    <b>Abstract</b> The mirror technique (MT) is a useful skill for operating in difficult-to-see areas and is considered to be an essential basic dental skill. However, the acquisition of MT is not easy for inexperienced learners. Therefore, the authors aimed to develop an effective method for teaching MT for inexperienced dentists and described the drilling of dental caries using MT in their first and second reports. In contrast to the previous report focusing on trainee dentists (TDs), in the present study the same experiment was conducted as in the second report on instructor dentists (IDs). The proficiency in cutting accuracy was examined by comparing experienced IDs with inexperienced TDs.

     Using the same conditions as in the first and second reports, the excessive cutting range, largest cutting depth, mean cutting depth, number of checking times and cutting time were used as the evaluation parameters. The proficiency in cutting accuracy was examined by comparing IDs with TDs on the designated four cutting areas (incisal, cervical, mesial and distal) and finger rest (FR) position.

     As a result, regarding each of the excessive cutting range, largest cutting depth and mean cutting depth, IDs showed superior cutting skills compared with TDs. In addition, regarding FR for the upper right canine, IDs checked the drilling cavity more frequently than TDs. Thus, it was revealed that IDs cut more carefully than TDs under difficult drilling conditions.

     It is suggested that the proficiency of MT skills improves through clinical experience, and the position of FR affects the cutting accuracy. Furthermore, improved MT skills would cover minor errors of FR position to some extent under these experimental conditions.

    <b>Abstract</b> Dental procedures are performed in the small oral cavity, therefore the mirror technique (MT) is considered to be an indispensable basic skill for dentists, especially in cases where it is impossible to check directly. However, because of the difficulty of acquiring MT skills, the authors have examined the positions of the dental mirror (DM) and finger rest (FR), with the aim of developing an effective teaching method for inexperienced dentists, and published the results in the first and second reports. Whereas the previous reports focused on trainee dentists (TDs), in the present investigation the same experiment as the first one was conducted with instructor dentists (IDs) who supervised TDs, and the level of proficiency in cutting accuracy was examined by comparing the IDs with the TDs.

    Background/Objectives: Appropriate use of a dental mirror (MT) is essential for dental treatment. However, because the mirror image differs depending on the operator's position, there are few effective tools available to train dental students and inexperienced dentists. To address this issue, a new virtual reality (VR) system was developed. Methods: Eighty-seven participants, including instructor dentists, graduate students, trainee dentists, and dental students from Niigata University, participated in the experiment. The participants manipulated the virtual dental mirror (DM) connected to three-dimensional (3D) control devices while wearing a head-mounted display (HMD) to simulate an intraoral examination and caries removal. The distance between the center of the DM and the caries, area ratio of the caries on the DM, ellipticity of the caries image, and manipulation time were measured and compared between groups. Results: The mean distance between the centers was 1.18 mm for instructor dentists and 1.56 mm for the students (p = 0.003; effect size, 0.48). Instructor dentists demonstrated significantly superior performance in all evaluation categories compared to the other groups. This suggests that the level of clinical experience affected the results. Conclusions: The newly developed VR system can quantitatively and objectively evaluate DM manipulation. Differences in the operator's clinical experience led to variations in the techniques used for mirror image confirmation and DM manipulation. This VR system enables quantitative feedback by outputting images from the HMD to a monitor. Therefore, it has the potential to evolve into an unprecedented training tool for dental students or trainees.

    Background/Objectives: Chronic craniofacial inflammation is recognized as a factor in anxiety-like behaviors, yet effective therapeutic options remain limited. Agmatine, a dietary bioactive compound found in fermented foods such as sake lees, exhibits modulatory effects on neural functions, alleviating psychological distress like anxiety associated with local inflammation. Methods: We investigated both the therapeutic and preventive effects of agmatine on anxiety-like behaviors and the related neural basis in a mouse model of persistent craniofacial inflammation induced by complete Freund’s adjuvant (CFA). Results: Comprehensive behavioral assessments, including the elevated plus maze, open field, dark–light box, social interaction, and novel object recognition tests, revealed that therapeutic agmatine administration (1.0 and 30 mg/kg) significantly reduced CFA-induced anxiety-like behaviors, with the higher dose showing more robust and sustained effects across multiple time points. These behavioral improvements were paralleled by reductions in acetylated histone H3, FosB, and c-Fos expression in key anxiety-related brain regions, suggesting a reversal of craniofacial inflammation-associated neural changes. In contrast, preventive agmatine treatment exerted modest and time-dependent behavioral benefits with minimal molecular normalization. Notably, preventive agmatine did not affect general locomotor activity (indicated by total movement distance), indicating that its anxiolytic effects were not confounded by altered locomotor activity. Metabolomic analysis confirmed the presence of agmatine in sake lees (~0.37 mM), supporting the hypothesis that fermented food products might offer dietary routes to emotional resilience. Conclusions: These findings underscore agmatine’s promise as a context-specific epigenetic modulator capable of mitigating anxiety-like behaviors by normalizing inflammation-driven molecular dysregulation in the brain.

    This study examined the effects of treadmill running (TR) regimens on craniofacial pain- and anxiety-like behaviors, as well as their effects on neural changes in specific brain regions of male mice subjected to repeated social defeat stress (SDS) for 10 days. Behavioral and immunohistochemical experiments were conducted to evaluate the impact of TR regimens on SDS-related those behaviors, as well as epigenetic and neural activity markers in the anterior cingulate cortex (ACC), insular cortex (IC), rostral ventromedial medulla (RVM), and cervical spinal dorsal horn (C2). Behavioral responses were quantified using multiple tests, while immunohistochemistry measured histone H3 acetylation, histone deacetylases (HDAC1, HDAC2), and neural activity markers (FosB and phosphorylated cAMP response element-binding protein (pCREB). The effects of both short-term TR (2 days, TR2) and long-term TR (10 days, TR10) regimens were conducted. TR10 significantly reduced anxiety- and formalin-evoked craniofacial pain-like behaviors in SDS mice. It normalized SDS-induced increases in histone H3 acetylation in both the anterior and posterior portions of the ACC, as well as the anterior portion of the IC. These inhibitory effects were also observed in SDS-related increases in HDAC1, FosB, and pCREB expression. Additionally, TR10 normalized increased histone H3 acetylation in the RVM and C2 regions, with specific effects on FosB and pCREB expression observed in the C2 region. In contrast, TR2 showed limited effects on craniofacial pain-like behaviors but reduced anxiety-like behaviors in SDS mice. Under sham conditions, TR2 had minimal impact on histone H3 acetylation. Paradoxically, TR2 increased formalin-evoked craniofacial pain-like behaviors during the early phase despite not altering acetylated histone H3 expression. In conclusion, the TR10 regimen is effective in attenuating SDS-induced craniofacial pain- and anxiety-like behaviors, likely by normalizing epigenetic modifications and neural activity in key brain regions.

    Aims: Dental procedures involve intricate techniques that can be challenging to visualize, often hindering imitation and assessment. In this study, the impression-taking performance of abutment teeth was quantitatively analyzed to create objective indexes for dental skill education.Methods: Participants were divided into two groups of different clinical experience levels: 10 dentists and 10 dental students. An aluminum model of abutment teeth was employed to simplify the experiment. An optical motion capture system (VICON, Oxford, UK) measured participants’ movements. The impression accuracy, time length on the analyzed section, impression material pouring speed, total amount of impression material used, and syringe tip trajectory were evaluated. Fisher’s exact test and Mann–Whitney U test were used to compare the two groups’ results (?=0.05).Results: In the dentist group, there were few apparent failures and a high impression accuracy. The amount of impression material dispensed tended to be larger in the dentist group, with longer practice time and slower syringe movement speed. This suggested that the inexperienced participants were sufficiently unable to pour out the impression material. The syringe tip trajectories were not significantly different between the groups. An instructor’s advice is often limited to abstract feedback; therefore, specific suggestions might assist in effective skill education.Conclusion: It is possible to quantitatively analyze impression-taking performance and provide helpful information for dental skill education by using this system.

    This narrative review provides an overview of current knowledge on the impact of nutritional strategies on chronic craniofacial pain associated with temporomandibular disorders (TMDs). Individuals experiencing painful TMDs alter their dietary habits, avoiding certain foods, possibly due to chewing difficulties, which might lead to nutrient deficiencies. Our literature investigation revealed that the causal links between nutritional changes and craniofacial pain remain unclear. However, clinical and preclinical studies suggest that nutraceuticals, including vitamins, minerals, polyphenols, omega-3 fatty acids, isoprenoids, carotenoids, lectins, polysaccharides, glucosamines, and palmitoylethanolamides, could have beneficial effects on managing TMDs. This is described in 12 clinical and 38 preclinical articles since 2000. Clinical articles discussed the roles of vitamins, minerals, glucosamine, and palmitoylethanolamides. The other nutraceuticals were assessed solely in preclinical studies, using TMD models, mostly craniofacial inflammatory rodents, with 36 of the 38 articles published since 2013. Our investigation indicates that current evidence is insufficient to assess the efficacy of these nutraceuticals. However, the existing data suggest potential for therapeutic intervention in TMDs. Further support from longitudinal and randomized controlled studies and well-designed preclinical investigations is necessary to evaluate the efficacy of each nutraceutical intervention and understand their underlying mechanisms in TMDs.

    The conditions of stress contagion are induced in bystanders without direct experiences of stressful events. This study determined the effects of stress contagion on masseter muscle nociception in mice. Stress contagion was developed in the bystanders after cohabitating with a conspecific mouse subjected to social defeat stress for 10 days. On Day 11, stress contagion increased anxiety- and orofacial inflammatory pain-like behaviors. The c-Fos and FosB immunoreactivities evoked by masseter muscle stimulation were increased in the upper cervical spinal cord, while c-Fos expressions were increased in the rostral ventromedial medulla, including the lateral paragigantocellular reticular nucleus and nucleus raphe magnus in stress contagion mice. The level of serotonin in the rostral ventromedial medulla was increased under stress contagion, while the number of serotonin positive cells was increased in the lateral paragigantocellular reticular nucleus. Stress contagion increased c-Fos and FosB expressions in the anterior cingulate cortex and insular cortex, both of which were positively correlated with orofacial inflammatory pain-like behaviors. The level of brain-derived neurotrophic factor was increased in the insular cortex under stress contagion. These results indicate that stress contagion can cause neural changes in the brain, resulting in increased masseter muscle nociception, as seen in social defeat stress mice.

    <b>Abstract</b> The Faculty of Dentistry, Niigata University has provided the program “Early Clinical Practice Ⅱ,” which includes mini-lectures concerning basic science and clinical site visiting, alternating with group discussions or presentations on the relationship between the basic and clinical aspects of the practice for third-year students. This study aimed to verify the effectiveness of this program. Eight-seven students who participated in the program in 2021 and 2022 were recruited for a questionnaire survey and 47 responses were analyzed. The results of the questionnaire showed that many students in both years responded that this program was meaningful, and that the number of students in each group or clinical departments to be visited was appropriate. Regarding infection control, which is essential for healthcare professionals, all students answered that they understood the lecture given at the initial guidance. The clinical departments that were relatively specialized or included hands-on training, as well as the basic science fields that were easily imagined to be directly related to clinical practice, tended to be selected as the fields of interest of the students. The results indicated that this program is useful in providing students with an understanding of the relationship between basic and clinical medicine. It is necessary to refine the program to raise students’ motivation to learn and encourage further cooperation with basic and clinical faculty members.

    We investigated the effects of social defeat stress (SDS) and treadmill running on masseter muscle nociception, which was quantified by the orofacial formalin test and c-Fos and FosB immunoreactivities in the upper cervical spinal cord (C1/C2) region in male mice. After daily SDS or non-SDS conditioning for 10 days, SDS-conditioned mice were categorized into SDS-susceptible versus resilient mice. Several mice, including non-SDS-conditioned, SDS-susceptible, and resilient mice, were selected to assess masseter muscle nociception on Day 11. SDS conditioning for 10 days increased masseter muscle-evoked nocifensive behaviors and c-Fos and FosB expression in SDS-susceptible compared to non-SDS and resilient mice. The remaining SDS-susceptible and non-SDS mice were subjected to an additional 10 days of SDS plus treadmill running or sedentary sessions before assessing masseter muscle nociception on Day 21. Daily treadmill running sessions reduced enhanced masseter muscle nociception in SDS-susceptible mice but not in non-SDS mice. The preventive effects of daily treadmill running immediately after each SDS conditioning for 10 days on orofacial nocifensive behaviors were assessed on Day 11. Treadmill running conducted immediately after daily SDS inhibited enhanced orofacial nocifensive behaviors. These findings indicate that repeated SDS increases masseter muscle nociception, which could be prevented by daily treadmill running exercise.

    平成9〜14年にインプラント外来を受診した新患症例と,そのうちインプラント手術を行った症例を対象に,現状の把握を目的として臨床統計的検討を行った.インプラント外来開設以降の新来患者数の年次推移は年々増加し,現在までに新来患者数は211名(男66名,女145名)で,50歳代の女性が30%を占めた.新患登録患者の初診時における訴えを複数回答で求めたところ,咬合が85.8%,審美が27%,発音が9.5%で,殆どの患者が咬合状態の改善を訴えて来院していた.新来患者の211例中,現在までにインプラント手術を行った症例は41例,今後1年以内に手術予定の30症例を加えると,インプラント手術の適応と判断された症例は71症例であった.インプラントの手術に際してFGG,GBR,MTMなど前処置を行った件数を年度別にみると,ここ2〜3年では急激に増加した

    This review highlights the regeneration of gingival oral epithelium and neural elements incident to dental implantation. The regeneration process of nerve fibers can be divided into the three following stages; 1) an epithelial healing stage; 2) a completion stage of epithelial regeneration; and 3) a stage of osseointegration establishment. Prior to gradient disappearance of inflammatory reaction around the epithelium, the nerve fibers begin to regenerate in the healing peri-implant epithelium. After completion of epithelial healing, the peri-implant epithelium exhibits the same neural density and terminal morphology as the intact junctional epithelium. These regenerated nerve fibers show a beaded-in-appearance, suggesting that these fibers contain neuropeptides including calcitonin gene-related peptide and substance P which have diverse functions. These findings indicate that the peri-implant epithelium share some structural and functional features with normal junctional epithelium, although these two epithelia have different origins.

    患者が抱える全人的な問題を正確に発見する，すなわち診る能力は医師・歯科医師にとって必須であり，歯科医師として「診る」ためには「見る」技術の習得が必要不可欠である。しかしながら歯科治療は口腔内という狭小な空間で行われる専門的な治療であり，その作業空間には歯だけではなく，粘膜や舌また場合によっては唾液や血液により術者にとっても視野を確保することは容易ではない。当然通常の治療見学から学修者である歯学生がそこから得る情報は限られたものであり，模倣するには不十分である。そこで本研究では術者目線の教材を作成し，その教材を学修者に見せることで「見える」ことを体感させ，その後の学修者の「見る」能力に生じる変化についての解析を行う。
    専門的機械的歯面清掃（PMTC）は熟練した歯科医師，歯科衛生士が機械的清掃器具を用いて全ての歯面からプラークを取り除く処置である。適切にPMTCを実施するには全ての歯面に対して適切に器具を当てること，全ての歯面を見る技術が必要とされる。そこでプラークを模す色素を塗布した模型をマネキンに装着し，被験者にPMTCを実施させたところPMTC後にも色素が残存している部位があることが確認された。残存した部位を被験者に確認させたのち，清掃器具の取り扱いについての技術的な指導は行わず再度その部分の清掃を行わせたところ概ねプラークを模した色素を除去することができたことから色素を残存させたのは見る技術の不足が一因である可能性が高いことが確認された。

    Dental treatment has the major characteristic of using a variety of instruments and materials, which gives dentists a craftsman-like aspect, and each has skills that they acquire through their own experience. However, in current society, facing the shortening of time and increasing needs regarding quality assurance, dentists are losing the margin to wait for perfection as they did in the past. In this study, focusing on the adjustment of dentures, which is particularly difficult for inexperienced dentists among the frequently performed general dentistry procedures. And the investigation was performed by using as teaching materials the magnitude of force during the procedure and the points that experienced dentists examine, which cannot be conveyed to learners in the usual way.

    Faculty members of dental colleges have recently tried to improve the technical education and have strived to enhance their clinical clerkship systems.
    A portfolio has been known as to be a useful formative evaluation tool, and it can chronologically record the cumulative learning process. Therefore, a web-based e-portfolio system including an assessment rubric for students and instructors was invented and applied to the clinical clerkship. The general clinical skill evaluation level that was assessed and marked on the system by the instructors had a tendency of gradually advancing with the progress of the clinical clerkship term, and it was suggested that the clinical skills of the students improved with their own clinical experiences. The web-based e-portfolio system on the clinical clerkship was considered to be a meaningful tool for reviewing the performance of students and facilitating effective training by the instructors.

    The purpose of this study was to establish a simulation model to predict bone tissue changes by occlusal force by means of 3-D finite element analysis. We established rat implant occlusion model first. Then, we used micro CT to have digital data of 3-D structure of the implant with surrounding bone tissue. Using 3-D finite element analysis program, we applied occlusal force to the implant and seek some correlation between the force distributions and bone tissue changes investigated by histological sections. As a result, we found that there were some correlations between those and it was shown that the bone tissue changes upon occlusal loading could be predicted by 3-D finite element analysis. The precision should be verified by further investigation yet.

    The purpose of this research was to establish the animal experiment model for the investigation of periodontal ligament response to the dynamic stress, and to find mechano-receptor in the periodontal ligament cells that convert a physical stress of occlusal load to a biochemical signal.
    In 2004, we tried to establish an animal model. Through bonding 1 mm in diameter wire to the rat upper molar, histological changes in periodontal ligament could be investigated that was simulating occlusal trauma. As a result, the time that the periodontal ligament converted from the destruction process to the regenerative process was able to be retrieved in detail.
    In addition, we selected several kinds of proteins related to the bone, and immunohistochemistry was done using the antibody to these. As a result, the production of Osteopontin was up regulated in osteoclasts. Then we tried to find RANKL production because it is thought to be working as the upstream cytokine, but we could not. This means there might be a specific pathway through which Osteopontin production is controlled.
    In 2005 and 2006, we investigated histological changes of periodontal ligament under no loading condition. Because loading machine was not available, we selected the temperature stimulus instead of mechanical stimulation. As a result, the common feature of a temperature and mechanical stimulation cannot be found. Further study is required to compare these more precisely.
    In 2007, we tried to establish animal model that enables us to investigate thermal influences to the periodontal ligament, but we could not achieve because we could not fabricate a handy device for this purpose. Further study is necessary.

    The purpose of the present study was to clarify the relationship between the chronological changes of immunocompetent cells and the expression of heat shock protein(Hsp) 25 in the process of pulpal regeneration after tooth injury in rat molars by immunocytochemistry for Hsp 25 and class II major histocompatibility complex(MHC) antigen. An intense Hsp 25-immunoreactivity was found in the differentiated odontoblasts. Both cavity preparation and tooth replantation caused the degeneration of the odontoblast layer to result in the loss of Hsp 25-immunoreactions in the suffered dental pulp at the early stages after tooth injury. Numerous class II MHC-positive cells appeared along the pulp-dentin border and extend their cell processes into the dentinal tubules at 12-24 hours after cavity preparation and 3 days after tooth replantation. Newly differentiated odontoblast-like cells with Hsp 25-immunoreactivity were arranged at the pulp-dentin border, and the class II MHC-positive cells retreated towards the subodontoblastic layer by postoperative 3-5 days after tooth injury. Thus, the common cellular events occur during pulpal regeneration following two different experimental injuries. These findings indicate that the time course of changes in the expression of Hsp 25-immunoreactivity reflects the degeneration/regeneration process of odontoblasts and that the temporal appearance of the class II MHC-positive cells at the pulp-dentin border suggests their participation in odontoblast differentiation as well as in initial defense reactions during the pulpal regeneration process. In the case of laser ablation, on the other hand, distinct abscess formation consisting of polymorphonuclear leukocytes was found in the dental pulp by 3-5 days postoperation. The penetration of masses of oral bacteria was recognizable in the dentinal tubules beneath the prepared cavity. These findings indicate that cavity preparation by laser ablation induces remarkable inflammation by continuous bacterial infections via dentinal tubules.

    1. Finding on the survival of neuronal cells on titanium plate
    By adding nerve growth factor in culture medium, PC12 cells, neuronal cells, could extend their cytoplasmic processes on titanium plate. However, these cells easily exfoliated from the surface of titanium plate. These findings suggest that nerve fibers cannot adhere to titanium.
    2. Tissue response to titanium implantation in rat maxilla
    Tissue response to titanium implantation was investigated in an animal model using maxilla by use of eletron microscopy and histochemical technique. The experimental data indicated that ossification proceeded at different modes around the titanium implant in rat maxilla, depending on the nature of the recipient bones and the dimension of the gap between the implant and osteotomy margin.
    3. Regeneration of nerve fibers in the per-implant epitelium.
    The response of nerve fibers in the peri-implant epithelium to titanium implantation was investigated with an experimental model using rat maxilla and immunohistochemical techniques. The experimental data indicated that the peri-implant epithelium showed the same innervation to that in normal junctional epithelium, and that the intraepitelial nerve fibers in the peri-implant epithelium might have diverse functions which have been suggested in the literature.
    4. Relationship between the surface conditon of implant and bone formation process.
    Tissue responses to titanium implantation with two different surface conditions in our established implantation model in rat maxillae were investigated by light and transmission electron microscopy and by histochemistry for tartrate resistant acid phosphatase activity. We here used two types of implants with different surface qualities : titanium implants sandblasted with A1203 (SA-group), and implants coated with hydroxyapatite (HA-group). Our data indicated that ossification around the titanium implants progressed in differ ent patterns, probably dependent on surface properties and quality.
