランニング・ワーキングメモリで弱さを示し,それらを基とした口腔保健行動支援の必要性が明らかになった。さらに,DEXとEFQ,EFQとEF-TBはそれぞれ,中程度の相関関係[ρ=-0.423,ρ=0.556]が認められ(p<0.05),EF-TBの適用可能性が示唆された。福祉施設に通う知的障害者は,実技指導を中心とした歯科保健指導の必要性と実行機能の弱さが示唆され,歯科専門職はそれらに配慮した歯科保健指導の実施と口腔保健行動の変容を支援することが求められる。(著者抄録)

    Objectives: (i) To assess comprehension of oral care-related terms among caregivers and nurses working at long-term care facilities, using a newly developed test
    (ii) to analyse the effect of participant characteristics on their comprehension. Background: Effective mutual communication between dental professionals and caregivers/nurses is essential for providing information on daily oral care for institutionalised elders. Methods: A 36-item word-knowledge test in Japanese was developed to assess comprehension of oral care-related terms. The test was administered to a convenience sample of 236 nursing staff (198 caregivers and 38 nurses) at six long-term care facilities in Niigata City, Japan, and its reliability and validity were verified. Associations of participant characteristics with their responses were investigated by multiple regression analysis. Results: Mean percentage of correct responses (accuracy rate) for nursing staff was approximately 62% (highest for oral care products and lowest for prosthodontic terms). Test internal reliability was high (Cronbach's alpha &gt
    0.8). Concurrent validity (test ability to distinguish between characteristically different groups) was confirmed. Mean accuracy rate was significantly higher among nurses (78.5 ± 19.3%) than among caregivers (58.7 ± 22.8%), and among respondents with interest in oral care (64.2 ± 21.1%) than among those with no such interest (51.5 ± 28.9%). Conclusion: The word-knowledge test was valid and reliable for nursing staff of six long-term care facilities in Niigata City. Their comprehension was low for perioral and intraoral structures, related symptom and disease names, and prosthodontics terms related to oral care. Understanding of oral care-related terms among the nursing staff was related to their occupation and interest in oral care.

    At present, long-term care benefits of oral intake and oral care management for residents of long-term care facilities are evaluated in publicly provided long-term care insurance system, but it cannot be said that they have spread sufficiently to these facilities.
    Therefore, we conducted a survey to clarify the overall cost/benefit of participation of dental professionals for oral intake and oral care management among residents of long-term care facilities. As a result, the average number of hospitalization days per resident due to pneumonia was reduced by about 40% at the facilities which provided oral care by dental Hygienists. And it is estimated that the profit of the facility has increased by about 120,000 yen, even if expenses increase such as labor costs for dental hygienists.

    Long-term Care insurance(LTCI) fees were revised in April 2015, and the revision focused on support of oral ingestion and oral health of residents in LTCI facilities. To investigate claim rates and estimated billing amounts for these benefits; cooperation between LTCI facilities and oral health professionals(OHPs), questionnaire surveys were carried out to all LTCI facilities in a Prefecture in Japan
    Benefits for transition from tube feeding to oral ingestion, and maintenance of oral ingestion were claimed in about 10% to 40% of facilities. Benefits for “oral health management system” and oral health care by dental hygienists were claimed in about 70% and 20%. Employment of OHPs by LTCI facilities was were strongly associated with claims for these benefits, and billing amount was about 62,000 JPY/month higher significantly for facilities that employed OHPs.
